Visualized renal access based on ultra-mini percutaneous nephroscope:a new method for tract establishment of percutaneous nephrolithotomy

Abstract:Objective:To introduce a new method to establish percutaneous renal access by direct vision based on ultra-mini percutaneous nephroscope and explore its efficacy and safety.Method:The clinical data of 131 patients who underwent ultra-mini percutaneous nephrolithotomy from June 2015 to October 2016 were retrospectively analyzed.The patients were divided into two groups according to the use of visualization via ultra-mini percutaneous nephroscope.Group A consisted of 66 patients who underwent visualized percutaneous neprholithotomy and Goup B consisted of 65 patients who underwent traditional percutaneous nephrolithomy.There was no statistical difference between the two groups in sex,age,BMI,stone burden,type,distribution of stone and degree of hydronephrosis.Result:Compared to group A in which one case of perforation of the collective system occurred,the intraoperative complications on renal access were higher in group B including 4 false tracts and 4 perforations of collective system (12.3% vs.1.5%,P<0.05).The postoperatiove hospital stay was shorter in group A than that in group B](1.18±0.96) d vs.(1.54±-0.83) d,P<0.05].There was no significant difference in renal access construction time,operation time,hemoglobin drops,postopertative complications defined as the modified Clavien-Dindo classification and stone-free rates between the two groups (P>0.05).Conclusion:The visualized renal access based on ultra-mini percutaneous nephroscope is a method worth to be popularized,especially for those with no or mild hydronephrosis or filling stones who will be treated with percutaneous nephrolithotomy under ultrasound guidance.It helps to confirm the location and further adjustment of the needle sheath,enough length of the guidewire.It makes the establishment of the renal access safer and less complications such as false tract or perforation of the collective system.
