Network Meta-analysis comparing the efficacy of different analgesics in extracorporeal shock wave lithotripsy

Abstract:Objective:To compare the efficacy of different analgesics in extracorporeal shock wave lithotripsy (ESWL) by network meta-analysis.Methods:Databases including the Ovid, Pubmed, Embase, CIN-AHL, CNKI, Wanfang data were searched for relevant clinical randomized controlled trials (RCTs) comparing the efficacy of different analgesics in ESWL from inception to June 30th 2019.Inclusion criteria involved the ESWL on out patients with urinary stones at least 18 years of age, using a single drug or the same medication as the control group, with or without intraoperative additional pain medication, the control group taking placebo or other, pain medication, drug delivery methods including intravenous, intramuscular, local invasion or the smear, and using visual analog pain score (VAS)as outcome indicator. Exclusion criteria involved the subjects in the studies had a history of drug abuse or were younger than 18 years of age, and data were incomplete or could not be extracted. Two reviewers dependently screened literature, extracted data and assessed the risk of bias of included studies. Meta-analysis was then performed by using Stata 13.0 and WinBugs 1.43 software to compare VAS of patients with different analgesics during ESWL.Result:A total of 17 RCTs involving 1 427 patients were included. All of included articles is indicated B of quality grade after the quality inspection. The funnel plot was used to evaluate the publication offset test and indicate no publication offset. The pairwise comparison results of network Meta-analysis showed that there were statistically significant differences in analgesic effects between strong opioids, non-steroids, local anesthesia, weak opioids, and placebo(95% confidence interval does not include 0). The results of network Meta-analysis showed that the order of analgesic effect in extracorporeal shock wave lithotripsy starting from the best was potent opioids, NSAIDS, EMLA and weak opioids(ranking probability: 0.7719, 0.7657, 0.4141, 0.4059, 0.1425).Conclusion:Potent opioids, NSAIDS, EMLA and weak opioids can achieve good analgesic effect in ESWL compared with placebo, and the order of analgesic effect in extracorporeal shock wave lithotripsy starting from the best is potent opioids, NSAIDS, EMLA and weak opioids.
