Clinical study on the changes of serum pepsinogen in chronic gastric lesions

Abstract:Objective To investigate the expression of serum pepsinogen ( PG) in patients with different chronic gastric diseases, and proved helps in the clinical diagnosis of digestive disease. Methods Three hundred and seventy-two hospitalized patients were included in the Renmin Hospital of Wuhan University from Jul. 2015 to Apr. 2017, inclu-ding 312 patients with stomach diseases and 60 patients without stomach diseases ( normal controls) . These major disea-ses were divided into 5 categories according to the different gastric lesions, which included H. pylori infection, gastritis, peptic ulcer, gastric cancer and gastric precancerous lesions; and gastritis included superficial gastritis, erosive gastritis and atrophic gastritis. Peptic ulcer included gastric ulcer, duodenal ulcer and compound ulcer. Precancerous lesions in-cluded polyps and stomatitis. The expression of PG ( PGⅠand PGⅡ) in serum was tested by automatic biochemical an-alyzer, and the ratio of PGⅠto PGⅡ( PGR) was calculated. The difference PG level in each group was analyzed by t test and ANOVA. Results Compared with control group, the expressions of PGⅠ, PGⅡand PGR in H. pylori infection patient was significantly higher (P<0. 05). In gastritis groups, the expressions of PGⅠ, PGⅡand PGR in superficial gastritis, the change was not obvious compared with control group (P>0. 05); compared with control group, the ex-pressions of PGⅠand PGR in erosive gastritis group were increased significantly ( P<0. 05) , the expression of PGⅡwas increased, but there was no statistically significant (P>0. 05); the expressions of PGⅠ, PGⅡand PGR in atrophic gastritis were significantly decreased significantly compared with control group (P<0. 05). In peptic ulcer groups, the expressions of PGⅠ, PGⅡ in gastric ulcer group were significantly higher than those in normal control group ( P <0. 05) , and there was no significant difference in PGR value between gastric ulcer group and normal control group ( P>0. 05) . The expressions of PGⅠ, PGⅡ and PGR in the duodenal ulcer patients and the complex ulcer patients were significantly higher than those in normal control objects (P<0. 05). Compared with normal control group, the expres-sions of PGⅠand PGR in gastric ulcer group was significantly higher; the value of PGⅡwas increased, there was no sig-nificant difference. The expressions of PGⅠ, PGⅡand the level of PGR in stump gastritis group were significantly lower than those in normal control group ( P<0. 05) . The expressions of PGⅠand PGR in gastric cancer group were signifi-cantly lower than those in normal control group (P<0. 05). The expression of PGⅡwas significantly increased, but the difference was not significant. Conclusion The combined use of serum PGⅠ, PGⅡ and PGR can contribute to screening, diagnosis and identification of chronic stomach diseases, which has significant clinical significance.
