Abstract:OBJECTIVE:To study the clinicopathologic features, differential diagnosis and prognosis of renal cell carcinoma associated with Xp11.2 translocations/TFE3 gene fusions.METHODS:The histopathologic findings and immunophenotype of 11 cases of renal cell carcinoma associated with Xp11.2 translocations/TFE3 gene fusions were studied. Follow-up data (ranged from 10 to 112 months) were also analyzed.RESULTS:There were a total of 7 females and 4 males. The age of patients ranged from 8 to 26 years (mean = 16.3 years). The diameter of the tumors varied from 2.5 to 6.0 cm. Histologically, two morphologic patterns were seen. The first pattern consisted of alveolar, papillary or nested architecture. The tumor cells contained voluminous, clear to eosinophilic cytoplasm, distinct cell borders, vesicular chromatin, and prominent nucleoli. Psammoma bodies were frequently found and could be abundant. In contrast, the second pattern was composed of nested and compact architecture. The tumor cells possessed less abundant cytoplasm and inconspicuous nucleoli. Few psammoma bodies were detected. Immunohistochemical study showed that all cases strongly expressed TFE3, CD10 and P504s. Variable positivity for pan-cytokeratin, epithelial membrane antigen and vimentin was also noted. None of them expressed CK7, Ksp-cadherin and CD117.CONCLUSIONS:Renal cell carcinoma associated with Xp11.2 translocations/TFE3 gene fusions is a newly described but rarely encountered subtype of renal cell carcinoma. Pathologic diagnosis can be established when taken age of the patients, histopathologic findings and immunoreactivity for TFE3 protein into consideration.

Abstract:Xp11.2 translocation renal cell carcinoma (RCC) with transcription factor E3 (TFE3) gene fusion is a rare tumor, and the prognosis of this tumor is poorer compared with that of other subtypes of RCC. The patient presented herein was a 70-year-old man who presented with a solid mass sized ~8.2×6.1 cm in the right kidney and underwent radical right nephrectomy. Following pathological and immunohistochemical (IHC) examination and fluorescent in situ hybridization (FISH), the patient was diagnosed with Xp11.2 translocation RCC with TFE3 gene fusion. These tumors are more commonly encountered in children rather than in adults, and adult Xp11.2 translocation RCC is associated with a poorer prognosis compared with its pediatric counterpart. IHC assay and FISH are important diagnostic methods. However, there is currently no established effective treatment for Xp11.2 RCC.
Renal cell carcinoma associated with Xp11.2 translocation/TFE3 gene fusion: clinicopathological analysis of 15 cases

Abstract:Objective To investigate the clinicopathological features, diagnosis, and differential diagnosis of Xp11.2 translocation/ TFE3 fusion-related renal cell carcinoma. Methods A total of 15 cases of Xp11.2 translocation/ TFE3 fusion-related renal cell carcinoma were analyzed with regard to clinical data, histology, immunohistochemistry, and fluorescence in situ hybridization (FISH) findings. Results Six patients were male and the other nine were female. Their mean age was (31.3±9.3) years. The main clinical manifestations were hematuria and waist pain. Mean tumor diameter was 5.4 cm (range, 1.5-15 cm). Ten cases of the gross specimens were solid and had a grey, yellow, or tan cut surface. Five cases of the gross specimens were cystic-solid. Microscopic examination showed that the distinctive histological pattern was papillary, nested, and acini-like structure. The tumor cells had a clear boundary, eosinophilic or clear cytoplasm, and prominent nucleoli. And in 4 cases, psammoma bodies were observed. Immunohistochemistry results showed that TFE3, PAX-8, and CD10 were diffusely expressed in all cases. The expression of AE1/AE3, vimentin, HMB45, and AMACR/P504S varied among cases. The expression of CAIX, CK7, and CD117 was negative in all cases. All cases showed positive results in the FISH assay. Conclusion Xp11.2 translocation/TFE3 gene fusion-associated renal cell carcinoma is a rare renal malignancy and mainly occurs in females. Its diagnosis relies mainly on histopathology, immunohistochemistry, and FISH assay. Abstract:Renal carcinomas associated with Xp11.2 translocations/TFE3 gene fusions are cognized as a new subtype of renal malignancy in recent years, resulted in gene fusion products between the helix-loop-helix leucine zipper transcription factor (TFE3) and the alveolar soft part sarcoma locus (ASPL), along with several other identified genes, including PRCC, PSF, CLTC and NonO. The incidence of renal carcinomas associated with Xp11.2 translocations/TFE3 gene fusions is low, mainly influence children and adolescents. For the moment, the diagnosis of this carcinoma has no uniform standards. Hematuria is frequent, CT shows a significant enhancement during medullary period. Characteristic of pathology relies mainly on the cell morphology and psammoma bodies, strong TFE3 immunoreactivity, fluorescence in situ hybridization detection and karyotype analysis. The best choice for the treatment is radical nephrectomy, targeted therapy can prolong patient’s overall survival if it was necessary. Prognosis of this carcinoma is poorer in adults, long-term postoperative follow-up is requisite.

