Abstract:Asthma is a phenotypically heterogeneous chronic disease of the airways . Studies have found that neutrophils are crucial to airway inflammation in acute asthma , persistent asthma , particularly in asthma of poor response to glucocorticoid treatment . The role of neutrophils in development of bronchial asthma is complex , as they can release a potent source of cytokines and inflammatory mediators participating in asthma . Differing from eosinophilic inflammatory asthma , neutrophilic inflammatory asthma is not depend on helper T ( Th)2 cells, but may be related to Th1 and Th17 cells.This review highlights the role of neutrophils in the development of asthma , and the treatment of neutrophilic asthma with biological agents and novel small molecules .

Abstract:Bronchial asthma is a chronic respiratory disease，characterized by airway inflammation，airway hyperresponsiveness，reversible airway obstruction and airway remodeling，in which a variety of cells including airway inflammatory cells and structural cells are involved. Previous studies have shown that asthma is mainly driven by Th2 cytokines IL-4，IL-5，and IL-13，leading to airway eosinophil inflammation. With further research，however，it has been found that neutrophils are also closely related to asthma. Numbers of neutrophils are elevated in airway through increased chemotaxis and decreased apoptosis，which is earlier than eosinophils，leading to airway neutrophilic inflammation. Neutrophils can produce elastase，myeloperoxidase，neutrophil extra- cellular traps，chemokines and cytokines，participating in the occurrence and development of asthma. The antagonists against these molecules，such as anti-IL-8 receptor antibody，anti-IL-17 antibody，and DNase，have shown positive effects on neutrophilic asthma，but further studies are needed to support their clinical application. This article mainly reviews the role of neutrophils in asthma and related mechanisms.

Abstract:Asthma is a heterogeneous disease that manifests as a chronic inflammatory response in the airways and afflicts more than 300 million people worldwide. [ 1 ] It is characterized by variable airflow obstruction and airway hyperresponsiveness (AHR). Environmental factors such as allergens are major triggers of the narrow airway response in asthma. The physiology of asthma is characterized by bronchial hyperresponsiveness, a tendency of airway smooth muscle to contract when stimulated by allergens. This leads to acute airway narrowing, which is relieved by bronchodilator therapy. However, some patients with asthma that show poor disease control require high doses of inhaled steroids, a second medication, and systemic corticosteroids to stabilize the disease, and reductions in dosage can exacerbate symptoms. Some patients with more severe symptoms cannot control their condition even at the highest medication dose. Asthma that is difficult to control medically is called severe asthma, [ 2 ] which accounts for 5% to 10% of the patients with asthma and consumes more than 60% of the total disease costs, [ 3 ] representing a global treatment challenge. In recent years there have been considerable advances in the study of the underlying molecular mechanisms or endotypes in asthma. As a result, asthma has been separated into 2 population groups. [ 4 ] The first group is "type 2 high" asthma, which includes allergic and eosinophilic asthma characterized by secretion of high levels of interleukin (IL)-4, IL-5, and IL-13. This promotes an increase in the number of eosinophils, periplasmic serum proteins, and total IgE, leading to an increase in airway mucus and bronchial hyperresponsiveness. The other group, "type 2 low" asthma, includes neutrophilic asthma (NA) and obesity asthma, usually without T2 biomarkers or elevated eosinophils.

Abstract:Asthma is a chronic inflammatory disease that undermines the airways. It is caused by dysfunction of various types of cells, as well as cellular components, and is characterized by recruitment of inflammatory cells, bronchial hyperreactivity, mucus production, and airway remodelling and narrowing. It has commonly been considered that airway inflammation is caused by the Th2 immune response, or eosinophilia, which is a hallmark of bronchial asthma pathogenesis. Some patients display a neutrophil-dominant presentation and are characterized with low (or even absent) Th2 cytokines. In recent years, increasing evidence has also suggested that neutrophils play a key role in the development of certain subtypes of asthma. This review discusses neutrophils in asthma and potentially related targeted therapies.
Airway Inflammation and Small Airway Wall Remodeling in Neutrophilic Asthma

