Abstract:As the population ages, the incidences of age-related diseases such as chronic obstructive pulmonary disease(COPD)and sarcopenia are on the rise.A growing number of studies have found that there is a close relationship between COPD and sarcopenia.Quadriceps weakness, atrophy, and fiber type conversion of skeletal muscles are common in patients with COPD.At the same time, respiratory muscles also have adaptive changes, resulting in limited mobility, decreased quality of life and increased hospitalization rates and mortality.However, mechanisms underlying the interaction between COPD and sarcopenia have not been fully illuminated.Moreover, clinicians usually fail to make early identification and start intervention for COPD-related skeletal muscle dysfunction.This article reviews progress on the epidemiology, mechanisms linking the two conditions and treatment of COPD-related sarcopenia as a reference for clinical practice.

Abstract:PURPOSE: The authors determined the degree of respiratory and peripheral muscle weakness in patients with moderate to severe chronic obstructive pulmonary disease (COPD). Differences in severity of muscle weakness among muscle groups may provide treatment options, such as selective muscle training, to adapt the exercise prescription in pulmonary rehabilitation programs. In addition, this information may add to the knowledge on the mechanisms of muscle weakness.METHODS: Respiratory and peripheral muscle force were quantified in 22 healthy elderly subjects and 40 consecutive COPD patients (forced expiratory volume in 1 second, percent of predicted value [% pred] 41 +/- 19; transfer factor for carbon monoxide, % pred 47 +/- 26) admitted to a pulmonary rehabilitation program. Lung function, diffusing capacity, isometric force of four peripheral muscle groups (handgrip, elbow flexion, shoulder abduction, and knee extension), neck flexion force, and maximal inspiratory and expiratory pressures were measured.RESULTS: Patients had reduced respiratory muscle strength (mean 64% of control subjects' value [% control]) and peripheral muscle strength (mean 75% control) compared to normal subjects. Inspiratory muscle strength (59 +/- 18% control) was significantly lower than expiratory muscle strength (69 +/- 25% control) and peripheral muscle strength (P < 0.01). Neck flexion force (80 +/- 19% control) was better preserved than maximal inspiratory pressure and shoulder abduction force (70 +/- 15% control, P < 0.01). Handgrip force (78 +/- 16% control) and elbow flexion force (78 +/- 14% control) were significantly less affected than shoulder abduction force (70 +/- 15% control, P < 0.01). Finally, shoulder abduction force and knee-extension force (72 +/- 24% control) were not significantly different.CONCLUSIONS: Muscle weakness in stable COPD patients does not affect all muscles to a similar extent. Inspiratory muscle force is affected more than peripheral muscle force, whereas proximal upper limb muscle strength was impaired more than distal upper limb muscle strength.

Abstract:Chronic obstructive pulmonary disease (COPD) is a global health problem.Malnutrition is a common complication of COPD.Malnutrition affects lung function,quality of life and prognosis in COPD patients,and is also an independent risk factor for mortality.Through nutritional screening and assessment,nutritional status and metabolic phenotype can be determined.Proper nutritional intervention may improve the quality of life and prognosis of COPD patients.

Abstract:BACKGROUND Low body weight in patients with chronic obstructive pulmonary disease (COPD) is associated with an impaired pulmonary status, reduced diaphragmatic mass, lower exercise capacity and higher mortality rate when compared to adequately nourished individuals with this disease. Nutritional support may therefore be a useful part of their comprehensive care. OBJECTIVES To conduct a systematic review of randomised controlled trials (RCTs) to clarify whether nutritional supplementation (caloric supplementation for at least 2 weeks) improved anthropometric measures, pulmonary function, respiratory muscle strength and functional exercise capacity in patients with stable COPD. SEARCH STRATEGY Randomized controlled trials (RCTs) were identified from the Cochrane Airways Group register of RCTs, a hand-search of abstracts presented at international meetings and consultation with experts. Searches are current as of March 2004. SELECTION CRITERIA Two reviewers independently selected trials for inclusion, assessed quality and extracted the data. DATA COLLECTION AND ANALYSIS Within each trial and for each outcome, we calculated an effect size. The effect sizes were then pooled by a random-effects model. Homogeneity among the effect sizes was also tested. MAIN RESULTS Eleven studies recruiting 352 participants met the inclusion criteria. Eight papers were considered as high quality. Two studies were double-blinded. For each of the outcomes studied, the effect of nutritional support was small: the 95% confidence intervals around the pooled effect sizes all included zero. The effect of nutritional support was homogeneous across studies. AUTHORS' CONCLUSIONS Nutritional support had no significant effect on anthropometric measures, lung function or exercise capacity in patients with stable COPD.