Abstract:Objective To investigate the clinical and pathological characteristics of Xp11.2 translocation/TFE3 gene fusion-associated renal cell carcinoma.Methods Between Jan 2009 and Jul 2013,37 patients with Xp1 1.2 translocation/TFE3 gene fusion-associated renal cell carcinoma,consisting of 17 males and 20 females,were treated in our institution.The median age was 22 years old,which ranged from 3 to 73 years old.The clinical symptoms included painless gross hematuria and (or) flank pain in 18 cases,abdominal mass in 3 cases and no significant clinical presentation in 16 cases.3 patients were diagnosed with distant metastasis,including cervical metastasis in 1 case and hepatic metastasis in 2 cases.All patients were treated with surgical treatment,of which 29 patients received radical nephrectomy,4 patients received nephron sparing surgery and 4 patients received palliative nephrectomy.Of the 37 patients,10 cases received adjuvant sorafenib or sunitinib target therapy after surgery.Results The final pathological diagnosis was Xp11.2 translocation/TFE3 gene fusion-associated renal cell carcinoma in all patients.The maximum diameter of tumor ranged from 1 to 15 cm (mean 6.5 cm).The pathological examination showed the tumor cells contained eosinophilic cytoplasm with psammoma bodies in renal interstitial tissue.Immunohistochemical examination showed that all patients were positive for TFE3,30 patients were positive for CD10,28 patients were positive for P504S and variably positive for CK7,CK8,Vimentin and EMA.The mean follow-up duration was 22 months (range,1-55 months).33 patients survive and 4 patients died due to the tumor recurrence and systemic metastasis.As being treated with adjuvant target therapy,the patient with cervical metastasis has maintained stable,and another 2 patients with hepatic metastases have achieved partial remission.Conclusions Xp11.2 translocation/TFE3 gene fusion-associated renal cell carcinoma is a newly defined rare entity with a high predominance of young patients.The diagnosis mainly depends on patient's age,histopathological characteristics,and TFE3-positive immune markers.Patients with metastatic Xp1 1.2 translocation/TFE3 gene fusion-associated renal cell carcinoma could benefit from the target therapy.

Abstract:Aim: To analyze the clinicopathological features of renal cell carcinoma (RCC) associated with Xp11.2 translocation/TFE3 gene fusions (Xp11.2 RCC) in our institution. Materials & methods: We screened 983 RCC specimens. TFE3 immunohistochemical staining and FISH assay confirmed 22 Xp11.2 RCCs out of 65 suspicious cases. Clinicopathological and treatment outcomes of 22 patients were retrospectively analyzed. Results: In total, 22 patients included 13 females and nine males with a mean age of 27 years. Ten patients showed gross hematuria. Treatments included surgeries, immunotherapy and molecular-targeted therapy. Seven cases were at stage III/IV and four cases had tumor thrombosis or distant metastasis. During a median follow-up of 34 months, 19 patients were alive while three died of distant metastasis. Conclusion: Xp11.2 RCC is rare and FISH proved a useful diagnostic tool. Surgical resection achieved favorable outcome for early disease. Adult patients at advanced stage had poorer outcomes even with postoperative adjuvant therapy.