Abstract:OBJECTIVE:To investigate the distribution of airway inflammation phenotype in patients with bronchial asthma (asthma), and to analyze clinical characteristics, inflammatory cytokines, pulmonary small vessels remodeling and small airway wall remodeling in patients with neutrophilic asthma.METHODS:Sixty-three patients with asthma were enrolled from January 2015 to December 2015 in Peking University Third Hospital. Clinical data including gender, age, body mass index (BMI), pulmonary function tests (PFTs), asthma control test (ACT) were recorded. All the patients underwent sputum induction. The cellular composition of the sputum was evaluatedand the concentration of active MMP-9 in the sputum tested. Blood routine tests were done and the concentration of IgE, periostin, and TGF-beta1 levels were measured in serum by enzyme-linked immunosorbent assay (ELISA). Small airway wall remodeling was measured in computed tomography (CT) scans, as the luminal diameter, luminal area, wall thickness and wall area % adjusted by body surface area (BSA) at the end of the 6th generation airway, in which the inner diameter was less than 2 mm. Small vascular alterations were measured by cross-sectional area (CSA), and the total vessel CSA < 5 mm2 was calculated using imaging software.RESULTS:The distributions of airway inflammatory phenotypes of the asthmatic patients were as follows: neutrophilic asthma (34.9%, 22/63), eosinophilic asthma (34.9%, 22/63), mixed granulocytic asthma (23.8%, 15/63), and paucigranulocytic asthma (6.3%, 4/63). The neutrophilic subtype patients had a significantly higher active MMP-9 level in sputum compared with the eosinophilic phenotypepatuents, as 179.1 (74.3, 395.5) vs. 50.5 (9.7, 225.8), P<0.05. Sputum neutrophil count was negatively correlated with FEV1%pred (r=-0.304,P<0.05), and positively correlated with active MMP-9 level in sputum (r=-0.304, P<0.05), and positive correlation trend with airway wall thickness (r=0.533, P=0.06). There was a significantly negative correlation of active MMP-9 level in sputum with FEV1%pred (r=-0.281, P<0.05), in positive correlation with small airway wall area (%)(r=0.612, P<0.05), and inpositive correlation trend with airway wall thickness (r=0.612, P=0.06). Neutrophils count in peripheral blood was positively correlated with neutrophil counts in sputum.CONCLUSION:Neutrophil count in airway is related to lung function in asthmatic patients. Neutrophils may accelerate small airway wall remodeling through the release of active MMP-9. Neutrophil count in peripheral blood is related to neutrophils count in sputum, which may be used as a substitute for evaluating inflammatory phenotype.

Abstract:Asthma is a common chronic airway disease affecting about 334 million people worldwide, and an estimated 7 million children globally. Approximately 10% of patients with asthma have severe refractory disease, which is difficult to control on high doses of inhaled corticosteroids and other modifiers. Among these, are patients with severe neutrophilic asthma. Neutrophilic asthma is a phenotype of asthma that is very severe and persistent, with frequent exacerbations, and characterized by fixed airway obstruction. It is associated with comorbidities such as respiratory infections, obesity, gastroeosophageal reflux disease, and obstructive sleep apnoea. Immunopathologically, it is characterized by the presence of high levels of neutrophils in the lungs and airways. Neutrophils and the interleukin-17 family of cytokines play a pivotal role in the pathogenesis of severe neutrophilic asthma. Most patients with the disease do not achieve control with high dose inhaled corticosteroids, and probably to novel IgE, interleukin and interleukin monoclonal antibodies.

Abstract:Asthma is a complex airway inflammatory disease that can be roughly classified into eosinophilic phenotype and non-eosinophilic phenotype. Most of the latter manifested as airway inflammation dominated by neutrophil infiltration, namely neutrophil-dominated asthma (NA). Neutrophil extracellular trapping (NETs) is a newly discovered antimicrobial mechanism of neutrophils; however, NETs can not only resist killing pathogenic microorganisms, but also promote tissue damage and autoimmune response. In the present study, we successfully established NA model in C57BL/6 mice and observed the increased formation of NETs. In NA mice, the free DNA abundance, the airway resistance, the cell numbers (total cell number, macrophage number, and neutrophil number), and inflammatory cytokine levels were significantly increased while the lung dynamic compliance was significantly reduced. After DNase I treatment, the above indexes in NA mice were all improved. In NA mice, either treatment with macrophage scavenger or IL-1β neutralizing antibody also improved the above-described indexes. In vitro, in human peripheral blood-derived neutrophils, PMA treatment significantly increased the formation of NETs. Furthermore, in macrophages differentiated from THP-1 monocytes, LPS or isolated NETs both significantly increased the levels of cytokines. In conclusion, NETs can stimulate macrophages to secrete IL-1β, which promotes neutrophils infiltration in the airway; infiltrated neutrophils, in turn, generates NETs, which can amplify the tissue damage caused by NETs and macrophages, inducing and aggravating NA.