Abstract:Objective To investigate the nutritional status in elderly patients with benign respiratory disease and assess the correlation between nutritional status and clinical outcomes.Methods A prospective study of the elderly patients with benign respiratory disease,aged 65 years and over,from April 2012 to May 2012,was conducted.Mini-Nutritional Assessment-Short Form (MNA-SF) was used to assess the nutritional status.Associations between nutritional status and clinical outcomes were analyzed.Results A total of 1269 patients who met inclusion criteria were enrolled in this study,with a mean age of (74.0±6.5) years,a mean body mass index of(23.3±3.8) kg/m2,a mean grip strength of (16.7±17.2) kg,a mean upper arm circumference of(25.7±3.7) cm,a mean calf circumference of (32.3±3.8)cm.The grip strength declined with the increase of age.1 224 subjects completed the MNA-SF,among whom 145 cases (11.8％) suffered from malnutrition,411 cases (33.6％) were at a risk of malnutrition,668 cases (54.6％) were well nourished.The incidence of malnutrition and the risk of malnutrition were increased with ageing.People with malnutrition had a higher mortality,longer hospital stay and more hospital costs (all P＜0.01).Conclusions Elderly patients with benign respiratory disease have a higher prevalence of malnutrition and a higher risk of malnutrition,which may lead to worse clinical outcomes.More attention should be paid to the assessment of nutritional status during hospitalization.

Abstract:To explore the therapeutic effect of aerobic exercise nursing plans based on target heart rate in cardiopulmonary exercise tests on male patients with chronic obstructive pulmonary disease. This study recruited 90 male patients with chronic obstructive pulmonary disease who met specific screening criteria and were evenly divided into a control group and an experimental group based on a random number table. The control group received respiratory training based on the target heart rate in the cardiopulmonary exercise test, while the experimental group received aerobic exercise based on the target heart rate in the cardiopulmonary exercise test. Both groups received 12 weeks of exercise each. Cardiopulmonary function tests showed that peak oxygen consumption (peak VO2), anaerobic threshold (AT), forced expiratory volume in the first second (FEV1), forced vital capacity (FVC), and the FEV1/FVC ratio in the test group were significantly higher than those in the control group (p < 0.05); furthermore, the carbon dioxide ventilation equivalent (VE/VCO2) slope was significantly lower than that in the control group (p < 0.05). The 6MWT of the test group was significantly higher than that in the control group (p < 0.05). The CAT score, Borg score and scores related to the symptoms, activities, and impacts related to quality-of-life were significantly reduced after intervention (p < 0.05). Aerobic exercise based on cardiopulmonary exercise testing can improve the health status, quality of life and prognosis of COPD patients.

Abstract:Objective:The purpose of this study was to conduct a Meta-analysis to assess the influence of physical therapy and respiratory muscle training on rehabilitation of patients with stable chronic obstructive pulmonary disease. To provide a basis for better rehabilitation exercise for patients, families, communities and hospitals.Methods:According to the literature search strategy, the relative literatures of randomized controlled trials (RCTS) of physical therapy and respiratory muscle training on rehabilitation of patients with chronic obstructive pulmonary disease were searched in Cochrane Library、MEDLINE、Embase、CINAHL、CBM、Wan Fang、CNKI and VIP from January 1990 to October 2017. Languages are limited to Chinese and English.The literatures were screened according to the predefined inclusion and exclusion criteria by two people . Cochrane Handbook for Systematic Reviews of Interventions Version 5.1.0 were used to assess the quality of articles. The data were analyzed by Revman5.3.Results:A total of 7 articles were finally included, among of these 5 of which were written in English , Others were written in Chinese .They included 7 studies of randomized controlled trial and 267 patients with chronic obstructive pulmonary disease .7 studies show that treadmill exercise and respiratory muscle exercise are the most common rehabilitation methods.7 articles result of Meta-analysis showed that there were statistical differences between physical therapy and respiratory muscle training and other therapies in 6MWD、mMRC and FEV 1%;More research is needed on the impact of other outcome measures. Conclusions:Physical therapy and respiratory muscle training is a reliable way of rehabilitation, it to the stabilization of COPD patients FEV 1 % of expected value, activity ability, the influence of the degree of difficulty in breathing has a positive role.It is not stable for the positive effect of FVC and FEV 1 .However, the effects of other outcome measures were not determined.

