The effect of blood pressure variability on cognitive function affer ischemic stroke

Abstract:Objective To study the association between blood pressure variability and PSCI in acute ischemic stroke (AIS) patients.Methods Seven hundred and eighty-six patients with first ever AIS admitted to our hospital within 24 h after stroke onset were included in this study.Their blood pressure variation coefficient (BPVC) within 7 days were calculated.The patients were divided into SBPV Q1 (n=158,4.5-7.7),SBPV Q2 (n=157,7.8-8.1),SBPV Q3 (n=157,8.2-8.5),SBPV Q4 (n=157,8.6-9.2),SBPV Q5 (n=157,9.3-15.1),and into DBPV Q1 (n=158,3.7--6.9),DBPV Q2 (n=157,7.0--7.5),DBPV Q3 (n=157,7.6--8.2),DBPV Q4 (n=157,8.3 -8.8),DBPV Q5 (n=157,8.9-14.6).The relationship between blood pressure variability within 7 days of stroke onset and the incidence of PSCI at 3 months after stroke onset was analyzed by multivariate logistic regression analysis.Results The incidence of PSCI was significantly higher on day 14,at months 3,6 and 12 after stroke onset than at admission (30.2%,72.0%,62.0%,30.3% vs 19.2%,P<0.05),and in SBPV Q2-Q5 groups than in SBPV Q1 group at month 3 after stroke onset (72.8%,74.6%,74.8%,78.5% vs 59.4%,P<0.01).The risk to develop PSCI was higher in Q2-Q5 groups than in Q1 group at 3 months after stroke onset (OR=1.91,2.20,2.07and 2.88,P<0.05).Conclusion SBPV in AIS is an independent risk factor for PSCI and can significantly affect the visual space,executive ability,naming and delayed rccall of ischemic stroke patients at 3 months after stroke onset.