Abstract:Asthma is a complex heterogeneous disease. The neutrophilic subtypes of asthma are described as persistent, more severe and corticosteroid-resistant, with higher hospitalization and mortality rates, which seriously affect the lives of asthmatic patients. With the development of high-throughput sequencing technology, an increasing amount of evidence has shown that lower airway microbiome dysbiosis contributes to the exacerbation of asthma, especially neutrophilic asthma. Nontypeable Haemophilus influenzae is normally found in the upper respiratory tract of healthy adults and is one of the most common strains in the lower respiratory tract of neutrophilic asthma patients, in whom its presence is related to the occurrence of corticosteroid resistance. To understand the pathogenic mechanism by which nontypeable Haemophilus influenzae colonization leads to the progression of neutrophilic asthma, we reviewed the previous literature on nontypeable Haemophilus influenzae colonization and subsequent aggravation of neutrophilic asthma and corticosteroid resistance. We discussed nontypeable Haemophilus influenzae as a potential therapeutic target to prevent the progression of neutrophilic asthma.

Abstract:Respiratory diseases, always being a threat towards the health of people all over the world, are most tightly associated with immune system. Neutrophils serve as an important component of immune defense barrier linking innate and adaptive immunity. They participate in the clearance of exogenous pathogens and endogenous cell debris and play an essential role in the pathogenesis of many respiratory diseases. However, the pathological mechanism of neutrophils remains complex and obscure. The traditional roles of neutrophils in severe asthma, chronic obstructive pulmonary diseases (COPD), pneumonia, lung cancer, pulmonary fibrosis, bronchitis, and bronchiolitis had already been reviewed. Abstract:Airway epithelial transcriptome analysis of asthma patients with different severity was used to disentangle the immune infiltration mechanisms affecting asthma exacerbation, which may be advantageous to asthma treatment. Here we introduce various bioinformatics methods and develop two models: an OVA/CFA-induced neutrophil asthma mouse model and an LPS-induced human bronchial epithelial cell damage model. Our objective is to investigate the molecular mechanisms, potential targets, and therapeutic strategies associated with asthma severity. Multiple bioinformatics methods identify meaningful differences in the degree of neutrophil infiltration in asthma patients with different severity. Then, PTPRC, TLR2, MMP9, FCGR3B, TYROBP, CXCR1, S100A12, FPR1, CCR1 and CXCR2 are identified as the hub genes. Furthermore, the mRNA expression of 10 hub genes is determined in vivo and in vitro models. Reperixin is identified as a pivotal drug targeting CXCR1, CXCR2 and MMP9. We further test the potential efficiency of Reperixin in 16HBE cells, and conclude that Reperixin can attenuate LPS-induced cellular damage and inhibit the expression of them. In this study, we successfully identify and validate several neutrophilic signatures and targets associated with asthma severity. Notably, Reperixin displays the ability to target CXCR1, CXCR2, and MMP9, suggesting its potential therapeutic value for managing deteriorating asthma.

