Laparoscopic radical gastrectomy for early gastric cancer

Abstract:Objective To evaluate laparoscopic radical gastrectomy for early gastric cancer. Methods Seventy nine patients with early gastric cancer undergone laparoscopic radical gastrectomy from October 2004 to December 2009 were retrospectively reviewed and analyzed for the surgical procedure,operative time,blood loss,time for passage of flatus,post-operative hospital stay,complications and pathology. Results All but 1 patients underwent laparoscopic gastrectomy and lymph nodes dissection successfully without conversion to open surgery,including 74 distal gastrectomy,2 proximal gastrectomy,and 2 total gastrectomy. D1+α,D1+β,and D2 lymph node dissections were performed in 34 cases,15 cases and 29 cases,respectively. The mean operative time was (202.9±45.6) min,blood loss was (144.5±146.5) mL,time for passage of flatus was (2.8±1.0) d and the post-operative hospital stay was (11.3±5.6) d. Operative complications such as anastomostic leakage,intra-abdominal bleeding and intestinal obstruction occurred in 8 patients (10.1%). The distances between tumor and resection margin were (4.0±1.9) cm proximally and (3.6±1.7) cm distally,and the mean number of dissected lymph node was 13.1±6.5,including 3 patients with lymph node metastasis. After 2-64 months follow-up,there was no tumor recurrence and metastasis. Conclusions Laparoscopic radical gastrectomy is a safe,feasible,effective and less invasive surgery for early gastric cancer.
