Changes of Clinical and Colonscope's Characteristics of Ulcerative Colitis in A Hospital in the Recent 20 Years

Abstract:Objective: To investigate the changes of clinical and colonscopes characteristics of ulcerative colitis(UC) in a hospital in Wuhan city,central China in the recent 20 years.Methods: The clinical and colonscope's characteristics of the patients with ulcerative colitis were retrospectively analyzed in Zhongnan Hospital of Wuhan University from Jan 1,1991 to Dec 31,2010.All patients were divided into two groups according to different time phases.Results: Of all the 404 consecutive recorded patients,104 patients with a mean age of 45.1 years were from year 1991 to 2000(group A),and the other 300 patients with a mean age of 41.9 were from year 2001 to 2010(group B).The age of the patients in group B was younger than that in group A at the time of diagnosis(P0.01).The difference of sex ratio between the two groups was significant(P0.01).Compared with that respectivley in the group A,blood stool,mucus and pus stool,weight loss in UC were more common in the group B(P0.05),whereas abdominal pain and tenesmus were less common(P0.05);extensive colitis and proctitis were more common,and left-sided colitis was less common(P0.01);moderate UC was more common in the group B(P0.05).However,the incidence of extraintestinal manifestation was lower in the group B(P0.05),the diffuse lesions,continuous distribution congestion and edema,erosion,superficial ulceration and purulent secretion of intestinal mucosa were more common in the group B(P0.01).Conclusion: During the recent 20 years,the prevalence of UC in this hospital increases significantly,gender difference tends to decrease,the age of onset is getting younger,UC tends to spread to the extensive colon,moderate UC increases and mild UC decreases gradually,the UC clinical and colonoscopy manifestations are more severe,and the incidence of extra-intestinal manifestations decreases.
