Abstract:Objective: To evaluate the use of titanium mesh reconstruction after sternal tumor resection. Methods: From January 2007 to January 2011, 14 patients with sternal tumors were admitted into Peking Union Medical Hospital. The clinical characteristics, surgical resection, and technique of reconstruction were reviewed. Results: Of the 14 patients, 3 had a metastatic sternal tumor, the primary sites of which were as follows: hepatic carcinoma in one case (metastasis 19 years after operation), breast carcinoma in another case (metastasis 5 years after operation), and renal carcinoma in the other case (found simultaneously). Two patients showed local involvement of the sternum: 1 had thymic carcinoma, and the other had myofibrosarcoma. The remaining 9 patients had primary tumors: 4 were osteochondroma, 3 chondrosarcoma, 1 eosinophilic granuloma, 1 non-Hodgekin's lymphoma. En bloc resection of the sternal tumor was performed in all the 14 patients. The defect was repaired with the titanium mesh adjusted to the shape of the defect and fixed with the stainless steel wire. Eleven patients were followed up for a period from 2 months to 4 years, during which no translocation or broken of the titanium mesh was observed. Conclusions: Radical en bloc excision remains the treatment of choice for sternal tumors. Sternum defect reconstruction using titanium mesh as a rigid replacement proves appropriate and effective.
Surgical Techniques in the Radical Resection of Hilar Cholangiocarcinoma

Abstract:Hilar cholangiocarcinoma remains a formi-dable challenge to hepatopancreatobiliary surgeons since the reported resection of a primary cancer originating at the hepatic duct confluence by Brown and Myers in 1954. Emerging evidence has indicated that aggressive surgery with a curative resection offers a better option for long-term survival compared with conservative therapy. Liver transplantation has also been considered as a management opportunity for the treatment of cholangiocarcinoma. However, the survival rate has been poor due to the high proportion of disease recurrence. This review highlights recent techniques in hilar cholangiocarcinoma resec-tion, with special attention to the management of the resection margin, clinical skills of liver resection, lymph node clearance, and portal vein or hepatic artery resection or reconstruction. In addition, technical advances have been proposed in hepatopan-creatoduodenectomy and liver transplantation for hilar cholangio-carcinoma treatment. In the current hepatic procedures, promis-ing survival outcomes have been obtained in patients with hilar cholangiocarcinoma, exhibiting a decreased operative mortality and a steady improvement in long-term survival. Overall, the correct clinical strategy and appropriate surgical techniques may provide an increased chance to cure patients with hilar cholan-giocarcinoma.
Reconstruction of Anterior Chest Wall: a Clinical Analysis

Abstract:ObjectiveTo investigate the methods and clinical efficacy of reconstruction of chest defects with titanium sternal fixation system after the surgical resection of sternal tumors.MethodsA total of 6 patients with sternal tumor who were diagnosed and underwent resection and repair of the chest wall defects by titanium plates system, from 2017.3 to 2017.11 in our hospital were reviewed. Their pathological types, surgical reconstruction methods, follow-up results were analyzed.ResultsSix cases of sternal tumor were completely resected and the sternums were reconstructed with titanium sternal fixation system. There was no operative death, postoperative chest wall deformity, abnormal breathing or complications of respiratory circulation. After 3 to 10months of follow-up, there was no loose screw or plate exposure. Not only the thoracic appearances were good, but patients' satisfaction was high.ConclusionsSurgical resection is the best treatment for sternal tumors, no matter it is benign or malignant. Titanium sternal fixation system combine with other soft materials can reconstruct the chest wall well after resection, and this technique is efficient as well as easy to learn.
Sternal resection and reconstruction: a review

