Abstract:The significance of the microcirculation on the development of atherosclerosis has not been fully appreciated. Atherosclerosis in known to develop in areas of low shear stress, but this shear stress is affected by a complex interplay of arterial anatomy and distal arteriolar tone. This study evaluated the interrelationship between renal artery anatomy, intrinsic distal resistance, pressure, and shear stress in order to understand the forces leading to renal artery atherosclerosis. Patient-specific computational modeling of renal artery blood flow as derived from computed tomography (CT) arteriography was used to evaluate the relationship between renal artery anatomy, intrinsic renal arteriolar resistance, pressure, shear stress, and plaque formation. The fluid dynamic techniques used have been validated with patient-specific duplex ultrasound results and experimental studies done in our laboratory and by others. The anatomy of the aorta and renal arteries had the greatest effect on vorticity and shear stress, and our findings of eddy currents and areas of low shear on the cephalad wall of the renal artery correlated with a previous experimental finding of plaque development primarily occurring in this region. Beyond anatomical effects, small changes (25 percent) in distal intrinsic renovascular resistance consistent with increased arteriolar tone had a significant effect on further flow distortions, pressure elevations, and shear stress reductions. The presence of up to a 40% orificial renal artery stenosis had little effect on these parameters. Blood flow is parametrically studied by varying the size of the renal stenosis (SN) and the degree of distal resistance (DRT) represented by varying the outflow resistance of the right renal artery. The condition of the left renal artery remains the same. The anatomic relationship of the aorta and renal arteries as well as small changes in renal arteriolar tone have significant effects on renal artery blood flow patterns, pressures, and shear stress, which potentiates the development of atherosclerosis. Up to a 40% orificial renal artery stenosis had little effect on factors thought to affect progression of atherosclerotic disease. Change in the intrinsic renovascular resistance is the one factor that is most treatable for the prevention of renal artery stenosis. Unfortunately, the renal resistance index may not be sufficiently sensitive to detect the small changes in renal resistance that may be detrimental to the hemodynamics within the renal artery. Improved methods for detecting small changes in renovascular resistance and medications or treatments specifically modifying end arteriole tone are needed to limit progressive renovascular occlusive disease.
