Application of modified Bentall technique in aortic root surgery

Abstract:Objective To analyze the clinical application of modified Bentall technique in patients with proximal ascending aortic lesions, and provide the data about clinical therapeutic effects and experience of aortic root surgery. Methods The patients (n=24) with aortic sinus dilatation (diameter>50 mm) complicated by moderate or severe aortic valve lesions were chosen from Department of Cardiologic Surgery in Anhui Provincial Hospital of Anhui Medical University from Oct. 2015 to Dec. 2017. All patients were divided, according to random digital table, into classical Bentall group and modified Bentall group (each n=12). The classical Bentall group was given classical Bentall procedure, and modified Bentall group was given, on the base of classical Bentall procedure, the procedure of directly anastomosing aortic wall and artificial blood vessel around the coronary artery opening without releasing free coronary artery opening and with windowing at the coronary artery opening corresponding to artificial blood vessel by electric cautery pen. In addition, the original aortic wall was retained, and drainage was performed from aorta to right atrium outside artificial blood vessel. The artificial vessel was wrapped with original aortic wall. Results The time of cardiopulmonary bypass (CPB), bleeding volume, aortic occlusion duration, ventilator assistance time, ICU duration and postoperation days of hospital stay were higher in classical Bentall group than those in modified Bentall group (P<0.05). The differences in the total volume of drainage, LVEF and left ventricular end-diastolic diameter had no statistical significance between classical Bentall group and modified Bentall group (P>0.05) after the procedure. Conclusion The modified Bentall technique, compared with classical Bentall technique, can reduce bleeding, shorten procedure time, decrease ventilator assistance time, ICU duration and postoperation days of hospital stay and improve stationarity after the procedure. It is worthy of clinic popularization and application.
