Laparoscopic versus open Ladd surgery for intestinal malrotation: a Meta-analysis

Abstract:Objective:To explore the clinical outcomes of laparoscopic versus conventional open Ladd procedure for intestinal malrotation.Methods:Two researchers simultaneously conducted a comparative study on pediatric open Ladd procedure versus laparoscopic Ladd procedure by searching the databases of PubMed, Web of Science, Embase, CNKI, Wanfang and VIP.The parameters of postoperative feeding time, length of hospital stay, impact on postoperative immune function and incidence of postoperative complications were compared between two procedures.After literature screening, Cochrane risk of bias assessment tool and Newcastle-Ottawa Scale (NOS) were employed for evaluating the quality of the included studies and the relevant data extracted.Meta-analysis was performed with RevMan 5.3 software.Results:Among a total of 1, 278 articles, 52 were subjected to full-text analysis and 16 fulfilled the inclusion criteria.A total of 2, 820 cases of intestinal malrotation were included, with laparoscopic Ladd procedure (laparoscopic group, n=979) and open Ladd procedure (open group, n=1841). Meta-analysis revealed that, as compared with open group, laparoscopic group had earlier postoperative feeding time [ MD=-1.51, 95% CI(-2.09, -0.93), P<0.001], shorter hospital stay [ MD=-2.63, 95% CI(-3.80, -1.46), P<0.001], lower degree of immune function suppression [ MD=1.75, 95% CI(1.09, 2.41), P<0.001], lower incidence of adhesive intestinal obstruction [ RR=0.70, 95% CI(0.51, 0.96), P=0.03], lower rate of incisional infection [ RR=0.44, 95% CI(0.24, 0.80), P=0.007] and lower rate of incisional dehiscence [ RR=0.33, 95% CI(0.13, 0.81), P=0.02]. However, no statistically significant inter-group difference existed in operative duration [ MD=0.66, 95% CI (-16.35, 17.67), P=0.94] or postoperative occurrence rate of intestinal volvulus [ RR=1.17, 95% CI (0.63, 2.17), P=0.62]. Conclusions:Laparoscopic Ladd procedure has advantages over open Ladd procedure for infants and children with congenital intestinal malrotation, including faster postoperative recovery, lower degree of immune function suppression and fewer complications.