Abstract:Sternal resection and reconstruction is a rare but sometimes challenging procedure due to its profound anatomical and functional implications. For these reasons, an adequate preoperative evaluation is crucial in each patient, especially when we are faced with malignant lesions that sometimes require extensive radical resections, thus demanding an integrated reconstructive strategy that allows stabilizing the chest wall, protecting the underlying mediastinum and minimize resulting deformity. The large number of available reconstruction techniques and the lack of quality studies for their analysis mean that sternal reconstruction depends to a great extent on the consensus of experts or, more frequently, on the simple preference of each surgical team. This article aims to provide an overview of sternal resection and reconstruction. Indications for partial versus total or subtotal sternectomy are suggested and their surgical and oncological outcomes are presented. The use of rigid or semi-rigid prostheses is an ongoing debate, although recent functional data advise reserving rigid reconstructions for extensive defects. Sternectomy for primary tumors or local tumor involvement has a good prognosis with an overall survival of 5 and 10 years: 67% and 58%, respectively, provided that a radical resection with free surgical margins is performed. Breast cancer is the most common secondary sternal tumor, and surgery can offer 5-year overall survival ranging from 20% to 50% provided an R0 resection is achieved, although radical surgery does not appear to decrease rates. of recurrence. Metastases of origin other than the breast give the worst results (less than 40% at 36 months and 0% at 5 years) and although the data available on these cases are limited, the radicality of the resection does not seem to modify the survival or recurrence rates, so a conservative approach is probably more appropriate.
Surgical management of a giant sternal chondromyxoid fibroma: a case report

Abstract:Background A primary chondromyxoid fibroma (CMF) arising from sternum is quite uncommon tumor in thoracic surgery. Removal of giant sternal tumors requires extensive resection of the anterior chest wall, and results in deformity and paradoxical movement. Case Presentation A 40-year-old female presented a progressively enlarging mass of her anterior chest wall. Computed tomography revealed an osteolytic lesion with discrete calcification in the bone marrow of the sternum. The tumor extended across the destroyed cortex to the parietal and visceral soft aspects, involving some of the costal cartilage and most of the sternal body. Partial sternal resection was performed successfully and an individual-specific stainless steel plate was used to reconstruct the anterior chest wall. The early result was good, however, nine months after the first surgery, fractures of plate were found at bilateral plate-clavicular junction. The plate had to be removed, and a titanium mesh was used to reconstruction of the chest wall. The patient has been of disease free for more than 18 month after the second surgery. Conclusions Our experience indicated that the individual-specific plate may not be suitable for reconstructing both the anterior chest wall as well as the sternoclavicular joint after subtotal sternum resection.
Mastery of Chest Wall Reconstruction with a Titanium Sternum-Rib Fixation System: a Case Series

Abstract:Background:Chest wall disease is a common disease in thoracic surgery. For most chest wall lesions, surgical resection is the mainstay of treatment. Reconstruction is indicated for a wide range of chest wall defects. Currently, various reconstruction materials are used in clinic, including 3D printing materials and various types of metal materials. At present, most of the studies using titanium sternum-rib fixation system for reconstruction are case reports. The purpose of this paper is to analyze the experience to discuss our essential surgical techniques for treating various types of chest wall reconstruction with a titanium sternum-rib fixation system over the last 5 years.Case Description:A retrospective analysis was performed on patients with chest wall tumors treated with a titanium sternum-rib fixation system in our center from 2016 to 2020. Chest wall reconstruction techniques, experiences, postoperative complications, and quality of life including chest discomfort, chronic pain, average time to return to normal life, chest wall deformity after resection for various types of chest wall tumors were analyzed. In this study, a total of 57 patients were successfully operated without chest wall deformity and return to daily life early. With an average of 2.3 ribs removed, including 10 procedures involving sternotomy and reconstruction and 3 procedures involving sternoclavicular joint resection and reconstruction. The follow-up time of the whole group ranged from 3 months to 5 years. Postoperative chest discomfort occurred in 6 patients during follow-up; 2 patients had chronic pain. The average time to return to normal life was 1.4 months. One patient developed a deformed depression of the chest wall, and 2 patients developed wound infections. There was no perioperative death.Conclusions:In our clinical experience, the titanium sternum-rib fixation system is safe, effective, and feasible. The technique is straightforward. The early and middle postoperative curative effect is satisfactory and can be used clinically.

Abstract:Background and Objectives As one of the cutting‐edge advances in the field of reconstruction, three‐dimensional (3D) printing technology has been constantly being attempted to assist in the reconstruction of complicated large chest wall defects. However, there is little literature assessing the treatment outcomes of 3D printed prostheses for chest wall reconstruction. This study aimed to analyze the surgical outcomes of 3D custom‐made prostheses for the reconstruction of oncologic sternal defects and to share our experience in the surgical management of these rare and complex cases. Methods We summarized the clinical features of the sternal tumor in our center, described the surgical techniques of the application of 3D customized prosthesis for chest wall reconstruction, and analyzed the perioperative characteristics, complications, overall survival (OS), and recurrence‐free survival of patients. Results Thirty‐two patients with the sternal tumor who underwent chest wall resection were identified, among which 13 patients used 3D custom‐made titanium implants and 13 patients used titanium mesh for sternal reconstruction. 22 cases were malignant, and chondrosarcoma is the most common type. The mean age was 46.9 years, and 53% (17/32) of the patients were male. The average size of tumor was 6.4 cm, and the mean defect area was 76.4 cm2. 97% (31/32) patients received R0 resection. Complications were observed in 29% (9/32) of patients, of which wound infection (22%, 7/32) was the most common. The OS of the patients was 72% at 5 years. Conclusion We demonstrated that with careful preoperative assessment, 3D customized prostheses could be a viable alternative for complex sternal reconstruction.

