Investigating of the prognostic value of premature ventricular complexes in patients with acute heart failure

Abstract:Objective:To investigate the clinical value of premature ventricular complexes(PVCs) in long-term prognosis and risk stratification of patients with acute heart failure(AHF).Method:A total of 230 patients with AHF were enrolled into the registry from March 2012 to February 2015.The primary end-point was all cause mortality within 18 months.The patients were divided into death group and non-death group according to the endpoint event.The effects of clinical indicators and PVCs on the long-term prognosis of patients with AHF were assessed.Result:The number of PVCs(P=0.005),and multifocal PVCs(P=0.03) were associated with the risk of mortality in patients with AHF in 18 months,while non sustained ventricular tachycardia (NSVT) has no significant effection.In Cox regression analysis,the number of PVCs(HR 1.193,P=0.04),NT-proBNP,serum sodium and systolic blood pressure in acute phase were independent predictors of the all-cause mortality in patients with AHF.The receiver operator characteristics(ROC) curve showed that the AUC of the number of PVCs was 0.642,Cutoff was 1 193 beats/24h,sensitivity was 53.7% and the specificity was 73.6%.Kaplan-Meier Analyses found that group of patients with frequent multifocal PVCs were at high risk of death.Conclusion:In patients with AHF,the number of PVCs in acute stage has potential clinical value as a predictor of long-term prognosis and risk stratification.
