The value of radiomic features to evaluate microvascular invasion in hepatocellular carcinoma preoperatively based on computed tomography

Abstract:Objective:To investigate the value of preoperative tumor CT radiologic features and texture features on microvascular invasion (MVI) in hepatocellular carcinoma (HCC).Methods:HCC patients who underwent partial hepatectomy in General Hospital of Ningxia Medical University from January 2017 to June 2019 were retrospectively included. According to the clinical and pathological features, they were divided into MVI positive group and MVI negative group. Patients' preoperative imaging features and clinical data were analyzed by univariate and multivariate analysis, then the independent predictors of MVI were explored. A total of 208 texture features, classified into 6 types, were extracted from two groups HCC patients' CT images in arterial phase and portal vein phase using a self-compiled algorithm in MATLAB. Least Absolute Shrinkage and Selection operator (LASSO) was used to reduce the dimensionality with a method of 10-fold cross-validation, and to select the CT texture features related to MVI. Receiver opearating characteristic (ROC) curve analysis was performed to evaluate the efficacy of CT radiologic and texture features to diagnose MVI.Results:A total of 71 HCC patients were enrolled, including 50 males and 21 females, aged (53.89±11.29) years old, 42 patients in the MVI positive group (59.2%), and 29 patients (40.8%) in the MVI negative group. Nonsmooth tumor margins, tumor dimension, intratumoral arteries, peritumoral enhancement and proximity of tumors to hepatic capsule were found to be correlated with MVI in tumor CT radiologic features ( P<0.05). Nonsmooth tumor margins ( OR=13.978, 95% CI: 2.890-67.609) and intratumoral arteries ( OR=14.504, 95% CI: 2.629-80.012) were independent risk factor which can predictor for MVI. The numerical value of logAguanxing and Ventropy_LH1 in the MVI positive group was lower than them in the MVI negative group [(4.04±0.11) vs (4.13±0.11) & (1.43±0.22) vs (2.12±0.33)], with statistically significant difference ( P<0.05). LogAguanxing and Ventropy_LH1 were correlated with MVI in tumor CT texture features ( P<0.05). The area under the curve (AUC) of evaluating MVI combined with tumor radiologic features (nonsmooth tumor margins & intratumoral arteries) was 0.870 (95% CI: 0.769-0.938), the sensitivity was 76.19%, and the specificity was 79.31%; The AUC of evaluating MVI combined with tumor texture features (logAguanxing & Ventropy_LH1) was 0.890 (95% CI: 0.794-0.952), the sensitivity was 82.50%, and the specificity was 87.10%. The AUC between the two combined diagnostic methods has no statistically significant difference ( P>0.05). Conclusions:In the tumor CT radiologic features of HCC, nonsmooth tumor margins and intratumoral arteries can be used as independent predictive factors for MVI. In the tumor CT texture features of HCC, LogAguanxing and Ventropy_LH1 were correlated with MVI. The preoperative tumor CT radiologic features and texture features of HCC are of great value in evaluating MVI.