Abstract:Objective To analyze the clinical and pathological features of renal carcinoma associated with Xp11.2 translocation/TFE3 gene fusion.Methods Clinical and pathological data of 13 patients with Xp11.2 translocation/TFE3 gene fusion-related renal carcinoma were retrospectively analyzed.Results Of 13 patients,10 cases had painless gross hematuria as the first symptom and the high stage of tumors.Imaging of CT enhanced amplitude was(31.85±14.65) HU.The operations included open nephrectomy in 7 cases,laparoscopic radical nephrectomy in 5 cases and laparoscopic radiofrequency ablation in 1 case.Histologically,the most characteristic manefestation was the papillary structures composed by transparent cells,which was accompanied by nest structure consisted of eosinophilic granular cytoplasm of tumor cells.The high expression of TFE3 and the expressions of CD10,CK,EMA and vimentin in different degrees were detected in 13 cases.Conclusion Renal carcinoma associated with Xp11.2 translocation TFE3 gene fusion has a low incidence.The amplitude of enhanced CT imaging for Xp11.2 translocation/TFE3 gene fusion-related renal carcinoma is smaller than that of ordinary clear cell renal carcinoma.The diagnosis mainly depends on the characteristic histopathological changes and TFE3-positive immune markers.

Abstract:Objective To investigate the CT findings of renal cell carcinoma associated with Xp11. 2 translocation/TFE3 gene fusion and to improve the understanding of this entity. Methods Sixteen renal cell carcinoma associated with Xp11. 2 transloca-tion/TFE3 gene fusion and 71 other subtype renal cell carcinoma proven by histopathology were retrospectively collected and their demographics and CT findings were analyzed and compared. Results The average ages were 31. 2 years for 16 patients with Xp11. 2 translocation/TFE3 gene fusion renal cell carcinoma. Twelve masses were located in right kidney and 4 in left kidney (all P >0. 05). The maximal diameter of Xp11. 2 translocation/TFE3 gene fusion renal cell carcinoma was larger than papillary renal cell carcinoma ( P =0. 003) and had no difference with other subtype renal cell carcinoma ( P > 0. 05). The prevalence of in-tramural hemorrhage was the highest among 4 subtype renal cell carcinoma (all P <0. 05). Necrosis was significantly lower than clear cell renal cell carcinoma ( P =0. 015) but no difference with other subtype renal cell carcinoma ( P =0. 183, P =0. 723). Intramural calcifications were significantly higher than clear cell renal cell carcinoma ( P <0. 001) and papillary renal cell carcinoma ( P =0. 001) and had no difference with chromophobe cell renal carcinoma ( P =0. 154). Metastasis was ob-served more often than papillary renal cell carcinoma ( P =0. 050) and chromophobe cell renal carcinoma ( P =0. 043) but had no difference with clear cell renal cell carcinoma ( P =0. 108). Enhancement pattern of Xp11. 2 translocation/TFE3 gene fusion renal cell carcinoma was persistent enhancement type, which was different from other subtype renal cell carcinoma. Conclusion Renal cell carcinoma associated with Xp11. 2 translocation/TFE3 gene fusion has some typical CT findings, while comprehensive analysis can improve its diagnosis.

Abstract:Objectives To investigate the treatments and research progress of renal carcinomas associated with Xp11.2 translocation/ TFE3 gene fusions.Methods From January 2009 to June 2015,18 cases of Xp11.2 translocation renal cell cancer (RCC) were treated in our hospital.Clinical manifestations,characteristics of RCC and surgical treatments were retrospectively analyzed.Results Most of the patients were under 45 years old.All of RCC were unilateral.Nephron sparing surgery or radical nephrectomy was performed according to the tumor size and the location.All patients had not complication of operation,postoperative reactions.Postoperative follow-up time was 6 ～ 80 months.2 cases with lung metastases postoperatively,1 case with death after 35 months happened.Conclusions Xp1 1.2 translocation kidney cancer occurs in young patients,is a kind of new classificational independently subtype of kidney cancer,which has a unique genetic type,high malignant degree.The surgical treatment is the first choice for the disease.

Abstract:Objective: To analyze the clinical characteristics, treatment methods and prognosis of renal cell carcinoma associated with Xp11.2 translocation/TFE3 gene fusions (Xp11.2 tRCC). Methods: From January 2007 to February 2018, 48 patients were diagnosed with Xp11.2 tRCC at Nanjing Drum Tower Hospital. The epidemiological features, treatment methods and long-term follow-up results were retrospectively reviewed. Results: Of the 48 patients, 20 cases were female and 28 cases were male, aged from 2 to 72 years. Gross hematuria and flank pain were the most frequent symptoms, which occurred on 14 cases and 8 cases respectively. The mean tumor size of 48 cases was (5.3±2.5)cm. Among the 34 cases who were classified as stageⅠ/Ⅱ, 14 cases received laparoscopic nephron-sparing surgery(NSS)and 20 cases received radical nephrectomy(RN). The other 14 cases who were classified as stage Ⅲ/Ⅳ received RN but one case received target therapy. On univariate analysis, tumor diameter, adjuvant treatment, AJCC stage, lymph node metastasis and vein tumor thrombosis showed association with progression-free survival (PFS) and overall survival (OS) (P<0.05). Multivariate analysis indicated that AJCC stage (P=0.023, 95% CI: 0.048-0.081)and vein tumor thrombosis (P=0.046, 95% CI: 1.004-1.590)were independent prognostic factors of PFS. Conclusions: Xp11.2 tRCC mainly occurs in females. RN was the major method for Xp11.2 tRCC. However, NSS can also receive satisficed results for stage T1a case. High AJCC stage and the occurrence of vein tumor thrombosis indicated poor prognosis.
Postoperative recurrence of adult renal cell carcinoma associated with Xp11.2 translocation/TFE3 gene fusion.

