Effectiveness of Sex Psychological Assessment Inventory in the Evaluation of Gender Identity in Patients with 46,XY Disorders of Sex Development

Abstract:Objective To assess the effectiveness of sex psychological assessment inventory in the evaluation of gender identity in patients of 46,XY disorders of sex development (DSD).Methods The clinical data were analyzed retrospectively for 80 patients of 46,XY DSD between January 2000 and January 2014.Gender identity was evaluated with pre-school activities inventory (PSAI) for aged ＜5 years and children's sex role inventory (CSRI) for aged ≥5 years.The results of previous gender assignment were compared statistically with this psychological test.Meanwhile,40 control cases were evaluated with the same test.Results In DSD group aged ＜5 years,the previously assigned gender was male (n =28) and female (n =4).Sex character was masculinization (n =29) and feminization (n =3).The coincidence rate was 90.6％ (P＞0.05).For control group with same age,sex character was purely masculinization with a coincidence rate of 100％ (P =0.083).There was no statistical difference.Among 48 DSD patients aged ≥5 years,previously assigned gender was male (n =33) and female (n =15).Sex character was masculinization (n =39),feminization (n =8) and androgynous (n =1).The coincidence rate between assigned gender and sex character was 82.9％ (P＜0.01) and it had statistical difference.For 52 patients in control group,sex character was purely masculinization with a coincidence rate of 100％ (P =0.002).There was statistical difference.Conclusions Gender identity may be assessed initially by PSAI and CSRI for ＜5 and ≥5 years DSD patient respectively.Thus this psychological assessment provides rationales for gender assignment.
