Analysis of risk factors of surgical site infection after posterior minimally invasive lumbar discectomy

Abstract:Objective To analyze the risk factors of surgical site infection (SSI) after posterior minimally invasive lumbar discectomy.Methods Data of 536 patients who were suffered lumbar disc herniation and underwent lumbar minimally invasive surgery including sex,age,height and weight index (BMI),smoking,alcohol use,respiratory tract infections,combined diseases (diabetes),ASA scores,incision length,surgery duration,number of surgical segments,bleeding volume,use of antibiotics,implantation and hospitalization duration before surgery were analysised retrospectively to evaluate their influences for incidence of SSI.Results There were 11 cases (incidence rate 2.05%) of SSI including 2 cases of microendoscopic discectomy (MED),9 cases of minimally invasive approach combined with nucleus pulposus extraction under microscope and 1 case of intervertebral infection,10 cases of incision infection.All incision infection were deep infection.The clinical manifestations were fever (6 cases),incision drainage (7 cases),wound dehiscence (10 cases).11 patients were cured by wound debridement and lavage catheter.Display data analysis,sex,age,height and weight index (BMI),smoking,alcohol use,respiratory tract infections,combined diseases (diabetes),ASA scores,bleeding volume,use of antibiotics,implantation,hospitalization duration before surgery were not associated with SSI;length of the incision (>4 cm),surgery duration (>3 h),number of surgical segments (multiple segment surgery) improved the incidence rate of SSI significantly.Conclusions Length of the incision (>4 cm),surgery duration (>3 h) and number of surgical segments (multiple segment surgery) are the high risk factors for SSI of posterior minimally invasive lumbar discectomy.
