The disparity in depression status between the urban and rural elderly based on Fairlie decomposition method

Abstract:Objective:To analyze the disparity of depression status between the urban and rural elderly, and to investigate the contribution rate of each factor to this difference by applying Fairlie decomposition method.Methods:Data from 1 075 participants, aged 65 years and above, of the China Health and Retirement Longitudinal Study(CHARLS, 2015)were analyzed.The disparity in depression status between the urban and rural elderly was analyzed by using state14.0 software.Results:Of 1075 subjects, the depression rate was 28.8%, which was lower in urban elderly than in rural elderly(23.5% or 92/392 vs.31.9% or 218/683, χ2=8.66, P<0.01). Multivariate unconditional logistic regression analysis demonstrated four aspects as follows.The depression status was poorer in the urban elderly who had poor self-evaluation memory, discontent of life, pains and ADL impairment.It was better in the urban elderly who had moderate savings and daily sleep time between 5.5 h and 7.0 h. The depression status was poorer in the rural elderly who were female, unsatisfied with life, and had poor self-evaluation memory, pains and ADL impairment.It was better in the rural elderly with four-week illness.Based on Fairlie decomposition method, the differences in depression status between the urban and rural elderly were mainly affected by pains, ADL impairment, savings level, life satisfaction degree, the existing of four-week illness and sleep abnormality ( P<0.05), with the contribution rates of 39.38%, 30.10%, 26.60%, 12.78%, 10.93% and 8.04%, respectively. Conclusions:There are differences in the depression status between the urban and rural elderly.The depression statue is less severe in the urban elderly than in the rural elderly.Physical health and economic level are correlated with the disparity of depression status between the urban and rural elderly.
