Mid-term clinical observation of interspinous process dynamic fixation system for adjacent segment lumbar disc degeneration after interbody fusion by posterior approach

Abstract:Objective To observe the prevention and treatment of adjacent segment lumbar disc degeneration after interbody fusion with Coflex interspinous process dynamic fixation by posterior approach. Methods Forty patients with lumbar disc herniation, who were treated with surgery from June 2011 to December 2012 in our hospital, were analyzed retrospectively. There were 20 males and 20 females whose age ranged from 20 to 38 years ( mean:28.6 years ). All patients received posterior lumbar discotomy and interbody fusion at L5- S1, and Coflex at L4-5. The values of visual analog scale ( VAS ), Oswestry disability index ( ODI ), disc height index ( DHI ) and relative signal intensity ( RSI ) at L4-5were compared preoperatively and postoperatively. Effects of Coflex for the degeneration of L4-5were evaluated. Results All 40 cases ( ranged 48 - 72 months, averaged 60.2 months ) were followed up. Clinical symptoms improved significantly. The average operation time was 104 min and the average blood loss was 164 ml. VAS and ODI before operation were ( 8.4 ± 0.9 ), ( 44.8 ± 2.8 ) respectively. VAS at 3 months, 6 months, 12 months after the operation and at last follow-up were ( 1.8 ± 0.5 ), ( 1.7 ± 0.4 ), ( 1.6 ± 0.3 ), ( 1.4 ± 0.3 ). ODI at 3 months, 6 months, 12 months after the operation and at last follow-up were ( 3.8 ± 0.4 ), ( 2.3 ± 0.4 ), ( 2.1 ± 0.3 ), ( 1.8 ± 0.2 ). The values of VAS and ODI were significantly statistically different ( P < 0.05 ), while DHI and RSI not ( P > 0.05 ) preoperatively and postoperatively. Conclusions Posterior lumbar discotomy+interbody fusion could achieve significant symptomatic improvement. However, for the patients with pre-existing mild or moderate adjacent segment degeneration, the application of Coflex could effectively defer the degeneration of adjacent-level intervertebral discs.
