Predicting preeclampsia by integrating maternal characteristics, mean arterial pressure and serum biomarkers at 15~20 gestational weeks

Abstract:Objective To evaluate the prediction accuracy of preeclampsia by combining maternal characteristics,mean arterial pressure (MAP) and serum biomarkers at 15-20 gestational weeks.Methods A prospective study was taken out from May 2014 to May 2015.Maternal serum samples were obtained in Down syndrome screening,and all gravidas were followed up until delivery.31 cases of severe preeclampsia (SPE),29 non-severe preeclampsia (NonSPE),35 gestational hypertension (GH) and 120 normal pregnancies (NP) were recruited.Maternal characteristics,MAP at enrollment,serum concentrations of 7 selected biomarkers detected by ELISA were analyzed.Results Compared with NP group,mean maternal age was older in SPE group,while BMI were higher in both Non-SPE and GH groups(both P < 0.01);MAP were significantly increased in both PE and GH groups (P < 0.01).PlGF and PAPP-A levels were both lower in PE group,while HCG levels were significantly higher only in the SPE group (P < 0.01).There were no significant differences in PP13,sFlt-1,sEng and VEGF levels between PE and NP group.Levels of all serum biomarkers in the GH group were not significantly different from the NP group.Logistic regression revealed that,the most predictive model for SPE was the combination of maternal age,MAP,PAPP-A,PlGF and HCG,with an AUC of 0.924(95% CI:0.870~0.96 l;P < 0.01).For Non-SPE was the combination of BMI,MAP,PAPP-A and PlGF,with an AUC of 0.877(95%CI:0.813~0.925;P < 0.01).Conclusion An integration of maternal age,BMI,MAP and PAPP-A,PlGF,hCG could provide a possible screening protocol for PE,especially SPE at 15~20 gestational weeks.
