Abstract:Objective To understand the clinical characteristics of colorectal cancer(CRC) occurring in different age groups,and provide clinical/demographic information for early diagnosis and effective treatment.Methods Primary CRC cases were retrieved from the in-patient department of our hospital during 2000~2008,and a total of 1 308 cases were found.Patients were classified into three groups according to age including younger group(≤40 years,5.0%),middle-age group(41~70 years,61.5%),and elder group(>70 years,33.4%).The patients of 66,86 and 76 cases were randomly selected from each group respectively and their complete medical records were abstracted.Characteristics compared included demographic factors,clinical features,diagnosis time,localization of tumors,and tumor histology and stage.Results Male patients accounted for 48.5%,60.5% and 65.8% of cases in younger,middleage and elder groups,respectively(χ2=4.34,P=0.04).Initial symptoms in these three groups were similar,including rectal bleeding,abdominal pain or discomfort,and change in bowel habits.The median diagnostic time for the three groups were 120,68 and 65 days and 50.0%,41.2% and 28.6% of cases were in advanced stage when diagnosed(χ2=6.36,P=0.04),respectively.However,the misdiagnosing rates and the primary diagnosis method were not significantly different among the three groups.Cases concurrent with polyps were 53.8%,78.6% and 87.3%,respectively(χ2=14.2,P=0.00).The rates of tumors located in distal colon were 83.8%,67.4% and 64.9% in each group respectively(χ2=7.38,P=0.03),including 48.5%,24.1% and 26.6% in rectum alone (χ2=12.1,P=0.00).The younger group had more mucinous cancers(χ2=16.2,P=0.04) and a higher rate of undifferentiated adenocarcinoma(χ2=6.29,P=0.04) than the othergroups.Conclusion Colorectal cancers in different age groups show different clinical characteristics.Young cases diagnosed by longer time appear to be more aggressive,to present with later stage,and to have poorer pathologic findings.A slight male predominance,more proximal tumor site,increased coexist of colorectal polyps are found in elders.
