Transfrontotemporal Extradural Approach for Microsurgery on Cavernous Sinus Region Tumors

Abstract:Objective To report the experience in the transfrontotemporal extradural approach for microsurgery on 8 patients with tumors invading the cavernous sinus region. Methods The clinical materials including major clinical manifestations, preoperative CT and MRI features and operative techniques, and the outcomes of 8 patients with cavernous sinus tumors were retrospectively analysed. Results Total resection was achieved in 5 of the 8 patients and subtotal in 3 patients. Cranial nerve deficits were improved or remained unchanged by 90. 9% , and worsened postoperatively by 9. 1%. None of the patients had new cranial nerve deficits after the operation. There was no operative mortality or morbidity. Follow-up of 7 patients for 20-23 months revealed no tumor recurrence. Conclusion It is suggested that tumors invading the cavernous sinus region can be safely and radically removed by the transfrontotemporal extradural approach. And the familiarity with this approach can greatly reduce postoperative complications and mortality.