Abstract:Objective: To study the effect of the muscle strength exercise on knee osteoarthritic patients with muscle weakness. Method: The isokinetic parameters of pre and post muscle strength exercise were compared on 26 involved limbs and the parameters of pre and post passive motion on 19 involved limbs as well. Result: Although muscle strength increased in both groups, muscle strength exercise group was more successful on reducing muscle weakness. Conclusion: Muscle strength exercise can reduce muscle inhibition of osteoarthritic patients through recruitment.

Abstract:Mechanically ventilated patients are at high risk for malnutrition, and it is now accepted that nutrition can influence the respiratory function. In particular, malnutrition can adversely affect lung function and the adverse effects of such malnutrition include: decreased ventilatory drive, decreased respiratory muscle function, alterations of lung parenchyma and depressed lung defense mechanisms. Therefore, nutrition support should be considered if a patients has a severe chronic pulmonary disease or an acute respiratory disease. Recent studies showed that malnourished patients have a reduced respiratory muscle strength and that nutritional intervention can return muscle ventilatory function to normal levels. Furthermore, it seems very likely that the ventilatory drive can be influenced by dietary intake of amino acids and glucose. The structure of the pulmonary parenchyma can be affected by starvation and the pulmonary defense mechanisms are depressed in malnourished patients. The incidence of post-operative pneumonia or atelectasis is higher in protein-depleted patients. in comparison with well-nourished patients. In conclusion, the importance of nutrition support in the management of patients with respiratory failure, particularly those mechanically ventilated, is stressed in the paper.

A Study of Respiratory Muscle Strength in Patients with Chronic Obstructive Pulmonary Disease

Abstract:Purpose In order to assess respiratory muscle strength and central inspiratory drive and their relationship to lung function tests in patients with chronic obstructive pulmonary disease (COPD). Methods We measured in 66 patients with chronic obstructive pulmonary disease the maximum inspiratory and expiratory pressure (Pimax, Pemax, cm H2O% pred) , Inspiratory pressure after 0.1 sec. at tidal breathing (P0.1) , lung function and arterial gas analysis respectively. Results COPD patients showed significantly lower Pimax and Pemax and higher P0.1 than normal subjects. The FRC/TLC% ,RV% pred,FRC% pred and FEV1 were significantly correlated with Pimax and P0.1. Among them, Pimax was the most closely correlated with FRC/TLC% ( r = - 0.581 P 0.001). There were no relationship between FEV1, FRC/TLC% and Pemax. PaO2 and PaCO2 were only correlated with Pimax. No relationship between PaO2, PaCO2 and Pemax, P0.1.TLC% pred was not correlated with Pimax,Pemax and P0.1. Conclusions There is respiratory muscle strength dysfunction in COPD patients. Hyperinflation and airway obstruction predominantly affected inspiratory muscle strength in a group of COPD patients.
Effects of Exercise Intervention on Peripheral Skeletal Muscle in Stable Patients With COPD: A Systematic Review and Meta-Analysis

