Surgical enlargement of cranial vault in infant craniosynostosis using bifrontal expending craniotomy

Abstract:Objective To deal with the problems of cranial vault narrowing, chronic increase of intracranial pressure, and forehead deformities in infant craniosynostosis. Methods Through bicoronal incision, we employed bifrontal-expanding craniotomy from frontal-orbit to the roof of skull extending to temporal area bilaterally. The superior frontal-orbital bar was made by transverse osteotomy across the nasal root and ethmoid. While the superior frontal-orbital bar was pushed forward slanting 30~40 degrees,the volume of anterior cranial fossa was enlarged and best contour of the frontal-nasal area was achieved. The bone pieces of forehead and the roof of skull were split and remodeled. Then reposition and reset of the new forehead followed for a better contour and the remaining pieces of bone were filled in the skull vault. Result Best results with enlarged cranial vault, better contour of forehead and optimum space of skull have been achieved in 12 cases of infant craniosynostosis ranging from 6 months to 3 years old (average:1.5 years).There were no complications in this series. The operation results were kept well for 12~16 months of follow-up in 8 cases. Conclusion Bifrontal expanding craniotomy from the superior frontal-orbital bar to the roof of skull was proved to be save and effective. It not only enlarges the cranial vault but creates best contour of the forehead as well.