APPLICATION OF TITANIUM PLATE AND Teflon PATCH IN CHEST WALL RECONSTRUCTION AFTER STERNAL TUMOR RESECTION

Abstract:OBJECTIVE To study the reconstruction method and effectiveness of titanium plate and Teflon patch for the chest wall after resection of sternal tumors. METHODS Between October 2006 and November 2009, 4 patients with sternal tumors were treated and the thoracic cages were reconstructed. There were 2 males and 2 females, aged 30-55 years. The patients were admitted because of chest lump or pain. The sizes of palpable lump ranged from 4 cm x 3 cm to 10 cm x 8 cm. CT examination showed bone destruction. After sternal tumor resection, defect size ranged from 10 cm x 8 cm to 18 cm x 14 cm, and titanium plate and Teflon patch were used to repair and reconstruct the chest wall defect. RESULTS The operations of the tumor resection and reconstruction of chest wall defect were successfully performed in 4 cases. Incisions healed by first intention with no abnormal breath, subcutaneous emphysema, pneumothorax, and infection. One case failed to be followed up after 6 months; 1 case died of intracranial hemorrhage; and 2 cases were followed up 1 and 4 years respectively without tumor recurrence. The chest wall had good remodeling. No loosening and exposure of titanium plate, difficulty in breathing, chest distress, and chest pain were observed during follow-up. CONCLUSION Surgical resection of sternal tumors will cause large chest wall defect which can be repaired by titanium plate and Teflon patch because it had the advantages of easy operation, satisfactory remodeling, and less complication.
Progresses of surgical treatment for the cervicothoracic spinal primary malignant tumors

Abstract:Cervicothoracic spine is a special position in the anatomy andbiomechanics. The anterior includes a variety of tissues, organs and nerves. And the lesion gets deeper owing to the chest. In addition, the concentrated spinal stress makes a high risk of failure of the internal fixation. So, surgical treatment is of great difficulty for primary malignant tumors at the cervicothoracic junction. There are many disputes in the surgical approach for the cervicothoracic tumors. The proximal C7-T1 lesions can obtain a clear exposure by an anterior low suprasternal approach alone. On the contrary, access to the distal T2-4 lesions through an anterior approach is generally considered to be dififcult and need to becombined with sternotomy or thoracotomy. But the single posterior total spondylectomy can avoid large injury and severe complications caused by the anterior approach. Total spondylectomy can make spinal tumors reach the wide or marginal excision. In recent years, total spondylectomy is being widely used in the treatment of spinal primary malignant tumors, which is helpful to complete removal of the tumor and effective decrease of the local recurrence. Cervicothoracic junction changes from the lordosis and active cervical to kyphosis and relatively ifxed thoracic vertebrae. It is also a signiifcant change for vertebral morphology and spinal stress. For this reason, in the spinal reconstruction, the form and stress of the internal ifxation have a certain speciifcity. This article reviewed the progress of surgical treatment and spinal reconstruction for cervicothoracic primary malignant tumors.
Radical resection and reconstruction of a large sternal chondrosarcoma

Abstract:Introduction. Primary malignant tumors of the sternum are rare, with chondrosarcoma being the most common primary malignant tumor of the chest. The gold standard in treatment is surgical treatment with wide resection margins, where the rigidity of the chest wall must be ensured, with protection of internal organs with satisfactory lung function. Case outline. We present a 67-year-old patient in whom previous computed tomography and magnetic resonance imaging examinations confirmed a tumor mass involving the sternum with the associated ribs, with involvement of the soft tissues above. First, sternum resection was performed with partial resection of the associated ribs and soft tissues. Reconstruction and stabilization of the chest wall were achieved with two layers of polypropylene mesh and methyl methacrylate bone cement with antibiotics. We reconstructed the primary soft tissue defect with a combination of a large local fasciocutaneous flap raised from the abdomen and a smaller sliding flap from the chest. The secondary defect was reconstructed by wide undermining of the skin in the area of the anterior abdominal wall and a small Thiersch-type free skin graft. In the postoperative period, the flaps were vital, but there was necrosis of the free skin graft. That defect was closed secondary thanks to bandaging. Respiratory function was preserved. Conclusion. Surgical treatment is the main treatment for sternal chondrosarcoma. With an adequate preoperative and intraoperative approach, it is necessary to enable good postoperative oncological outcomes with the achievement of chest rigidity and satisfactory respiratory status.

