Preliminary results of minimally invasive transforaminal lumbar interbody fusion with bilateral decompression via unilateral approach in the treatment of degenerative lumbar canal stenosis

Abstract:Objective To investigate the early clinical effect of minimally invasive transforaminal lumbar interbody fusion(MIS-TLIF) with bilateral decompression via unilateral approach in the treatment of degenerative lumbar canal stenosis.Methods Total 32 patients of degenerative lumbar canal stenosis were treated with MIS-TLIF with bilateral decompression via unilateral approach.Operative time and the operative blood loss were recorded as well as the hospitalization time,and the postoperative complications;imaging changes were observed before and after surgery;VAS and ODI were used to evaluate the clinical effect.MacNab criterion at last follow-up was used to evaluate the clinical effect.Results The operations were performed successfully in all patients, without dural tear or nerve root injury during operation.The operative time ranged from 70~140(97.0±11.7)min, the operative blood loss was 60~380(140.5±40.7)ml, and the hospitalization time was 3~6(4.6±1.4)d.All 32 cases were followed up for 9~18 months.All patients with postoperative CT examination showed effectively expanded vertebral canal.The neurologic symptom and signs in all cases had significant improvement.The VAS and ODI were significantly improved at 3 days postoperation and last follow-up when they were compared with preoperative ones(P<0.05).According to Macnab criterion at last follow-up, the results were excellent in 18 cases, good in 12 cases, and fair in 2,the excellent and good rate was 93.8%.Conclusions It suggests that MIS-TLIF with bilateral decompression via unilateral approach in the treatment of degenerative lumbar canal stenosis may yield good short-term functional results, further follow-up is necessary to evaluate mid-and long-term outcome.
