Abstract:The gastrointestinal system is essential in the control of energy metabolism, not only because it is the site of nutrient absorption, but also because a variety of gastrointestinal hormones and neuropeptides integrate peripheral and central regulation of food intake and body weight. Among these hormones, ghrelin is the only currently identified orexigenic hormone in the circulation. Three major forms of ghrelin have been identified: acyl ghrelin, des-acyl ghrelin and obestatin. Acyl ghrelin is the only one capable of binding and activating the ghrelin receptor. Blocking the action of acyl ghrelin by receptor antagonist, neutralizing antibodies or Spiegelmer RNA has been reported to affect food intake and body weight in preclinical models. However, the long-term use of these approaches is offset by potential increases in circulating ghrelin due to the feedback regulation. Defining novel pathways to inhibit the production of acyl ghrelin is appealing for the pharmaceutical intervention of obesity and would shift therapeutic focus to gastric targets.Ghrelin producing cells comprise approximately one quarter of gastric endocrine cells. The secretion of ghrelin is tightly coupled to the fasting or fed state. While it is presumed that precise control in the production and secretion of ghrelin is critical for the maintenance of energy balance, the molecular mechanisms by which ghrelin producing cells modulate transcription and translation of ghrelin to match overall energy status are unknown. Their studies demonstrate that mammalian target of rapamycin (mTOR) is present in the gastric ghrelin cells and its activity is altered during fasting and obesity. In addition, there exists a reciprocal relationship between gastric mTOR signaling and the expression and secretion of ghrelin during changes in energy status. Both in vivo and in vitro data support the concept that gastric mTOR activity is reciprocally linked to the production of ghrelin and ghrelin O-acyl transferase (GOAT), the enzyme mediating the octanoylation of ghrelin. Consistent with the increase of circulating ghrelin, inhibition of the gastric mTOR activity stimulates food intake. This orexigenic effect of gastric mTOR inhibition can be blocked by antagonist of ghrelin receptor.In summary, their studies suggest that gastric ghrelin cells may recognize energy status at the organism level. The mTOR signaling may serve as a unique fuel sensor in gastric ghrelin cells, whose activity is linked to the regulation of energy intake through ghrelin.
