The influence of body mass index on prognosis of patients with upper tract urothelial carcinoma under-went radical nephroureterectomy

Abstract:Objective To explore the influence of body mass index (BMI)on prognosis of pa-tients with upper tract urothelial carcinoma (UTUC)after radical nephroureterectomy (RNP). Methods Between January 2006 and January 2012,184 patients with UTUC underwent RNP.A to-tal of 184 patients were classified into three groups according to the BMI criteria for Chinese:normal weight,BMI≥18.6 and ≤22.9 kg/m2 (n = 110,59.8%);overweight,BMI≥23.0 and BMI≤24.9 kg/m2 (n =35,19.0%);and obesity,BMI≥25 kg/m2 (n =39,21.2%).Clinicopathological features and prognostic factors were analyzed using univariate and multivariate cox regression analyses. Re-sults All patients were operated successfully and were followed up for 5-60 months.The history of bladder cancer (P =0.038),urinary cytology (P =0.024),tumor stage (P =0.014),tumor grade (P =0.039)and lymphovascular invasion (LVI)(P =0.013)were significantly differences among three groups.Univariate analysis demonstrated BMI,the history of bladder cancer,pT stage,pT grade and LVI were associated with 5-year recurrence-free survival (RFS)and overall survival (OS). Multivariate cox regression analysis showed that BMI was the independent risk factor of the recur-rence of UTUC (HR =1.351,95%CI 1.019-1.791,P =0.037).However,there was no significant difference between BMI and OS (P =0.085). Conclusions Obesity is the independent risk factor of the recurrence of UTUC.However,BMI was no related to OS in patients with UTUC undergoing RNU.Elevated BMI is correlated with increased recurrence rate of UTUC.
