Neurophysiologic Monitoring During Operation in the Motor Speech Area of the Brain: Report of 52 Cases

Abstract:Objective To study method for intraoperative neurophysiologic monitoring of operation in the motor speech area of the brain. Methods Fifty-two lesions in the motor speech area of the brain were resected micro-surgically under general anesthesia with a waking state of the patient, and cortical somatosensory evoked potential (CSEP), direct cortical electric stimulation (DCES), electromyogram (EMG) and electroencephalogram (EEG) were monitored during the operation. Results Forty-two patients received total resection, and 10 subtotal resection. CSEP was recorded in 38 patients, including 23 with waveform inversion, 20 with maximum wave amplitude. The locations of the DCES were concordant with those of the precentral gyrus judged by the CSEP in 33 cases, and CSEP was atypical but DCES positive in 5 cases. The accordance of the EMG changes with the movement was seen in 20 cases. EMG was definite but the movement not found in 10 cases. Epileptic waves were recorded by EEG in 20 cases and generalised slow waves in 40. The symptoms were improved or disappeared in 47 patients. Conclusion It is very useful for thorough resection of the lesion, reserving the important function maximally, improving postoperative functional impairment and ensuring the safety of operation to resect the lesions in the motor speech area of the brain under awaken general anesthesia with intraoperative monitoring of CSEP, DCES, EMG and EEG.
