Treatment strategies for spinal cavernous malformations

Abstract:Objective To ascertain the criteria of choosing microsurgery or conservative treatment strategies for spinal cavernous malformations.Methods The clinical and image data of 15 patients with spinal cavernous malformations (SCM) admitted in our hospital from April 2003 to October 2012 were analyzed retrospectively.All of them had MRI examination and the image interpretation were SCM,10 of them had surgery,the other 5 SCMs located in the medullary of spine cord with relatively mild sympton were treated conservatively.Results 5 patients had a cervical segment spine cord SCM,10 patients had a SCM in the thoracic segment.The 10 operated SCMs were totally removed with microsurgery,7 of them located in the medullary,one on the surface of the spine cord,2 were extradural.All the SCMs were completely removed,and were confirmed as SCM by pathological examination.The symptom was improved in 8 patients,no change in one,and aggravated in one,no morbid case in this series.The other SCMs of 5 patients located intramedullary were treated conservatively,one of them experienced SCM hemorrhage and was paraplegia,the other 4 patients were clinically stable.Conclusions The SCMs patient with severe symptom,especially those extramedullary or near the surface of the spine cord are indicated for microsurgery,the SCMs located in the medullary with mild symptom can be treated conservatively,but rebleeding risk exists.
