Abstract:To the editor: A 27-year-old young man without coronary risk factors was admitted to the cardiology department for episodes of chest discomfort. The electrocardiogram (ECG) on admission showed the pathologic Q waves in leads I, avL, V1 to V5 and the ST-segment elevation in leads V1-V3 (Figure 1A). The echocardiography revealed hypokinesia of anterior wall motion with normal left ventricular ejection fraction of 58%. Arrows indicate the leaking hole, Asterisks indicate the pseudoaneurysm cavity.Two years ago, he was involved in a motor vehicle accident in which his chest was crushed against the steering wheel and he had hit his head on the ground. He felt no chest pain due to a traumatic coma at that time. He was diagnosed with intracranial hemorrhage and acute anterior myocardial infarction according to the emergency ECG, which showed the ST-segment elevation in the anterior leads (Figure 1B) and the elevation of cardiac biomarkers in another hospital. He lost consciousness for 22 days and no reperfusion therapy and cardiac catheterization procedure were given to him. He recovered after conservative treatment and did not take any medication after that. Multislice coronary computed tomography angiography (MSCTA) in our hospital two years after the accident detected an aneurysm (7.9 mm × 8.6 mm) at the proximal part of the left anterior descending artery (LAD) and otherwise normal coronary arteries (Figure 1C and 1E). Invasive coronary angiography confirmed the aneurysm and the patent coronary arteries (Figure 1D). Intravascular ultrasound (IVUS) showed an aneurysm with disruption of the external elastic membrane at the proximal LAD with a clear leaking hole and huge aneurysm cavity, which was diagnosed as a pseudoaneurysm (Figure 1F–1H). The advice of surgical removal of the aneurysm was refused by the patient. Because of the absence of flow-limiting stenosis or thrombus at the aneurysmal site and two years uneventful clinical course, we decided to manage the patient conservatively under close follow-up. He was discharged with oral perindopril and metoprolol after seven days of an uneventful hospitalization. The patient remained asymptomatic without residual ischemia after discharge and his cardiac status has remained at New York Heart Association Class I for three years after the accident. The incidence of coronary artery injury after non-penetrative chest trauma is extremely low. Posttraumatic coronary aneurysm is a relatively late manifestation of blunt chest trauma, and was incidentally found during coronary angiography in patients with myocardial infarctions after chest trauma. The development of myocardial infarction immediately after trauma, as described in most reports1,2 including this one, suggests that an intimal tear and subsequent occlusion may precede the aneurysm formation. The lesion in our patient was clearly a pseudoaneurysm with full-thickness disruption and hematoma formation, which could be judged by IVUS. The pseudoaneurysm was less likely caused by the atherosclerosis process because IVUS showed there was no atherosclerotic lesion in the coronary. In our patient, the pseudoaneurysm might have occurred when blood leaked from the artery days to even years after this traumatic event. A posttraumatic coronary artery dissection can heal spontaneously. The major concerns with post-traumatic coronary aneurysm are stasis and clot formation with distal embolization. Although anticoagulation may help prevent a distal embolization of the thrombus, the theoretical risk of slowing the obliteration of a pseudoaneurysm and eventually causing aneurysmal rupture cannot be excluded. A pseudoaneurysm is considered prone to rupture. In our patient, the advice of surgical removal of the aneurysm was refused and we decided to manage him conservatively without anticoagulation therapy. During the one-year follow-up period, we observed no signs of ischemia or aneurysm rupture.
