Abstract:Ebola virus causes a severe haemorrhagic fever in humans with high case fatality and significant epidemic potential. The 2013–2016 outbreak in West Africa was unprecedented in scale, being larger than all previous outbreaks combined, with 28 646 reported cases and 11 323 reported deaths. It was also unique in its geographical distribution and multicountry spread. It is vital that the lessons learned from the world's largest Ebola outbreak are not lost. This article aims to provide a detailed description of the evolution of the outbreak. We contextualize this outbreak in relation to previous Ebola outbreaks and outline the theories regarding its origins and emergence. The outbreak is described by country, in chronological order, including epidemiological parameters and implementation of outbreak containment strategies. We then summarize the factors that led to rapid and extensive propagation, as well as highlight the key successes, failures and lessons learned from this outbreak and the response. Bibliometric Analysis of Ebola Research Indexed in Web of Science and Scopus (2010-2020)

Abstract:Background. Within the past decade, Africa has faced several recurrent outbreaks of Ebola virus disease (EVD), including the 2014-2016 outbreak in West Africa and the recent 2018-2020 Kivu outbreak in the Democratic Republic of Congo. The study thus aimed at quantifying and mapping the scientific output of EVD research published within 2010-2020 though a bibliometric perspective. Methods. EVD-related publications from 2010 to 2020 were retrieved from the Web of Science (WoS) and Scopus databases by using the keywords ‘Ebola’, ‘Ebola Virus Disease’, ‘Ebolas’, and ‘ebolavirus’. Biblioshiny software (using R-studio cloud) was used to categorise and evaluate authors’, countries’ and journals’ contribution. VOSviewer was used for network visualisation. Results. According to the used search strategy, a total of 3865 and 3848 EVD documents were published in WoS and Scopus, respectively. The average citation per document was 16.1 (WoS) and 16.3 (Scopus). The results show an overall increase in the publication trend within the study period. The leading countries in EVD research were the USA and UK, with over 100 papers in both databases, including Nigeria and South Africa. NIAID and CDC-USA were the most influential institutions, while “Infectious Diseases” and “Medicine” were the most decisive research fields. The most contributing authors included Feldmann H and Qiu XG with over 60 papers in each database, while Journal of Infectious Diseases was the most crucial journal. The most cited article was from Aylward et al. published in 2014, while recent years displayed a keyword focus on “double-blind”, “efficacy”, “ring vaccination” and “drug effect”. Conclusion. This bibliometric analysis provides an updated historical perspective of progress in EVD research and has highlighted the role played by various stakeholders. However, the contribution of African countries and institutions is not sufficiently reflected, implying a need for increased funding and focus on EVD research for effective prevention and control.
A Historical Review of Ebola Outbreaks

Abstract:Ebola Virus Disease (EVD) is a severe, often fatal illness in humans caused by the Ebola virus. Since the first case was identified in 1976, there have been 36 documented outbreaks with the worst and most publicized recorded in 2014 which ravaged three West African Countries, Guinea, Liberia and Serial Leone. The West African outbreak recorded 28,616 human cases, 11,310 deaths (CFR: 57–59%) and left about 17,000 survivors, many of whom have to grapple with Post-Ebola syndrome. Historically, ZEBOV has the highest virulence. Providing a historical perspective which highlights key challenges and progress made toward detecting and responding to EVD is a key to charting a path towards stronger resilience against the disease. There have been remarkable shifts in diagnostics, at risk populations, impact on health systems and response approaches. The health sector continues to gain global experiences about EVD which has shaped preparedness, prevention, detection, diagnostics, response, and recovery strategies. This has brought about the need for stronger collaboration between international organizations and seemingly Ebola endemic countries in the areas of improving disease surveillance, strengthening health systems, development and establishment of early warning systems, improving the capacity of local laboratories and trainings for health workers.

