Medial compartment osteoarthritis associated with lateral meniscus injury:a combination of unicompartmental knee arthroplasty and arthroscopic treatment

Abstract:Background:In clinics, we found, through MRI or arthroscopy, some cases in whom medial compartment osteo-arthritis was associated with lateral meniscus injury. Are these cases suitable to be treated with unicompartmental knee ar-throplasty (UKA)? Does the lateral meniscus need to be operated simultaneously? Will the simultaneous processing impact the UKA's clinical results? Objective:To observe the short-term curative effect of a combination of UKA and arthroscopic lateral meniscoplasty in the treatment of knee medial compartment osteoarthritis associated with lateral meniscus injury. Methods:Among 516 UKA patients undergoing UKA from January 2014 to January 2016, 12 patients (12 knees) were diag-nosed as medial compartment osteoarthritis associated with lateral meniscus injury (7 cases by preoperative MRI, and 5 cas-es by arthroscopy). They were treated by routine arthroscopic debridement and the lateral meniscus forming surgery com-bined with UKA. The preoperative signs, imaging data, surgical process, and knee function evaluated by the American Knee Society score (KSS score) of these 12 cases were analyzed retrospectively. Results:The average arthroscopic time was 12.2 min (range, 10-15 min). All the patients were followed up for 12 months on average (range, 6-18 months). No infection, fat embolism or deep vein thrombosis of the lower extremity were observed. No prosthesis abrasion, loosening or dislocation appeared. No developed osteoarthritis or any other complications happened. In the last follow-up, the knee pain disappeared or relieved obviously, and the symptoms disappeared completely in the lateral compartment. A statistical difference was found in postoperative and preoperative KSS knee score (93 ± 5 vs 53 ± 8, t=21.78, P<0.05). Similar difference was also showed in postoperative and preoperative KSS function score (94±6 vs 55±8, t=24.11, P<0.05). Conclusions:For the cases diagnosed clearly before operation as medial compartment osteoarthritis associated with lateral meniscus injury, arthroscop-ic lateral meniscus meniscoplasty combined with UKA does not increase surgical complications or operative risk. The short-term clinical curative effect is good.