Abstract:Objective To analyze the postoperative recurrence of renal cell carcinoma associated with Xp11.2 translocation/TFE3 gene fusion (Xp11.2 tRCC). Methods This retrospective study was approved by the institutional review board and performed in accordance with the ethical standards established by the institution. Demographic, clinical, pathological, and follow-up data were compiled for the study cohort. Results During a mean follow-up of 41.3 months (range, 3-104 months), 8 of 34 patients with Xp11.2 tRCC were confirmed to have recurrence. Three of these patients died with poor outcomes due to a vena cava tumor embolus, and one died of distant metastasis 48 months after the initial nephrectomy during which lymph node metastasis was found. Three patients survived after cytoreduction surgery. One patient was diagnosed with lung metastasis 11 months postoperatively. Conclusions The TNM classification provides significant prognostic information for Xp11.2 tRCC. A relatively active surveillance algorithm is recommended, and cytoreduction surgery is an effective approach for recurrent Xp11.2 tRCC. Larger studies are required to more extensively investigate the recurrence of these potentially aggressive tumors.

Abstract:To investigate the clinical characteristics, treatments and prognosis of renal cell carcinoma associated with Xp11.2 translocation/TFE3 gene fusions (Xp11.2 tRCC), the epidemiological features and treatment results of 34 cases of Xp11.2 tRCC, which were diagnosed by immunohistochemistry staining of TFE3 and fluorescence in situ hybridization at our center, were retrospectively reviewed. The 34 patients included 21 females and 13 males aged 3 to 64 years (median age: 27 years). Four patients were children or adolescents (<18 years of age), and 26 patients were young or middle-aged adults (18-45 years). Radical nephrectomy was performed on 25 patients. Laparoscopic nephron-sparing surgery was performed on 9 patients who presented with an isolated mass with a small diameter (<7 cm) and well-defined boundary on computed tomography imaging. Postoperative staging showed that 25 cases (73.53%) were at stage I/II, while 9 cases (26.47%) were at stage III/IV. All stage I/II patients received a favorable prognosis with a three-year overall survival rate of 100%, including the patients who underwent laparoscopic nephron-sparing surgery. With the exception of 2 children, the other 7 stage III/IV patients died or developed recurrence with a median follow-up of 29 months. On univariate analysis, maximum diameter, adjuvant treatment, TNM stage, lymph node metastasis, inferior vena cava tumor thrombosis and tumor boundary were identified as statistically significant factors impacting survival (P<0.05). Multivariate analysis indicated that TNM stage and inferior vena cava tumor thrombosis were independent prognostic factors (P<0.05). In conclusion, Xp11.2 tRCC is a rare subtype of renal cell carcinoma that mainly occurs in young females. Nephron-sparing surgery was confirmed effective preliminarily in the treatment of small Xp11.2 tRCCs with clear rims. Advanced TNM stage and inferior vena cava tumor thrombosis were associated with poor prognosis.