Abstract:Highlights • A nanotherapy with multiple pharmacological activities is engineered from a bioactive material for the treatment of asthma. • The intravenously injected or inhaled nanotherapy can effectively accumulate in multiple pathologically relevant cells. • The nanotherapy can ameliorate asthmatic symptoms, attenuate pulmonary inflammation, and reduce airway hyperresponsiveness. • The nanotherapy inhibits the neutrophil extracellular traps formation and inflammasome activation in neutrophils. • Targeting and therapeutic effects of the nanotherapy are enhanced by surface engineering with the neutrophil cell membrane. Asthma is a serious global public health concern. Airway neutrophilic inflammation is closely related to severe asthma, for which effective and safe therapies remain to be developed. Here we report nanotherapies capable of simultaneously regulating multiple target cells relevant to the pathogenesis of neutrophilic asthma. A nanotherapy LaCD NP based on a cyclic oligosaccharide-derived bioactive material was engineered. LaCD NP effectively accumulated in the injured lungs of asthmatic mice and mainly distributed in neutrophils, macrophages, and airway epithelial cells after intravenous or inhalation delivery, thereby ameliorating asthmatic symptoms and attenuating pulmonary neutrophilic inflammation as well as reducing airway hyperresponsiveness, remodeling, and mucus production. Surface engineering via neutrophil cell membrane further enhanced targeting and therapeutic effects of LaCD NP. Mechanistically, LaCD NP can inhibit the recruitment and activation of neutrophils, especially reducing the neutrophil extracellular traps formation and NLRP3 inflammasome activation in neutrophils. Also, LaCD NP can suppress macrophage-mediated pro-inflammatory responses and prevent airway epithelial cell death and smooth muscle cell proliferation, by mitigating neutrophilic inflammation and its direct effects on relevant cells. Importantly, LaCD NP showed good safety performance. Consequently, LaCD-derived multi-bioactive nanotherapies are promising for effective treatment of neutrophilic asthma and other neutrophil-associated diseases. Abstract:INTRODUCTION:Asthma is the most common chronic respiratory disease and has been declared a global public health problem by the World Health Organization. Due to the high heterogeneity and complexity, asthma can be classified into different 'phenotypes' and it is still difficult to assess the phenotypes and stages of asthma by traditional methods. In recent years, mass spectrometry-based proteomics studies have made significant progress in sensitivity and accuracy of protein identification and quantitation, and are able to obtain differences in protein expression across samples, which provides new insights into the mechanisms and classification of asthma.AREAS COVERED:In this article, we summarize research strategies in quantitative proteomics, including labeled, label-free and targeted quantification, and highlight the advantages and disadvantages of each. In addition, new applications of quantitative proteomics and the current status of research in asthma have also been discussed. In this study, online resources such as PubMed and Google Scholar were used for literature retrieval.EXPERT OPINION:The application of quantitative proteomics in asthma has an important role in identifying asthma subphenotypes, revealing potential pathogenesis and therapeutic targets. But the proteomic studies on asthma are not sufficient, as most of them are in the phase of biomarker discovery.

Abstract:Asthma, one of the most common lung diseases, remains a serious global health problem. Currently there is still an urgent need for effective and safe therapies against severe asthma. Neutrophil extracellular traps (NETs) have emerged as a new therapeutic target for asthma, while precision regulation of NETs is highly challenging. Here, we report site-specific attenuation of neutrophilic inflammation in the lungs via a cyclic oligosaccharide-derived nanotherapy (termed TPCN) for targeted treatment of asthma. Either intravenous or inhaled TPCN can distribute in lung neutrophils of asthmatic mice, thereby significantly mitigating oxidative stress, suppressing inflammatory responses, reversing airway remodelling, and improving pulmonary function in mice with allergic/neutrophilic asthma. Notably, TPCN is effective even at an actual inhalation dose of 0.06–0.07 mg/kg. Mechanistically, TPCN alleviates eosinophilic asthma by directly attenuating neutrophil-mediated T helper 2 (Th2) cell responses. For neutrophilic asthma, a typical phenotype of severe asthma, efficacies of TPCN are achieved by inhibiting the reactive oxygen species (ROS)-induced formation of NETs, which further promotes immune homeostasis via regulating the balance of regulatory T (Treg) and T helper 17 (Th17) cells. Thus, TPCN holds great promise for precision therapy of eosinophilic and neutrophilic asthma, while the ROS-NETs-Treg/Th17 pathway can function as intriguing therapeutic targets for the treatment of severe asthma and other neutrophil-mediated inflammatory diseases.

Abstract:Neutrophils can regulate adaptive immune responses and contribute to chronic inflammation including asthma. However, the roles and mechanisms of neutrophils in initiating eosinophilic airway inflammation remain incompletely understood. Neutrophil elastase (NE) is a component of azurophilic granules and a serine protease with potent functions during inflammation. Here, we showed that neutrophils were early recruited at the onset of asthmatic inflammation by related chemokines. Furthermore, neutrophils could capture allergens and release NE to promote neutrophil aggregation at first. Then they prompt eosinophil infiltration and amplify type 2 immune responses in later phases. Also, this process can be rescued by administration of the NE inhibitor (GW311616). Our data collectively indicate that neutrophils could contribute to asthmatic inflammation by releasing NE.

