Analysis of risk factors in open operation for ruptured abdominal aortic aneurysms-hospital based experience of 30 years

Abstract:Objective To evaluate the efficacy of open operation for ruptured abdominal aortic aneurysms (RAAA) and to analyze the perioperative death related risk factors.Methods Clinical data of 103 RAAA patients who received open operation in our hospital from January 1980 to June 2014 were analyzed retrospectively.Disease- and procedure-related risk factors were analyzed by both univariate and multivariate methods.Results The open operations were successfully performed in 96 cases but failed in 7 cases who died during the operation. After operation, 71 patients recovered while 25 died from post-operative complications. The whole perioperative survival rate was 68.9%(71/103). Based on univariate analysis, age(P=0.012), time interval from disease onset to admission(P=0.023), combined with coronary heart disease(P=0.041), chronic obstructive pulmonary disease(P=0.018), cardiac arrest(P=0.036), duration of shock(P=0.007), systolic pressure(P=0.015), types of rupture(P=0.011), time interval from emergency room to operating room(P=0.001), operating time(P=0.024), aortic clamping time(P=0.030) and hemorrhage volume(P=0.039) were statistically higher in the death group than those in the survival group. Multivariate analysis showed that age(P=0.049), combined with coronary heart disease(P=0.016), duration of shock(P=0.007), time interval from emergency room to operating room(P=0.025), <br> operating time(P=0.041), hemorrhage volume(P=0.021)were statistically different between the two groups. Conclusion Open operation remains an effective management for RAAA. Early diagnosis and early operation are the keys to increase survival rate and reduce mortality.