Abstract:PURPOSE:To retrospectively analyze MRI and computed tomographic (CT) findings from renal carcinomas associated with Xp11.2 translocations/TFE3 gene fusions (Xp11-RCC). MATERIALS AND METHODS:Institutional review board permission was obtained to review patient medical records, and the requirement for informed consent was waved . The clinical and MRI/CT features of five cases with Xp11-RCC that were confirmed by pathology were analyzed retrospectively. The image characteristics included the lesion location and size, contribution of cystic and solid components, intratumoral necrosis or hemorrhage, invasion of perinephric tissue and renal sinus, lymphadenopathy, major venous or arterial vascular invasion, pattern of the tumor growth, intratumor calcification and lipids, homogeneity of SI on T2-weighted images, attenuation and SI of the mass with respect to the normal renal cortex on precontrast and contrasted CT/MRI images, tumor SIs, tumor attenuations and tumor-to-cortex indices, homogeneity of enhancement on the contrasted images. RESULTS:The mean age was 32 years (range, 15-47 years). Most patients (4/5) were women. All tumors showed a cortical location. The average tumor size was 9 cm (range, 4-18 cm). Four tumors comprised a predominantly solid lesion with focal necrosis, and one tumor comprised a solid lesion with significant necrosis. All tumors showed intertumor hemorrhage, infiltrative growth and invasion of the perirenal adipose/renal sinus. Four cases showed retroperitoneal lymphadenopathy, of which one case showed simultaneous mediastinal and supraclavicular lymphadenopathy. All tumors from four cases showed mild hyperintensity on T1-weighted MRI images, and three tumors showed hypointensity on T2-weighted MRI images relative to the renal cortex except for 1 tumor that showed significant hemorrhage and a relative hyperintensity. For 3 cases who were imaged with CT, two tumors imaged using nonenhanced CT images showed mild hyperdensity relative to the renal cortex. Calcification was noted in all three tumors. All tumors showed mild, persistent enhancement. CONCLUSION:Typical Xp11-RCC manifests as an advanced, solid renal mass with mild persistent enhancement, a prevalence of intertumor hemorrhage/calcification, and a cortical epicenter location. The predilection for children and young adults is a useful clinical feature when confirming a diagnosis of Xp11-RCC.

Abstract:Objective To investigate the dynamic contrast-enhanced computed tomography (CT) characteristics of renal cell carcinoma associated with Xp11.2 translocation and TFE gene fusion (Xp11.2 RCC) by comparison with clear cell renal cell carcinoma (CCRCC). Methods Dynamic contrast-enhanced CT images and clinical and pathological records of 20 adult patients with Xp11.2 RCC confirmed by TFE3 immunohistochemical and fluorescence in situ hybridization assay were retrospectively analyzed and compared with the findings of 21 contemporary CCRCCs. Results Renal cell carcinoma associated with Xp11.2 translocation and TFE gene fusions often occurred in young (30.6 8.6 years) patients with hematuria (9/20). They presented as well-defined (17/20) cystic-solid (17/20) mass with hemorrhage (8/20) and circular/rim calcifications (6/20). Dynamic contrast-enhanced CT showed heterogeneous moderate prolonged enhancement. A tumor-to-cortex attenuation ratio in corticomedullary phase less than 0.62 gave a sensitivity of 90.0% and a specificity of 92.9% in differentiating Xp11.2 RCC from CCRCC (area under the receiver operating characteristic curve = 0.957, P < 0.001). Conclusions Computed tomographic characteristics and dynamic contrast-enhanced patterns and index can differentiate Xp11.2 RCC from CCRCC.
Genomic Alterations and Tumor Mutational Features of Chinese Xp11.2 Translocation Renal Cell Carcinoma Patients

Abstract:736 Background: Xp11.2 translocation renal cell carcinoma (Xp11.2 RCC) is a rare subtype of RCC which occurs predominantly in children and adolescents. Xp11.2 RCC is characterized by the gene fusions of transcription factor E3 (TFE3). Since there is a high false-positive and false-negative rate of immunohistochemistry (IHC) for TFE3 and an uncertainty of TFE3 fusion partners by fluorescence in situ hybridization (FISH), a genetic approach, such as DNA sequencing, is necessary for auxiliary diagnosis. Methods: Thirty Chinese Xp11.2 RCC patients with positive TEF3 fusion diagnosed by FISH were involved in this analysis. Formalin-fixed, paraffin-embedded (FFPE) samples were collected for a next-generation sequencing (NGS)-based 576 gene panel assay. Genomic alterations including single base substitution, copy number variations, gene fusions, and rearrangements were assessed. Tumor mutational burden (TMB) and microsatellite instability (MSI) status were also analyzed by an NGS algorithm. Results: The 30 patients in the Xp11.2 RCC cohort included 13 males and 17 females, and had a median age of 30 years. TFE3 fusions of these patients were confirmed by both FISH and NGS. Out of the 30 Xp11.2 RCC patients, 2 patients (6.7%) showed negative TFE3 IHC expression and 4 patients (13.3%) showed equivocal TFE3 IHC expression. ASPL (9/30), PRCC (5/30), PSF (5/30), and NONO (3/30) were the most common partners of TFE3 fusion in this cohort. Moreover, we identified uncommon or novel partner genes by NGS, including ZNF627 (1/30), PTPN12 (1/30), PBM10 (1/30), PARP14 (1/30), MED15 (2/30), MATR3 (1/30), LUC7L3 (1/30), KHSRP (1/30), and EWSR1 (1/30). Based on the NGS results, several actionable genomic alterations including CDKN2A, BRCA2, and ATM were found in this study. All samples were identified as microsatellite stable (MSS). The median TMB of the entire cohort was 1.35 muts/Mb (range, 0-9.2 muts/Mb). Conclusions: In summary, we identified the partner genes of TFE3 fusion in 30 Chinese Xp11.2 RCC patients by NGS. Ten uncommon or novel partner genes (33.3%) of TFE3 were identified in this study. Overall, our results provide evidence for diagnosis and therapeutic strategies for Xp11.2 RCC patients.

