Comparison for Patellar Fracture by Absorbable Screw and Tension Band Kirschner's Wire

Abstract:[Objective]To compare the relative problems of patellar fracture by absorbable screw and tension band Kirschner's wire,and to explore the value of absorbable screw in patellar fracture.[Method]There were 94 cases in the PDLLA absorbable screw group,it involved 53 males and 41 females,the average age was 40.5 years,the average following up time were 32 weeks.Another 90 cases were in the traditional tension band wire group,it involved 47 males and 33 females,the average age was 38.5 years,the average following up time were 30 weeks.The t test was used to compare the problems,including the mean union time of fracture,the knee joint flexion and keen flexion loss at 8 weeks postoperatively,operation time,in-patient cost at one time and complications.[Result]In the absorbable screw group,the mean union time were(10±2.0)weeks,the knee flexion and knee flexion loss at the 8th week postoperatively were 115°±10°and 10°±6.8°,operation time was 60 min,and no complications.In the other group,the mean clinical union time were(12±2.0)weeks,the mean knee flexion and flexion loss were 105°±8°and 15°±6°at 8th week postoperatively,the mean operation time was 55 min,cost in hospital was 9430±10000 yuan,In this group,there were 6 cases with Kirschner's wire pulled out and infection,2 cases with Kirschner's wire broken,3 cases with tension band wire loosen and fracture removed,and one cases with patella was damaged and used plaster fixation.[Conclusion]To compare absorbable screw with Kirschner's wire and tension band,it has less stimulation to soft tissue,more stable,shorter union time,better knee joint recovery,less total cost in hospital,no complications and no need to pull out internal fixation again.It can be popularized and researched deeply.