Autologous Ilium Graft Combination with Y-shaped Titanium Plate Fixation for Chest Wall Reconstruction after Resection of Primary Sternal Tumors-a Clinical Study from Three Institutions

Abstract:Background The technique for anterior chest wall reconstruction after resection of primary sternal tumors (PST) still continue to evolve. Methods A total of 12 PST patients from three hospitals who underwent en-bloc resection were included in our study. After finishing sternum resection, autologous iliac bone combined with Y-shaped titanium plate were applied to rebuild the anterior chest wall. Postoperative outcomes were analyzed. Results There were 10 different types of tumors located in manubrium (6 cases), sternum body (4 cases) and Louis’s angle (2 cases) in our research. For these patients, the median resected tumor size and the area of defect after sternal resection were 279.0 cm3 and 215.0 cm2, respectively. The mean operative time was 299.2±65.2 min and intra-operative blood loss was 431.2±213.0 mL. Mean duration of drainage was 9.9±2.6 days. In their perioperative period, significant circulatory and respiratory complications occurred in 8 patients. Postoperative chest X-ray and tridimensional CT images showed autogenous reconstruction of the sternum and titanium in good position. No side effects were observed 6–12 months post reconstructive surgery, but one patient suffered from anchor loss and prosthesis migration. Y-shaped titanium plates from two patients were separately removed at 24 and 26 months when the reconstructive sternum integrated with skeleton anterior chest wall well. Conclusions Our study demonstrates the safety and feasibility of this new technique for anterior chest wall reconstruction after sternectomies.
Application of Multi-Functional Modularized Prosthesis in Reconstruction for Sternal Defect

Abstract:Objective: To investigate the feasibility and effectiveness of sternal reconstruction using a multi-functional modularized sternal reconstruction system. Methods: In June 2016 and August 2017, the modularized sternal prosthesis system was used on two patients for sternal reconstruction in the Department of Thoracic Surgery of Shanghai Pulmonary Hospital, Tongji University. Both patients were female aging 48 and 72 years, respectively, with the primary diagnosis of tumor of the lower sternal body and huge mediastinal tumor. Partial sternal resection and reconstruction was performed through median sternotomy. The multi-functional modularized sternal reconstruction system consisted of manubium, superior sternal body, inferior sternal body, rib and clavicle modules. Each module was designed into 3 to 6 sizes. Appropriate modules were chosen in each case to be assembled as a sternal reconstruction prosthesis. Results: Both operation were smooth, with operation time of 240 minutes and 280 minutes, intraoperative blood loss of 100 ml and 400 ml. The patients were followed up for 18 months and 4 months, respectively. In both cases, the sternal reconstruction was satisfactorily healed, without local infection, fluid accumulation, loose part or dislocation. No local recurrence or distant metastasis was found. Conclusion: The multi-functional modularized sternal reconstruction system can be safely and effectively applied for sternal reconstruction in 2 cases.
Surgical Treatment for Complex Neoplasms Involving the Chest Wall

