Clinicopathological Features and Prognostic Analysis of Extragastrointestinal Stromal Tumor in an Asian Center.

Abstract:e21509 Background: Extragastrointestinal stromal tumor (EGIST) shows features of GIST outside the gastrointestinal tract, occurring mainly in the omentum, mesentery and retroperitonium. EGIST is rare, comprising only 5% to 10% of all GISTs. Few studies have been performed with a focus on EGIST. The aim of this study was to better define the clinicopathological features and prognostic factors of EGISTs. Methods: Data were collected from 20 patients with a histological diagnosis of EGIST were admitted to the third affiliated hospital of Sun Yat-Sen university from January 2003 to May 2014. We recorded the patients’ clinicopathological information including age, gender, tumor size, surgery type, capsular integrity, metastatic sites at diagnosis and tumor risk category by reviewing the medical records. Immunohistochemistry was performed for HSP90, CD117, CD34, DOG1, VEGF, Ki-67, desmin, SMA, S-100 protein. Univariate analyses were performed using the Kaplan-Meier method, a multivariate analysis were done by the Cox regression analysis. Results: Mean follow-up was 48 months. The median disease free survival time (DFS) was 28 months. The 1- and 2-year survival rates were 85% and 70% respectively. The EGISTs were positive for CD117 in 95 % (19/20), CD34 in 65% (11/20), DOG-1 in 75% (15/20), HSP-90 overexpression in 40%(8/20),HSP-90 low expression in 60%(12/20), desmin in 15%(3/20), SMA in 5%(1/20), S-100 in 25% (5/20) of samples. For Ki-67 labeling index (Ki-67 LI):30% were < 5%, 45% were between 5-10%, 25% were > 10%.Survival analysis indicated that mitotic counts, overexpression of HSP90 and Ki-67 index were significant predictors for survival. Conclusions: The results of this study show mitotic counts, overexpression of HSP90 and Ki-67 index may be useful for predicting the prognosis of EGIST.