Abstract:Asthma is a chronic airway inflammatory disease with complex mechanisms, and these patients often encounter difficulties in their treatment course due to the heterogeneity of the disease. Currently, clinical treatments for asthma are mainly based on glucocorticoid-based combination drug therapy; however, glucocorticoid resistance and multiple side effects, as well as the occurrence of poor drug delivery, require the development of more promising treatments. Nanotechnology is an emerging technology that has been extensively researched in the medical field. Several studies have shown that drug delivery systems could significantly improve the targeting, reduce toxicity and improve the bioavailability of drugs. The use of multiple nanoparticle delivery strategies could improve the therapeutic efficacy of drugs compared to traditional delivery methods. Herein, the authors presented the mechanisms of asthma development and current therapeutic methods. Furthermore, the design and synthesis of different types of nanomaterials and micromaterials for asthma therapy are reviewed, including polymetric nanomaterials, solid lipid nanomaterials, cell membranes-based nanomaterials, and metal nanomaterials. Finally, the challenges and future perspectives of these nanomaterials are discussed to provide guidance for further research directions and hopefully promote the clinical application of nanotherapeutics in asthma treatment.

Differential Therapeutic Activity of a Cargo-Free Nanoparticle in Eosinophilic and Neutrophilic Asthma Mouse Models

Abstract:Asthma is a complex, heterogeneous chronic airway inflammatory disorder, which can be simply categorized into eosinophilic and neutrophilic asthma. Although most patients with eosinophilic asthma can be well managed by glucocorticoid therapy, the patients with neutrophilic asthma have poor responses to corticosteroids, and there are no effective clinical treatments for neutrophilic asthma. Studies have shown that M1 macrophages are associated with the pathophysiology of neutrophilic asthma. We previously developed a novel cargo-free anti-inflammatory nanoparticle (P12), which can promote macrophage polarization from M1 to the M2 phenotype. Herein, we aim to explore the therapeutic activities of P12 in eosinophilic asthma and neutrophilic asthma and the mechanisms of action behind. Using the mouse models of eosinophilic and neutrophilic asthma, we found that the intratracheally administered P12 reduced the airway resistance in both models. However, P12 had higher therapeutic effects in neutrophilic asthma than in eosinophilic asthma, particularly on the pathological score, the serum IgE level, the BAL differential cell count and the proinflammatory cytokine IL-17A and IL-6 levels. Mechanistically, P12 facilitated M2 macrophage polarization by inhibiting transcription factors NF-κB and STAT1 activation and up-regulating STAT3 signaling in vitro; in the asthma models, P12 promoted M2 polarization of both alveolar and interstitial macrophages, and reduced the downstream Th17/Treg ratio in mice with neutrophilic asthma. Finally, P12 showed a good biosafety profile in healthy mice, facilitating the clinical translation of P12. This study provides a new strategy and lung macrophage targeted nanotherapeutics for the treatment of neutrophilic asthma.
[The current status of the study on asthma in China]

Abstract:Purpose: To review the recent developments in asthma research in China, presenting the background of further asthma research and promoting academic exchanges between Chinese and foreign respirologists. Data sources: A literature search was conducted using 5 authoritative journals covering the years 1994-1998. Study selection: Sixty-seven of 238 asthma-remated original articles published between the years 1994-1998 were selected to reflect the recent developments in asthma in China. Data extraction: The papers reviewed pertain mainly to the epidemiology, genetics, pathogenesis, prevention and treatment of asthma. Results: Since 1993, the hot topics in asthma research in China have been pathogenesis, prevention and treatment. There has been relatively less investigation into the genetics of asthma in China. An epidemiological survey of asthma has been conducted in parts of China and was reviewed. Conclusions: More and more attention has been paid to the study of asthma during the last five years in China. The prevalence of asthma has increased in recent years. The study of the pathogenesis of asthmatic airway inflammation has made further progress, which will help the management and treatment of asthma. The treatments for asthma, including herbal medicine, have been explored and the clinical efficacy of inhaled corticosteroids has been established.
