Laparoscopic Versus Open Abdominoperineal Resection in the Treatment of Low Rectal Cancer：a Clinical Comparative Study

Abstract:Objective-To-investigate-the-feasibility-,-safety,-radicality,-and-short-term-clinical-outcomes-of-laparoscopic-abdominoperineal-resection-of-rectal-cancer-.-Methods-Clinical-data-of-117-patients-with-lower-rectal-cancer-,-who-received-abdominoperineal-resection-from-June-2010-to-October-2012,-were-reviewed-retrospectively-.Sixty-five-patients-received-laparoscopic-operation-(-laparoscopic-group-)-and-52-patients-received-open-resection-(-open-group-)-.The-intraoperative-indicators-,-recovery-indicators-and-number-of-lymph-nodes-resected-of-the-two-groups-were-compared-.-Results-The-surgery-was-performed-successfully-in-all-the-117-patients-of-both-groups.There-was-no-significant-difference-between-the-two-groups-in-the-operation-time-[(195.7-±-30.4)-min-vs.(186.5-±29.1)-min,-t=1.673,-P=0.095].As-compared-to-the-open-group,-the-laparoscopic-group-showed-less-intraoperative-blood-loss-[(162.1-±28.1)-ml-vs.(256.9-±48.6)-ml,-t=-13.215,-P=0.000],-shorter-gastrointestinal-function-recovery-time-[(3.1-±1.0)-d-vs.(4.3-±0.8)-d,-t=7.018,-P=0.000],-shorter-urination-recovery-time-[(7.2-±0.9)-d-vs.-(8.6-±1.1)-d,-t=-7.568,-P=0.000],-and-less-incision-infection-[0-(0/65)-vs.9.6%-(5/52),-P=0.016].There-were-no-significant-differences-between-the-two-groups-in-average-number-of-removed-lymph-node-[(15.6-±4.1)-vs.(14.3-±3.5),-t=1.817,-P=0.075]-and-pulmonary-infection-[3.1%-(2/65)-vs.7.7%-(4/52),-χ2-=0.494,-P=0.482].-Conclusions-Laparoscopic-abdominoperineal-resection-is-safe-and-feasible-for-the-patient-with-lower-rectal-cancer-,-with-advantages-of-less-blood-loss-and-rapid-postoperative-recovery-.The-short-term-outcomes-of-laparoscopic-operation-are-better-than-open-procedure-.It-is-worthy-of-outreach-.
