Abstract:To the editor: A 7-month-old appeared with acyanosis, an III/6 continuous murmur at left sternal border and weak femoral pulses. Electrocardiography presented biventricular enlargement with strain. Chest X-ray showed asymmetric bilateral pulmonary blood flow. Echocardiography revealed absence of right pulmonary artery (RPA), aortic coarctation (CoA), patent ductus arteriosus (PDA), moderate to severe tricuspid regurgitation, enlargement of right atrium and right ventricle. Cardiac catheterization and angiogram were performed under general anesthesia. The main pulmonary artery was extended only by the left pulmonary artery (LPA). A small isolated RPA was shown by right pulmonary vein wedge angiography, and there were fine collateral vessels into the right lung from right brachiocephalic artery and the descending aorta. The right ductus arteriosus closed with a diverticulum left in the base of the innominate artery in this left aortic arch. The long tube-shaped left ductus was patent with 3 mm diameter, and there was preductal discrete aortic coarctation with systemic pressure gradient 34 mmHg (Figure 1): QP/QS was 1.25 and PVR was 8.2 wood unit.Figure 1. F: A 4-mm modified symmetric perimembrous VSD device was deployed to occlude the PDA and a 7 mm×2 cm balloon was inflated to dilate the CoA.The diagnosis was established and informed consent was signed by the parents. The transcatheter closure of PDA with a VSD device and balloon dilatation of the CoA were attempted and successed (Figure 1). After the procedure, the child was given aspirin for 3 months. In a 12 months follow-up, the boy was doing well with no residual shunt nor recoarctation by echocardiography, the heart chambers recovered to normal and tricuspid regurgitation was only mild. Unilateral absence of a pulmonary artery (UAPA) is a rare cardiovascular malformation. Since the symptoms of UAPA are non-specific and even asymptomatic for a long period, the actual prevalence is difficult to establish, and one current estimate is 1 in 200 000 individuals. There is now extensive documentation of patients in whom the sole cardiovascular abnormality is pulmonary artery nonconfluence with distal ductal or ligamental origin of one pulmonary artery, and the closure of the arterial duct isolates the involved pulmonary artery, resulting in progressive diminution in the size of the isolated pulmonary artery, lung and hemithroax. Some of these patients will develop small aortapulmonary collateral arteries from bronchial, intercostal, internal mammary, subdiaphragmatic, subclavian, or even coronary arteries.1 In distal ductal (or ligamental) origin of one pulmonary artery, it is more frequent to have the involved ductus and “absent” pulmonary artery on the side opposite the laterality of the aortic arch. This case was exactly an example. However, in near 300 cases reported till now,2 no patients were found to have association of both PDA and CoA. Therapeutic strategies for isolated unilateral absence of a proximal pulmonary artery remain unclear. Some authors recommended using serial echocardiography to monitor asymptomatic adults for the development of pulmonary hypertension.3 Revascularization is an altered treatment for those UAPA patients with identifiable artery at the hilum, however the outcome is dependant.4 For this case, after the PDA occlusion and CoA dilatation, the pulmonary hypertension significantly improved, whether further surgical revascularization of isolated RPA or not depends on the following-up pulmonary hypertension.
