Granular cell tumors of soft tissue:A clinicopathological preliminary analysis of 23 cases

Abstract:Objective:To study the clinicopathological feature of soft tissue granular cell tumor(GCT)and im-prove the diagnosis of the tumor.Methods:A total of 23 GCTs were retrieved,immunohistochemical staining,molecu-lar pathology detection and analysis of the clinicopathological characters were performed.Results:The average age of the GCT was 47.9 years old(ranging from 16 to 70 years old),with 16 women(69.6%)and 7 men(30.4%).21 cases(91.3%)were benign and 2(8.7%)were malignant.Under the microscope,benign GCT tumor cells were round,oval or polygonal,and the cytoplasmic eosinophils were clear.None of the 21 cases(91.3%)were seen in the case of mitose and necrosis.2 cases of malignant GCT cells were distinct nuclear pleomorphism,and the mitoses were 7 and 4/10HPF,1 of which was visible necrosis.Immunohistochemical staining revealed the positive ratio of S-100, CD68 and Vimentin expression was 100%(21/21),90.5%(19/21),100%(21/21)in benign GCTs individually, but 100%(2/2)in malignant GCTs. The average benign proliferation index of Ki -67 were 2.1%(range 0 ~10%),while the average malignant proliferation index of Ki -67 were 22.5%(range 15% ~30%).To further exclude clear cell sarcoma,EWSR1 rearrangement was not detected in 1 case MGCT occurred in the foot by fluores-cence in situ hybridization analysis.There was no recurrence in 19 cases of BGCT followed from 14 months to 8 years, whereas 2 MGCT cases showed recurrence.Conclusion:GCT was a rare peripheral nerve tissue tumor,immunohisto-chemical staining is the conventional tool for GCT diagnosis,and molecular genetics yet remains to be investigated. The study showed GCT should be carefully identified with a variety of lesions because of the possibility of misdiagnosis in frozen pathology.