Abstract:Background: MED15-TFE3 renal cell carcinoma is a rare subtype of the Xp11 translocation renal cell carcinoma. To data, only 14 cases of MED15-TFE3 renal cell carcinoma have been reported worldwide.Methods: MED15-TFE3 gene fusion was identified in two cases of Xp11 translocation renal cell carcinoma by using RNA-sequencing. We reviewed and summarized the clinicopathological characteristics, imaging features and prognosis of the MED15-TFE3 renal cell carcinoma. Results: Morphologically, MED15-TFE3 RCC have the typical cystic-solid (mostly cystic) structure. The tumor cells had round nuclei with inconspicuous nucleoli and abundant clear or eosinophilic cytoplasm. Psammoma bodies were randomly distributed within the stroma. Immunohistochemically, the typical TFE3 positive staining could be observed in both cases. The split red-green signal also revealed the positive results in fluorescence in situ hybridization (FISH) assay. Finally, RNA sequencing detected the existence of MED15-TFE3 gene fusion. One pediatric patient developed renal hilar lymph node metastasis before surgery. However, during the follow-up period, no disease recurrence and metastasis happened to both patients. Conclusions: Different from the traditional Xp11 translocation RCC, MED15-TFE3 RCC may have the distinctive cystic-solid structure and superior prognosis. These findings enhanced our understanding of the heterogeneity in Xp11 translocation RCC.

Abstract:Xp11 translocation renal cell carcinomas are characterized by several different translocations involving the TFE3 gene. Tumors with different specific gene fusions may have different clinicopathological manifestations. Fewer than 10 renal cell carcinoma cases with NONO-TFE3 have been described. Here we examined eight additional cases of this rare tumor using clinicopathological, immunohistochemical, and molecular analyses. The male-to-female ratio of our study cohort was 1:1, and the median age was 30 years. The most distinctive feature of the tumors was that they exhibited glandular/tubular or papillary architecture that was lined with small-to-medium cuboidal to high columnar cells with indistinct cell borders and an abundantly clear or flocculent eosinophilic cytoplasm. The nuclei were oriented toward the luminal surface and were round and uniform in shape, which resulted in the appearance of secretory endometrioid subnuclear vacuolization. The distinct glandular/tubular or papillary architecture was often accompanied by sheets of epithelial cells that presented a biphasic pattern. Immunohistochemically, all eight cases demonstrated moderate (2+) or strong (3+) positive staining for TFE3, CD10, RCC marker, and PAX-8. None of the tumors were immunoreactive for CK7, Cathepsin K, Melan-A, HMB45, Ksp-cadherin, Vimentin, CA9, 34βE12 or CD117. NONO-TFE3 fusion transcripts were identified in six cases by RT-PCR. All eight cases showed equivocal split signals with a distance of nearly 2 signal diameters and sometimes had false-negative results. Furthermore, we developed a fluorescence in situ hybridization (FISH) assay to serve as an adjunct diagnostic tool for the detection of the NONO-TFE3 fusion gene and used this method to detect the fusion gene in all eight cases. Long-term follow-up (range, 10–102 months) was available for 7 patients. All 7 patients were alive with no evidence of recurrent disease or disease progression after their initial resection. This report adds to the known data regarding NONO-TFE3 renal cell carcinoma.