Abstract:Objective:To summarize the clinical characteristics and surgical treatment for complex neoplasms involving the chest wall.Methods:The clinical and pathologic material as well as the of the surgical treating process were collected in patients with tumors invading the chest wall in Xinhua hospital affiliated to Shanghai Jiaotong University school of medicine from January 2011 to December 2015.The therapeutic efficacy and follow up data were also analyzed.Results:25 paticnts wcrc enrolled in our study including 19 men and 9 women.All the neoplasm were radically resected and chest wall reconstruction was performed in all the patients above.Among the 25 cases,artificial vascular replacement was applied in 4 cases because their superior vena cava or brachiocephalic veins was invaded.Sternum was partly or completely removed in 2 patients.There was another patient receiving tumor resection under extracorporeal circulation for the cancer cell embolus formation.The pathological diagnosis was as follows:6 of the 19 malignant tunor were osteosarcoma;3 of them were thymic carcinoma;In the rest of the malignant tumors,we also found 2 cases of plasmacytoma,2 rhabdomyosarcoma,2 neuroblastoma,2 spindle cell carcinoma,1 dedifferentiated chondrosarcoma and l mesothelioma.Benign neoplasm was observed in 6 cases including 2 solitary fibrous tumor,2 neurofibroma,1 vascular tumor and 1 inflammatory tumor.Conclusions:It was crucial to remove the mass complctcly and perform chest wall reconstruction primarily in treating complex neoplasms involving the chest wall.Artificial vascular replacement,neoadjuvant chemoradiotherapy and extracorporeal circulation may contributed to improve the prognosis for patients with superior vena cava or brachiocephalic veins invaded or for those with cancer cell embolus formation.

Abstract:Radical resection is the preferred therapy for primary malignant sternal tumors. Sternal reconstruction is required to guarantee the best preservation of respiratory mechanics, and adequate mediastinal protection and acceptable cosmetic results after extensive tumor resection. A wide variety of methods and materials have been described for sternal reconstruction. Titanium implants are preferred by many surgeons because of their optimal features. However, the smooth surface of the metal prostheses does not facilitate the inward growth of the tissue, and the high density of metal can block the X-ray and cause adverse effects on postoperative imaging and radiotherapy. Therefore, in this article we present a case of sternal reconstruction by means of a three dimensional (3D) custom-made carbon-fiber prosthesis following extensive resection of a sternal synovial sarcoma. The microporous structure on the surface of the carbon fiber composite material facilitates the inward growth of the tissue. Low density (1.5 g/cm(3)) of carbon-fiber implant will not block the X-ray and eliminates the adverse effects caused by metal material of postoperative imaging and radiotherapy. The 3D custom-made carbon-fiber prosthesis matched the thoracic defect perfectly and the chest wall reconstruction was stable for more than 24 months.
Surgical Progress of Pulmonary Metastases from the Osteogenic and Soft-tissue Sarcoma

Abstract:Pulmonary metastasectomy is an important curative option for patients with osteogenic and soft-tissue sarcoma spread to the lungs.Complete surgical removal of pulmonary metastases can improve survival and is recommended under certain criteria.Specific issues that require consideration when planning pulmonary metastasectomy include: preoperative assessment of the operation index and contraindications,choice of surgical strategies,pulmonary parenchymal preservation,and the role of lymphadenectomy.With the development of iconography and chemotherapy,the emergence of targeted drugs,and the innovation of radiotherapy,the concept of the diagnosis and treatment for pulmonary metastases from osteogenic and soft-tissue sarcoma is also undergoing great changes.
Experience of Chest Wall Tumor Resection and Chest Wall Defect Reconstruction

Abstract:Objective To investigate the resection of chest wall tumor and the reconstruction of it.Methods Retrospectively summarize the diagnosis and treatment of 91 cases of chest wall tumors in our hospital from January 2008 to November 2017.There are 55 males and 36 females and the ages ranged from 7 to 78 years,with a median age of 43 years.82 cases were operated for the first time,and 9 cases were reoperated.The postoperative pathology showed that the benign were 52 cases and malignant were 39 cases.The largest benign tumor was 15 cm × 10 cm × 3 cm and the largest malignant tumor was 30 cm × 20 cm × 15 cm.Local excision was performed in 66 cases and extended resection in 25 cases.After the resection,ilium,titanium mesh,Dacron mesh and Matrix reconstruction system were selected for reconstruction of chest wall respectivly.Results All the operation was successful and without death.Local resection was performed in all of benign tumor and in 15 cases of malignant tumor,extended resection was performed in the other 24 malignant tumor cases.After the local resection,the soft tissue was sutured directly to reconstruct the chest wall.The enlarged resection of the bone was performed by reconstructing the bony chest with different materials.In the soft tissue reconstruction,1 cases were treated with myocutaneous flap,and the other 23 cases were sutured directly.Postoperative chest wall integrity is stable,no chest wall floating and abnormal breathing.1 patients received ventilator supportion,and 2 patients had wound infection.The chest wall defect caused by benign tumor has good quality of life,no recurrence.After resection of the malignant tumor of the chest wall,2 died,and the rest were still alive.Conclusion Local resection is the main therapeutic method of benign chest wall tumor,and malignant chest wall tumor is mostly enlarged excision.Bone remodeling after extended resection plays an important role in the reconstruction of chest wall.It is necessary to select appropriate materials for reconstruction.Matrix titanium sternal or rib reconstruction system fixed reliably and the follow-up in a short term were also satisfactory.

