Risk Factors of Atrial Fibrillation after Coronary Artery Bypass Grafting

Abstract:Objective To identify the risk factors inducing atrial fibrillation (AF) after coronary artery bypass grafting (CABG) and to assess the efficacy of amiodarone in preventing AF after CABG.Methods Two hundred thirty five patients who underwent CABG between April 1998 and April 1999 were studied retrospectively.These patients were divided into two groups:one group was 155 patients who were given β blockers,calcium antagonist,and deslansidum;another group included 80 patients who were given amiodarone.Comparison was made between the two groups.The variables observed in the study included incidence and lasting time of AF,patients' age,electrolytes,heart function,left atrial diameter,severity of right coronary artery stenosis,myocardial enzyme,duration of cardiopulmonary bypass,aortic clamp,and so on.Univariate analysis and multivariate regression analysis were used in the study.Results AF occurred in 17.9% patients within 1～3 days after CABG and lasted for (12.0±9.14)hours;Older age (≥70 years),hypokalemia,hypomagnesemia,heart failure(EF≤0.40 and/or FS≤0.24),bigger left atrial diameter(≥35 mm) and severe right coronary artery stenosis(≥70%) were the risk factors for AF;Older age,heart failure,hypokalemia were independent preoperative predictor for postoperative AF;Postoperative administration of amiodarone significantly reduced ventricular rate and the incidence and duration of AF.Conclusion The incidence of AF is higher in patients after CABG.Older age,hypokalemia,hypomagnesemia,heart failure,big left atrial diameter,and severe right coronary artery stenosis were all risk factors for AF.Furthermore,older age,heart failure,hypokalemia were independent preoperative predictors for postoperative AF.Finally amiodarone could significantly reduce ventricular rate during AF and the incidence and duration of AF.
