Predictive value of serum procalcitonin level for prognosis of acute myocardial infarction

Abstract:Objective To investigate the association between serum procalcitonin (PCT) level and short-term clinical outcome in patients with acute myocardial infarction (AMI).Methods One hundred and forty-six patients were enrolled,serum PCT,brain natriuretic peptide(BNP),C-reactive protein(CRP) and cardiac troponin Ⅰ (cTnI) were detected within 24 hours.Patients were divided into surviving group and death group according to the 30-day survival outcome.The receiver operating characteristic curve (ROC) and Kaplan-Meier survival curves were plotted.The independent risk factors for predicting death were detected by Cox regression analysis.Results There were 109 cases in surviving group,37 cases in death group.Significant difference in serum PCT level was found between survival group and death group(P<0.001).ROC curve analysis showed that the area under the curve (AUC) of PCT was 0.869,larger than BNP (0.813),CRP (0.763) and cTnI (0.682);when PCT>0.2 μg/L,the sensitivity of predicting 30-day mortality was 83.8%,and the specificity was 81.7%.Cox regression analysis showed that PCT was the risk factor for predicting 30-day mortality of AMI [hazard ratio (HR)=1.192,95% confidence interval(CI):1.106-1.278,P=0.041].Conclusions PCT represented a reliable prognostic marker for 30-day mortality of AMI patients.
