Evaluation of acute cervical sPinal cord injury with diffusion tensor imaging

Abstract:Objective To evaluate the diagnostic value of diffusion tensor imaging(DTI) in acute cervical spinal cord injury at 3.0T MRI. Method Routine MRI and DTI were performed in 21 cases of acute cervical spinal cord injury and 16 cases of healthy volunteers. The value of fractional anisotropy (FA) and apparent diffusion coefficient (ADC) were retrospectively measured in the region of interests (ROI) of the case group and the control group. The differences in the value of ADC and FA between the two groups were evaluated and three dimensional fiber tractography (FT) changes were compared. Result There were statistically significant differences(P<0.05) in the value of ADC and FA between the case group and the healthy control group. In MRI-negative cases, the average ADC value was (1.15±0.27)× 10-3mm2/s, higher than that in the control group, and the average FA value was (0.58±0.13), lower than that in the control group;While in MRI-positive cases, the average ADC value was (1.35±0.34)×10-3mm2/s, much higher than that in the control group and the average FA value was (0.52±0.06), much lower than that in the control group;as is showed clearly in FT, the white matter fibre tracts in the injured area were swelling and sparse, and part of them were interrupted and displaced.Conclusion Compared with routine MRI, DTI may be more sensitive in the diagnosis of acute traumatic cervical spinal cord injury and may be helpful in assessing the severity of spinal cord injury. ADC and FA changes may be important in forecasting the prognosis of cervical spinal cord injury.