Abstract:Background: Ebola Hemorrhagic Fever (EHF) has become well known all over the world, especially following the West African outbreak in Guinea, Sierra Leone and Liberia (December 2013).The Ebola virus was first discovered in the Democratic Republic of Congo (DRC), an African country that has continued to register Ebola outbreaks.This study aims to summarize old and new experiences of Ebola in the DRC, in order to propose strategies for better prevention.Materials and Methods: Information was taken from databases such as PubMed and Cochrane library.A total of eleven full text and three abstracts were identified for the data extraction.Results: Since its discovery in the DRC, there have been seven Ebola outbreaks, accounting for a total of 1032 cases and 795 deaths.The presence of Non-Human Primates, also considered as the natural reservoir and susceptible host of Ebola virus, can be one major factor that has contributed to the increased number of Ebola outbreaks and cases in the Equatorial region.The existence of rumors and legends related to Ebola in DRC obscure the the viral nature of the disease, and lead to difficulty for health workers, to easily accomplish their tasks. Conclusion:It is important to scale up community education campaigns designed to give more details on the viral nature of the EHF, establish national agencies and institutions specialized in controlling hunting in the Equatorial region, for better prevention, since there is not yet a specific drug or vaccine to the Ebola Virus.
Forty years of the war against Ebola

Abstract:Humans have been fighting against the Ebola virus disease (EVD) since its first outbreak in 1976 in southern Sudan and Yambuku in Zaire which lies on the Ebola River. According to the data from the World Health Organization (WHO, 2014b), the first outbreak claimed 431 lives in 1976, and the disease awoke transiently in Sudan three years later and then disappeared for 15 years afterwards. Following that, large outbreaks appeared in 1995 in Zaire with 250 deaths of people, 2001-2002 in Uganda with 224 deaths, 2002-2003 in Congo with 128 deaths, and 2007 in Congo with 187 deaths. In 2014, the most severe and complicated outbreak swept through the West African countries having already taken 1069 lives, with the situation seeming to be out of control. To date, there have been 15 outbreaks in Africa, which have caused 4362 infected cases and claimed 2659 lives. The pandemics of Ebola show obvious independence from any season. Humans are generally susceptible to the Ebola virus without gender or age variation. The natural reservoir of the Ebola virus still remains unclear. During the past 40 years or so, the EVD disappeared after an outbreak in one region and erupted in another region without any warning. The difficulty in understanding the spreading pattern of Ebola was compared to that of the wave-particle duality of light.
Chinese Public Attention to the Outbreak of Ebola in West Africa: Evidence from the Online Big Data Platform

Abstract:Objective: The outbreak of the Ebola epidemic in West Africa in 2014 exerted enormous global public reaction via the Internet and social media. This study aimed to investigate and evaluate the public reaction to Ebola in China and identify the primitive correlation between possible influence factors caused by the outbreak of Ebola in West Africa and Chinese public attention via Internet surveillance. Methods: Baidu Index (BDI) and Sina Micro Index (SMI) were collected from their official websites, and the disease-related data were recorded from the websites of the World Health Organization (WHO), U.S. Centers for Disease Control and Prevention (CDC), and U.S. National Ministries of Health. The average BDI of Internet users in different regions were calculated to identify the public reaction to the Ebola outbreak. Spearman's rank correlation was used to check the relationship of epidemic trends with BDI and SMI. Additionally, spatio-temporal analysis and autocorrelation analysis were performed to detect the clustered areas with the high attention to the topic of "Ebola". The related news reports were collected from authoritative websites to identify potential patterns. Results: The BDI and the SMI for "Ebola" showed a similar fluctuating trend with a correlation coefficient = 0.9 (p < 0.05). The average BDI in Beijing, Tibet, and Shanghai was higher than other cities. However, the disease-related indicators did not identify potential correlation with both indices above. A hotspot area was detected in Tibet by local autocorrelation analysis. The most likely cluster identified by spatiotemporal cluster analysis was in the northeast regions of China with the relative risk (RR) of 2.26 (p ≤ 0.01) from 30 July to 14 August in 2014. Qualitative analysis indicated that negative news could lead to a continuous increase of the public's attention until the appearance of a positive news report. Conclusions: Confronted with the risk of cross-border transmission of the infectious disease, online surveillance might be used as an innovative approach to perform public communication and health education through examining the public's reaction and attitude.

