Long-term Prognosis in Idiopathic Membranoproliferative Glomerulonephritis

Abstract:Objective:To investigate the long-term prognosis of patients with membranoproliferative glomerulonephritis (MPGN). Methodology:Seventy-four patients with MPGN were studied including both type Ⅰ and Ⅲ of 37 cases diagnosed by renal biopsy.Their clinical course and influence of clinical and histological factors on prognosis were analyzed.An inductive analysis on patients survival was made by using the Cox proportional hazard regression model,and by using a stepwise procedure in order to select only those factors that were significantly associated with survival.For each individual risk factor,an univariate descriptive analysis of survival was performed using the Kaplan-Meier method. Results:For time of 48.6 months follow-up(range from 13 to 152 moths),43 patients with MPGN(58.1%) had impaired of renal function,17.6% developed end-stage renal disease(ESRD).The overall 5-years and 8 year cumulative proportion renal survival rates were 90% and 64%,respectively.With multivariate analysis,the factors affecting renal prognosis were elevated serum creatinine at the time of biopsy and severe proteinuria(3.0 g/d) as well as hypertension.The survival rate was decreased significantly if there were focal crescent formation and/or severe tubuleinterstitial lesions.The steroids did not influence the prognosis of MPGN,while angiotensin converting enzyme inhibitor(ACEI) could delay progressive deterioration of renal function. Conclusion:MPGN is a progressive renal disease.Renal failure was correlated to the histological lesions,renal function state,proteinuria and hypertension at biopsy.The therapy with ACEI can improve the prognosis of MPGN but not with steroid.