Abstract:Objectives: Peripheral skeletal muscle dysfunction is an important extrapulmonary manifestation of chronic obstructive pulmonary disease (COPD) that can be counteracted by exercise training. This study aimed to review the effect of three major exercise training modalities, which are used in pulmonary rehabilitation to improve on skeletal muscle mass, function, and exercise capacity in COPD. Methods: PubMed, Embase, EBSCO, Web of Science, and the PEDro database were searched on April 25, 2020. Only randomized controlled studies published in English evaluating the effects of exercise interventions on peripheral skeletal muscle mass, strength, and exercise capacity in stable COPD patients were included. The quality of included studies was evaluated using the PEDro scale. The mean difference (MD) or the standardized mean difference (SMD) with 95% CI was calculated to summarize the results. Subgroup meta-analysis was used to investigate the effects of different exercise training modalities and different outcome measures. The Grading of Recommendations Assessment, Development, and Evaluation guidelines were used to rate evidence quality. Results: A total of 30 randomized controlled trials involving 1,317 participants were included. Data from trials investigating endurance exercise (EE), resistance exercise (RE), and combined aerobic and resistance exercise (CE) were pooled into a meta-analysis, and the differences compared with the non-exercising COPD control were improvement in the muscle strength and exercise capacity in stable COPD patients. Subgroup meta-analysis for different exercise training modalities showed that RE significantly improved muscle strength (SMD = 0.6, 95% CI 0.35–0.84, I 2 = 61%), EE and CE significantly increased VO 2peak (EE: MD = 3.5, 95% CI 1.1–5.91, I 2 = 92%; CE: MD = 1.66, 95% CI 0.22–3.1, I 2 = 1%). Subgroup meta-analysis for different outcome measures showed that only isotonic strength was improved after exercise interventions (SMD = 0.89, 95% CI 0.51–1.26, I 2 = 71%). Conclusion: Moderate evidence supports that exercise training in stable COPD patients has meaningful and beneficial effects on peripheral skeletal muscle strength and exercise capacity. Peripheral skeletal muscle shows a higher response to RE, and the isotonic test is relatively sensitive in reflecting muscle strength changes. The proportion of aerobic and resistance exercise components in a combined exercise program still needs exploration. Abstract:Chronic obstructive pulmonary disease (COPD) associates with several extra-pulmonary effects. Muscle dysfunction and wasting is one of the most prominent extra-pulmonary effects and contributes to exercise limitation and health related quality of life (HRQoL), morbidity as well as mortality. The loss of muscle mass is characterised by an impaired balance between protein synthesis (anabolism) and protein breakdown (catabolism) which relates to nutritional disturbances, muscle disuse and the presence of a systemic inflammation, among other factors. Current approaches to reverse skeletal muscle dysfunction and wasting attain only modest improvements. The development of new therapeutic strategies aiming at improving skeletal muscle dysfunction and wasting are needed. This requires a better understanding of the underlying molecular pathways responsible for these abnormalities. In this review we update recent research on protein metabolism, nutritional depletion as well as physical (in)activity in relation to muscle wasting and dysfunction in patients with COPD. We also discuss the role of nutritional supplementation and exercise training as strategies to re-establish the disrupted balance of protein metabolism in the muscle of patients with COPD. Future areas of research and clinical practice directions are also addressed.

Abstract:Objective To investigate the improvement of arterial blood gases and pulmonary function of senile COPD patients by respiration muscle training.MethodsForty five super-senior COPD patients over 90-year old were trained to practice abdominal respiration and contractile lip respiration between May 2008 and January 2009.Their arterial blood gases and pulmonary function data were analyzed.ResultsThere were significant improvements in FEV1,FEV1/FVC,PaCO2 and PaO2(P<0.05) after training.ConclusionRespiratory muscle training can effectively improve the arterial blood gases and pulmonary function of super-senior COPD patients.

Abstract:Aims: The aim of this study was to investigate peripheral muscle oxygenation in patients with chronic obstructive pulmonary disease (COPD) at rest, during submaximal exercise, and during recovery, and to determine the effects of disease stage on peripheral muscle oxygenation. Methods: Of the 35 stable COPD patients (62.49±8.45 years), 18 patients in GOLD 1 and 2 were assigned to Group 1 and 17 patients in GOLD 3 and 4 were assigned to Group 2. Dyspnea perception of the patients was evaluated with the Modified Medical Research Council (mMRC) Dyspnea Scale, severity of the disease affecting daily life was evaluated with the COPD Assessment Test (CAT-COPD Assessment Test), respiratory function was evaluated with the Pulmonary Function Test, and quadriceps muscle strength was evaluated with a manual muscle testing device. Muscle oxygenation of the patients was measured with Near-infrared spectroscopy (NIRS) for 5 minutes at rest, 6 minutes during the 6-Minute Walk Test (6-MWT), and 5 minutes during recovery after the end of the test. The results of the two groups were compared. Enhancing effect of nutritional support of L-arginine on patients with chronic obstructive pulmonary disease