Abstract:Ebola disease (EBOD) remains a significant and ongoing threat to African countries, characterized by a mortality rate of 25% to 90% in patients with high viral load and significant transmissibility. The most recent outbreak, reported in Uganda in September 2022, was declared officially over in January 2023. However, it was caused by the Sudan Ebola virus (SUDV), a culprit species not previously reported for a decade. Since its discovery in 1976, the management of EBOD has primarily relied on supportive care. Following the devastating outbreak in West Africa from 2014 to 2016 secondary to the Zaire Ebola virus (EBOV), where over 28,000 lives were lost, dedicated efforts to find effective therapeutic agents have resulted in considerable progress in treating and preventing disease secondary to EBOV. Notably, 2 monoclonal antibodies—Ebanga and a cocktail of monoclonal antibodies, called Inmazeb—received Food and Drug Administration (FDA) approval in 2020. Additionally, multiple vaccines have been approved for EBOD prevention by various regulatory bodies, with Ervebo, a recombinant vesicular stomatitis virus-vectored vaccine against EBOV being the first vaccine to receive approval by the FDA in 2019. This review covers the key signs and symptoms of EBOD, its modes of transmission, and the principles guiding supportive care. Furthermore, it explores recent advancements in treating and preventing EBOD, highlighting the unique properties of each therapeutic agent and the ongoing progress in discovering new treatments.

Abstract:The 2013–2016 epidemic of Ebola virus disease in West Africa was of unprecedented magnitude and changed our perspective on this lethal but sporadically emerging virus. This outbreak also marked the beginning of large-scale real-time molecular epidemiology. Here, we show how evolutionary analyses of Ebola virus genome sequences provided key insights into virus origins, evolution and spread during the epidemic. We provide basic scientists, epidemiologists, medical practitioners and other outbreak responders with an enhanced understanding of the utility and limitations of pathogen genomic sequencing. This will be crucially important in our attempts to track and control future infectious disease outbreaks.

Abstract:Ebola epidemic in Africa was outbreak again in 2014 that arises a new wave of panic worldwide. There is a high mortality rates (30-70%) among Ebola infected people in virus-stricken areas. The direct communications between affected countries and rest of world were cut off then. But the medical capabilities against this deadly viral disease were provided by a limited number of agents, several patented agents, biotherapies or prophylactic/therapeutic vaccines have been entering into clinical validations and will be further developed into the global markets—including patents of small molecular chemicals, short sequences or oligomers of DNA/RNA, linkages of chemicals with bio-molecules, herbal medicine and so on. The possible mechanisms of action of these therapeutic options are underway. The multiple characters of Ebola infections—its origin, pathologic progresses, genomic changes, therapeutic context and economic considerations on the field of Ebola studies are outlined in this review. Finally great differences can be expected after these researches.

Abstract:As we all know, the Ebola virus is one of the most dangerous pathogens in the world. The outbreak of the Ebola virus diseases, began in December 2013, Guinea, West Africa, was the most serious one ever recorded in the history of mankind. Due to underestimation of the epidemic and the lack of effective drug, the effect of the prevention and control of this epidemic was not optimistic. ZMapp and TKM-Ebola were emergently approved by FDA for the treatment of the Ebola virus disease. However, the signiifcance of the use of two drugs was limited because of the unknown efifcacy, safety and the shortage of production capacity. In this paper, the existing therapies and the latest research progress of new drugs for Ebola virus diseases are reviewed so as to help the relevant persons get this information.