Current Challenges in Thoracic Surgery

Abstract:Reconstruction for a large area of sternal defect is challenging. The available sternal reconstruction techniques and materials have their limitations. We designed a modular sternal reconstruction system made of titanium alloy. This system was applied to a patient who received partial sternal resection for well-differentiated chondrosarcoma of the sternum. The sternal neoplasm measured 80×70 mm2. After resection of most of the sternal body and adjacent ribs, standard modules of adequate types and sizes were chosen and assembled during operation. The operation lasted for 240 min, with an intraoperative blood loss of 100 mL. The patient was followed up for 32 months, while the prosthesis remained stable, with no signs of infection or looseness.

Abstract:Primary sternal tumours are rare. Most of them are malignant in nature. When localized, cure can be achieved by radical resection. However, it leaves a large bony defect in front of the heart that makes reconstruction a challenge. In this report, we describe our experience of sternal reconstruction using a custom-made 3D-printed titanium neo-sternum after en-bloc resection of sternal body and anterior ends of bilateral second to fifth ribs for chondrosarcoma.

Abstract:Objectives: To investigate the clinical outcome of anterior approach and hemisectomy of stern for upper thoracic disease.Methods: 17 patients suffering from upper thoracic lesions underwent anterior hemisectomy of stern from July 2005 to September 2010.There were 8 males and 9 females with the age ranging from 27 to 77 years,with an average age of 57.7 years.The pathogenesis included T3 aneurysmal bone cyst in two cases,T2-T3 tuberculosis in seven cases,T3-T4 tuberculosis in three cases,T2 bone giant cell tumor in two cases,T3 giant cell tumor in two cases and T2 chondrosarcoma in one case.Before operation,upper thoracic sagittal CT was analyzed with the aim to determine the scope of surgery.Upper sternum was resected and removed as half-moon.Both sides of the sternoclavicular ligament were retained.Through the window of the brachiocephalic artery,lesion was exposed.The debridement or excision,bone grafting or reconstruction of internal fixation were performed afterward.Intraoperative,postoperative and follow-up complications were recorded.Results: 17 patients were perfomed surgery successfully.All lesions were exposed satisfactorily.Three cases were complicated with bradycardia,low blood pressure or increase of airway resistance during operation,and all resolved quickly after the discharge of traction.1 case was complicated with transient hoarse ness,that was resolved 2 month later.The surgical time was 2-3h,and the amount of blood loss was 200700ml,averaging 400ml.All patients were followed up for 17-27 months,with an average of 22.7 months.No fixation failure was noted,and all cases reached bony fusion.Physiological curvature of the cervical and thoracic segment recovered.17 cases with preoperative neurological deficit recovered partially.Conclusions: The modified upper thoracic vertebral approach can effectively expose T2-T4,which can retain the sternoclavicular joint and sternum,and is proved to be an ideal approach for managing upper thoracic lesions.

Abstract:OBJECTIVE:The aim of this study was to investigate the proper treatment of infected median sternotomy wounds.METHODS:A retrospective study was conducted to investigate the proper treatment of infected median sternotomy wounds on patients with sternal wound infections from January 2007 to July 2009. The characteristics of the sternal infections and the treatment outcomes were analysed.RESULTS:Ninety-seven patients with sternal wound infections were treated. A total of 32 patients acquired the infection within one month after open-heart surgery, 10 patients got the infection one to two months after the surgery, and 1 patient died two days after debridement. There were 54 patients who acquired the infection beyond two months post-surgery, while 1 patient died on the day before the operation. One patient received four cycles of wound debridement, 18 patients received two cycles and 78 patients only received one cycle. A total of 14 patients received a vacuum-assisted closure treatment. There were 73 patients who had surgery for repair of muscle flaps, 1 patient for breast tissue flap, 63 patients for pectoralis major muscle flap, and 9 patients with rectus abdominis muscle flap. There were 12 patients who received a transverse plate fixation of the sternum with titanium plating.CONCLUSION:A positive prognosis can be obtained by the algorithm treatment based on the onset and depth of the sternal infection.
