Socio-demographic and Clinical Features Between Patients with Bipolar Disorder and Ones with Major Depressive Disorder

Abstract:Objective Misdiagnoses of bipolar disorder (BD)as unipolar disorder(UPD) may lead to inappropriate treatment and poor outcomes.This study aimed to compare demographic and clinical features of patients with BD and MDD in China.Methods A total of 1 487 patients treated for MDD were consecutively evaluated in 13 psychiatric hospitals or units in China.The Mood Disorder Questionnaire (MDQ) and the Hypomania Checklist (HCL-32) was used to assess hypomanic symptoms.The patients'socio-demographic and clinical characteristics were recorded using a standardized protocol and data collection procedure.The Mini International Neuropsychiatric Interview (MINI) was used to establish DSM-Ⅳ diagnoses.Results (1) The proportions of BD-Ⅰ and BD-Ⅱ being misdiagnosed for MDD were 7.9% (118/1 487) and 12.8% (191/1 487),respectively.(2)Frequent attacks of depression in patients with MDD (year > 4 times/year) accounted for 7.6% (90/1 178),depressive episodes associated with obesity,sleep and increased weight accounted for 15.2% (179/1 178),depression with psychotic symptoms accounted for 13.4% (158/1 178),lifetime depressive episodes (1.9 ± 2.6),first onset age (34.6 ± 12.5) years old,and in BD-Ⅰ patients of the above indexes were 29.7% (35/118),39.8% (47/118),38.1% (45/118),(3.9 ± 4.1),(28 ± 10.5) years old,there were statistically significant differences between the two (P < 0.05 or P < 0.01) ; in BD-Ⅱ patients,depression with psychotic symptoms accounted for 28.8% (55/191),lifetime depressive episodes (2.5 ± 3) times,onset age (28.8 ± 10.9) years old,there were statistically significant differences as compared with patients with MDD (P <0.05 or P <0.01).Conclusion Depressive episodes in patients with BD-Ⅰ and BD-Ⅱ probably present different clinical features compared to MDD,and findings could be as reference for making clinical diagnosis.