Abstract:Background: The 2014 Ebola virus disease (EVD) outbreak remains unprecedented both in the number of cases, deaths and geographic scope. The first case of EVD was confirmed in Lagos Nigeria on 23 July 2014 and spread to involve 19 laboratory-confirmed EVD cases. The EVD cases were not limited to Lagos State as Rivers State recorded 2 confirmed cases of EVD with 1 out of the 2 dying. Swift implementation of public health measures were sufficient to forestall a country -wide spread of this dreaded disease. This exploratory formative research describes the events of the Nigeria Ebola crisis in 2014. Methods: This research was implemented through key informant in-depth interviews involving 15 stakeholders in the EVD outbreak in Nigeria by a team of two or three interviewers. Most of the interviews were conducted face-to-face at the various offices of the respondents and others were via the telephone. The interviews which lasted an hour on average were conducted in English, digitally recorded and notes were also taken. Results: This study elucidated the public health response to the Ebola outbreak led by Lagos State Government in conjunction with the Federal Ministry of Health. The principal strategy was an incident management approach which saw them identify and successfully follow up 894 contacts. The infected EVD cases were quarantined and treated. The Nigerian private sector and international organizations made significant contributions to the control efforts. Public health enlightenment programmes using multimodal communication strategies were rapidly deployed. Water and sanitary facilities were provided in many public schools in Lagos. Conclusions: The 2014 Ebola outbreak in Nigeria was effectively controlled using the incident management approach with massive support provided by the private sector and international community. Eight of the confirmed cases of EVD in Nigeria eventually died (case fatality rate of 42.1%) and twelve were nursed back to good health. On October 20 2014 Nigeria was declared fee of EVD by the World Health Organization. The Nigerian EVD experience provides valuable insights to guide reforms of African health systems in preparation for future infectious diseases outbreaks.

Abstract:Introduction: The 2014/2015 West African Ebola Virus Disease (EVD) outbreak attracted global attention. Numerous opinions claimed that the global response was impaired, in part because, the EVD research was neglected, although quantitative or qualitative studies did not exist. Our objective was to analyse how the EVD research landscape evolved by exploring the existing research network and its communities before and during the outbreak in West Africa. Methods/ principal findings: Social network analysis (SNA) was used to analyse collaborations between institutions named by co-authors as affiliations in publications on EVD. Bibliometric data of publications on EVD between 1976 and 2015 was collected from Thomson Reuters' Web of Science Core Collection (WoS). Freely available software was used for network analysis at a global-level and for 10-year periods. The networks are presented as undirected-weighted graphs. Rankings by degree and betweenness were calculated to identify central and powerful network positions; modularity function was used to identify research communities. Overall 4,587 publications were identified, of which 2,528 were original research articles. Those yielded 1,644 authors' affiliated institutions and 9,907 connections for co-authorship network construction. The majority of institutions were from the USA, Canada and Europe. Collaborations with research partners on the African continent did exist, but less frequently. Around six highly connected organisations in the network were identified with powerful and broker positions. Network characteristics varied widely among the 10-year periods and evolved from 30 to 1,489 institutions and 60 to 9,176 connections respectively. Most influential actors are from public or governmental institutions whereas private sector actors, in particular the pharmaceutical industry, are largely absent. Conclusion/ significance: Research output on EVD has increased over time and surged during the 2014/2015 outbreak. The overall EVD research network is organised around a few key actors, signalling a concentration of expertise but leaving room for increased cooperation with other institutions especially from affected countries. Finding innovative ways to maintain support for these pivotal actors while steering the global EVD research network towards an agenda driven by agreed, prioritized needs and finding ways to better integrate currently peripheral and newer expertise may accelerate the translation of research into the development of necessary live saving products for EVD ahead of the next outbreak.

Abstract:BackgroundThe ongoing West African Ebola epidemic began in December 2013 in Guinea, probably from a single zoonotic introduction. As a result of ineffective initial control efforts, an Ebola outbreak of unprecedented scale emerged. As of 4 May 2015, it had resulted in more than 19,000 probable and confirmed Ebola cases, mainly in Guinea (3,529), Liberia (5,343), and Sierra Leone (10,746). Here, we present analyses of data collected during the outbreak identifying drivers of transmission and highlighting areas where control could be improved.
Recurrent outbreaks of Ebola Virus Disease in Africa: A Meta-analysis of case fatality rates