Abstract:Objective To evaluate the enhancing effect of nutritional support of L-arginine on patients with chronic obstructive pulmonary disease (COPD). Methods 30 patients were randomly divided into 2 groups and observed: 12 patients in L-arginine group and 18 patients in control group. It took ten days to have the nutritional support which total calorie intake is 1.5 times of REE, while patients in L-arginine group orally took additional L-arginine 10g tid. Results Compared with control group, the nutritional parameters such as TSF, PALB and FM improved, maximal respiratory muscle strength increased and REE decreased significantly in L-arginine group. Conclusions L- arginine was helpful in the nutritional support for patients with chronic obstructive pulmonary disease, because it could enhance effects of nutritional support and reduce their energy consumption significantly and improve their nutritional status and prognosis.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background</h3><p>How respiratory muscle strength influences the effectiveness of pulmonary rehabilitation (PR) in patients with chronic obstructive pulmonary disease (COPD) is unclear.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Objective</h3><p>To investigate the benefits of PR in subjects with COPD according to respiratory muscle strength.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>Ninety-seven subjects with COPD were evaluated using maximal inspiratory pressure (MIP), maximal expiratory pressure (MEP), pulmonary function tests, the cardiopulmonary exercise test (CPET), and the St. George's Respiratory Questionnaire (SGRQ). Subjects were divided into four groups: 1 (normal MIP and MEP); 2 (low MIP); 3 (low MEP); and 4 (low MIP and MEP). Subjects underwent PR for 3 months; MIP, MEP, SGRQ, and CPET were evaluated post-PR.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>Subjects with both poor MIP and MEP had the highest dyspnea score, lowest exercise capacity, and poorest health-related quality of life (HRQoL). PR improved exercise capacity and HRQoL in all groups, with more improvement in MIP, MEP, tidal volume (on exercise), and dyspnea (at rest) in subjects with both low MIP and MEP.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3><p>Patients with respiratory muscle weakness had worse dyspnea, lower exercise capacity, and poorer HRQoL at baseline. Exercise training improved respiratory muscle strength with concurrent improvement of exercise capacity, HRQoL, and dyspnea score. Subjects with both poor baseline MIP and MEP showed greater benefits of PR.</p>

Abstract:The objective of this study was to probe the effect of comprehensive nutritional nursing on elderly patients with chronic obstructive pulmonary disease. To this end, a total of 82 elderly patients with chronic obstructive pulmonary disease were randomly divided equally into control (receiving conventional treatment) and nursing intervention (receiving conventional treatment and dietary nursing intervention) groups. The body mass index, forced expiratory volume in one second/forced vital capacity, plasma albumin, prealbumin, and hemoglobin levels in the observation group were higher in comparison with the control group. However, the self-rating anxiety scale and self-rating depression scale scores in the observation group were diminished compared to the control group. The scores of psychological, physiological, environmental, and social relations in the observation group were higher relative to the control group. The nursing satisfaction in the observation group was elevated compared to the control group. In conclusion, comprehensive nutritional nursing interventions can improve nutritional status, lung function, and quality of life in elderly patients with chronic obstructive pulmonary disease.

Abstract:ObjectiveTo investigate the effect of different nutritional intervention on muscle strength and content in patients with sarcopenia.MethodsA total of 91 patients with sarcopenia were collected and randomly divided into three groups(full nutritional products group,education+protein group,education group). The weight,grip,calf circumference,pace,5th-ups were compared before research,intervene for 2 months and 3 months.ResultsThe weight,grip,calf circumference,pace,5th-ups comparison in three groups had no difference (F=1.07,1.57,1.14,0.26,0.87,1.64,1.35,1.50,0.48,0.79,0.15,1.34,0.75,0.07,0.44,P>0.05). After 2 and 3 months of intervention,the pace and 5th-ups of full nutritional products group were significantly higher than before intervention(P<0.05),the pace of education+protein group were significantly higher than before intervention and 5th-ups were lower than before intervention(P<0.05). The limbs and trunk muscles were no significant differences in three groups(F=0.44,0.56,0.02,0.01,P>0.05),also before and after intervention were no significant differences(t=1.12,-0.29,-0.03,0.02,-1.84,-0.75,P>0.05).ConclusionAppropriate increase in nutritionally complete formula powder and protein powder can improve less pace,and time-ups five times in sarcopenia ,so suggest to appropriate supplemental energy and protein intake in elderly with sarcopenia on the basis of the diet.