Abstract:Both Xp11 translocation renal cell carcinomas and the corresponding mesenchymal neoplasms are characterized by a variety of gene fusions involving TFE3. It has been known that tumors with different gene fusions may have different clinicopathologic features; however, further in-depth investigations of subtyping Xp11 translocation-associated cancers are needed in order to explore more meaningful clinicopathologic correlations. A total of 22 unusual cases of Xp11 translocation-associated cancers were selected for the current study; 20 cases were further analyzed by RNA sequencing to explore their TFE3 gene fusion partners. RNA sequencing identified 17 of 20 cases (85%) with TFE3-associated gene fusions, including 4 ASPSCR1/ASPL-TFE3, 3 PRCC-TFE3, 3 SFPQ/PSF-TFE3, 1 NONO-TFE3, 4 MED15-TFE3, 1 MATR3-TFE3, and 1 FUBP1-TFE3. The results have been verified by fusion fluorescence in situ hybridization (FISH) assays or reverse transcriptase polymerase chain reaction (RT-PCR). The remaining 2 cases with specific pathologic features highly suggestive of MED15-TFE3 renal cell carcinoma were identified by fusion FISH assay. We provide the detailed morphologic and immunophenotypic description of the MED15-TFE3 renal cell carcinomas, which frequently demonstrate extensively cystic architecture, similar to multilocular cystic renal neoplasm of low malignant potential, and expressed cathepsin K and melanotic biomarker Melan A. This is the first time to correlate the MED15-TFE3 renal cell carcinoma with specific clinicopathologic features. We also report the first case of the corresponding mesenchymal neoplasm with MED15-TFE3 gene fusion. Additional novel TFE3 gene fusion partners, MATR3 and FUBP1, were identified. Cases with ASPSCR1-TFE3, SFPQ-TFE3, PRCC-TFE3, and NONO-TFE3 gene fusion showed a wide variability in morphologic features, including invasive tubulopapillary pattern simulating collecting duct carcinoma, extensive calcification and ossification, and overlapping and high columnar cells with nuclear grooves mimicking tall cell variant of papillary thyroid carcinoma. Furthermore, we respectively evaluated the ability of TFE3 immunohistochemistry, TFE3 FISH, RT-PCR, and RNA sequencing to subclassify Xp11 translocation-associated cancers. In summary, our study expands the list of TFE3 gene fusion partners and the clinicopathologic features of Xp11 translocation-associated cancers, and highlights the importance of subtyping Xp11 translocation-associated cancers combining morphology, immunohistochemistry, and multiple molecular techniques.

Abstract:PURPOSE:To characterize Xp11.2 translocation renal cell carcinoma (RCC) using magnetic resonance imaging (MRI) and computed tomography (CT). METHODS:This study retrospectively collected the MRI and CT data of twelve patients with Xp11.2 translocation RCC confirmed by pathology. Nine cases underwent dynamic contrast-enhanced MRI (DCE-MRI) and 6 cases underwent CT, of which 3 cases underwent MRI and CT simultaneously. The MRI and CT findings were analyzed in regard to tumor position, size, hemorrhagic, cystic or necrotic components, calcification, tumor density, signal intensity and enhancement features. RESULTS:The age of the 12 patients ranged from 13 to 46 years (mean age: 23 years). T2WI revealed heterogeneous intensity, hyper-intensity, and slight hypo-intensity in 6 cases, 2 cases, and 1 case, respectively. On DCE-MR images, mild, moderate, and marked rim enhancement of the tumor in the corticomedullary phase (CMP) were observed in 1, 6, and 2 cases, respectively. The tumor parenchyma showed iso-attenuation (n = 4) or slight hyper-attenuation (n = 1) compared to the normal renal cortex on non-contrast CT images. Imaging findings were suggestive of hemorrhage (n = 4) or necrosis (n = 8) in the tumors, and there was evidence of calcification in 8 cases by CT (n = 3) and pathology (n = 8). On dynamic contrast-enhanced CT images, 3 cases and 1 case manifested moderate and strong CMP enhancement, respectively. Nine tumors by MRI and 4 tumors by CT showed prolonged enhancement. Three neoplasms presented at stage I, 2 at stage II, 3 at stage III, and 4 at stage IV according the 2010 AJCC staging criteria. CONCLUSIONS:XP11.2 translocation RCC should be considered when a child or young adult patient presents with a renal tumor with heterogeneous features such as hemorrhage, necrosis, cystic changes, and calcification on CT and MRI and/or is accompanied by metastatic evidence.