Abstract:Abstract Background: In the last decade, Africa has witnessed several outbreaks of Ebola virus disease (EVD), each presenting with varying case fatality rate (CFR) and other socio-economic impacts. This study aims to summarise the CFR and identify potential factors that influenced the severity of EVD outbreaks in Africa.Methods: This was a systematic review and meta-analysis of EVD outbreaks published between January 2010 and March 2020, using Web of Science, Scopus and PubMed databases. Only English articles and reports, including the number of cases and deaths during the outbreak in Africa, were considered. Quality of the included articles was assessed using the Murad’s quality assessment tool. The analysis was conducted using Stata (version 12), pooled effect sizes were calculated using the random-effects model and heterogeneity was tested for using the I2 statistic.Result: Thirteen studies with 32,300 cases and 13,727 deaths were identified whose pooled CFR was 60% (95% CI: 47-73%). The most EVD-affected countries were DRC with 5 outbreaks and a pooled CFR of 65% (95% CI: 59-71%), followed by Uganda with 3 outbreaks and CFR= 83% (95% CI: 60-99%). Zaire ebolavirus caused the most outbreaks (10), with a CFR= 58% (95% CI: 45-71%). Besides, outbreaks with less than 1000 cases reported a higher CFR rate compared to those with more cases.Conclusion: The study has revealed a considerably high CFR caused by the recurrent EVD outbreaks in Africa. It also notes an implementation gap of the prevention and control strategies, and thus identifies a need to strengthen the surveillance systems and response mechanisms to enable early detection and prompt control of future outbreaks.

Abstract:The 2014 West African EbolaOutbreak raisedfrom Sierra Leone, Guinea, and Liberia at December in 2013 has been reported to cause 21296 cases and 8429 deaths until now which became the deadliest recorded in history. In this paper, we proposed the riskanalysis to assess the international spread risk from mentioned three African countries to China by GEM(Global Epidemic Mobility) Model. As another part of analysis, we crawled related online social media data of Ebola from the most four favorite online social networks (including SINA, TENCENT) in China from June to November in 2014. By analyzing these attained social media data and airline data of GEM, we found some interesting results. For example, Beijing has the most importing risk of Ebola while it has the hottest discussion on social network.. Furthermore, we showed analysis of combining social network data with geographicaldemonstration and Chinese citizen sentiment towards this disaster.
Fear, Stigma and Uncertainty: The short and long-term effects of Ebola on survivors, affected families, and community in Bundibugyo, Western Uganda

Abstract:In 2014, Uganda was identified as a high-risk country for Ebola Virus Disease (EVD), with a series of outbreaks recorded since 2000. In 2007, the second outbreak in Bundibugyo district resulted in 149 reported cases and 37 confirmed Ebola deaths. Through the outbreak response, a new strain of the Ebola virus (Bundibugyo ebolavirus) was discovered. Although much is known about the nature of Ebola, including disease signs and symptoms, transmission and management, there is limited understanding of the short and long-term sociocultural impacts of the disease in communities. The study team conducted a focused ethnography in Bundibugyo District 10 years after the 2007 outbreak. Data collection included a review of archival data, participant observation, field notes and 19 in-depth interviews with survivors and affected families. Results underscored time-limited social, cultural and economic disruptions caused by the outbreak. We interpreted findings through an Eco-health framework with an emphasis on ways that underlying stigma accentuated detrimental long-term effects of the outbreak. Affected women, particularly widows, experienced social exclusion, and economic strain, and acknowledged loss of opportunity for a better life for their orphaned children. Deepening fear of the possible recurrence of Ebola resulted in ethnic tension driven by speculations on the 2007 outbreak source. Survivors reported varying persistent health effects including impaired vision and general body weakness. Community members reported positive changes in health seeking behaviors. Health care workers described high levels of alert for early clinical signs of Ebola, a critical factor for early outbreak detection at the community level. Our findings can inform future Ebola response and recovery interventions, particularly those targeting community re-integration and a mitigation of the fear and stigma associated with survivorship.
