CT features of acinar cell carcinoma of pancreas

Abstract:Objective: To analyze the CT findings of the acinar cell carcinoma(ACC) of pancreas, so as to improve the understanding of the disease. Methods: CT findings of 6 patients with histologically confirmed pancreatic ACC were re-viewed retrospectively. CT features of these 6 ACC cases were analyzed with review of the literatures. Results: All 6 cases had single lesion. The tumors were located in the uncinate (n=2), neck (n=1), body (n=1), and tail(n=2) of pancreas. The aver-age tumor size was 4.0 cm (range from 2.5 to 4.9 cm). Two cases were ovoid and the others were irregular. Three lesions were well circumscribed and 3 lesions had a complete or incomplete capsule.One lesion was solid-cystic and the others were solid. Four of the 6 tumors were exophytic and the other 2 tumors were intraparenchymal. One tumor showed iso-and low density on the non-contrast enhanced CT, the others were isodense masses. On contrast-enhanced scan, all tumors showed mild and moderate enhancement in arterial phase and persistent enhancement in portal venous phase. Two cases showed pancreatic parenchymal atrophy. Pancreatic and biliary ductal dilatation was showed in 1 patient. Two patients had splenic artery inva-sion. Malignant peripancreatic lymph nodes were identified in 2 patients. None of the lesions showed any intratumoral calci-fication, intratumoral hemorrhage and distant metastasis of liver. Conclusions:The characteristics of CT images of ACC in-clude exophytic growth, capsule, isodense solid component of tumor on non-contrast scan, mildly or moderately persistent enhancement on contrast scan, and the absence of pancreatic and biliary ductal dilatation. Combination of CT findings men-tioned above and the elevated serum AFP are useful for improving the accuracy of diagnosis of ACC.
