Abstract:With the Achenbach Child Behavioral Scale(by parents) and a comprehensive battery of neuropsychological tests capturing domains of attention and inhibition to test relevant cognitive functions,the study assessed children's behavioral symptoms.Twelve early-school-age children with Asperger syndrome(AS) and 29 normal children matched in age and IQ participated in the study.We obtained the following results: with age and IQ controlled,children with AS showed worse social capability,and a series of behavioral problems such as impaired social interaction,social withdrawal,anxiety,depression,compulsion,aggression,and hyperactivity.Moreover,children with AS had impaired attention,inhibition and movement coordination.

Abstract:s Objective: To investigate the comorbidity of depression and anxiety in primary school students. Methods:565 pupils ranged from 2 to 6 grade in a primary school were tested by The Screen for Child Anxiety Related Emotional Disorders (SCARED) and Depression Self-Rating Scale for Children (DSRSC). Results: There were 140 pupils with a total score higher than cut-off point of SCARED; 97 pupils with a total score higher than cut-off point of DSRSC; 43 pupils with total scores higher than cut-off point of both SCARED and DSRSC, and the positive rate of comorbidity of anxiety and depression was 7.61%. 30.71 percent of children with anxiety disorder was considered co-morbiding depression; 44.33 percent of children with depressive disorder was considered comorbiding anxiety. Chil-dren with comorbidities had higher scores in Withdrawn(mean rank 297.7 ) than Anxiety group(220.8) and Depressive group(202.2);Somatic Complaints(268.2)than Anxiety group (232.2 ) and Depressive group(206.2 ) , Anxious/Depressed (277.2 )than Anxiety group (227.7 ) and Depressive group(219.2 )of CBCL(χ~2 =16.72～22.71,P<0.01). The subscale of "behavior" in Comorbidity group (9.1±3.3)and Anxiety group(9.7±2.9)had less scores than Depressive group(10.6±3.2);"Anxious"scores in Comorbidity group (5.8±2.7) less than Anxiety group(8.0±2.5), and Depressive group(9.2±3.0); "Popular"(5.07±2.11)less than Anxiety group (6.28±1.68)and Depressive group(6.63±1.84) ,"Happiness and satisfy"(5.1±1.8)less than Anxiety group (6.1±1.7)and Depressive group(6.0±2.0)of Piers-Harris Children's Self-concept Scale (F=4.45～74.34,P<0.01).Conclusions: Anxiety and depression were generally occurred in children. Children with comorbidity of anxiety and depression had more severe internalizing problems and less self- concept than those with anxiety or depression only. Thus, active intervention programs relevant internalizing problems would be proceeding in schools.

Abstract:Objective:To investigate the occurrence of comorbidity in children and adolescents with first-episode schizophrenia and explore the clinical features of schizophrenia with comorbidity and risk factors of comorbidity.Methods:Fifty-two children and adolescents meeting criteria for schizophrenia of the Diagnostic and Statistical Manual of Mental Disorders,Fourth Edition(DSM-IV) were included in this cross-sectional study.They were assessed with the self-made general condition questionnaire,Positive and Negative Syndrome Scale(PANSS),Kiddie-Sade-Present and Lifetime Version(K-SADS-PL),Chinese version of Mini International Neuropsychiatric Interview for Children and Adolescents(MINI Kid)(Parent Version),Global Assessment Scale(C-GAS) and the Social Disability Screening Schedule(SDSS).Results:There were 57.7%(30/52) of children and adolescents with first-episode schizophrenia having comorbidities.There were 14(26.9%) with current comorbidities,10(19.2%) with previous comorbidities,6(11.5%) with current and previous comorbidities.There were 24(46.2%) having 1 comorbidity,4(7.7%) having 2 comorbidities,2(3.8%) having 3 comorbidities.The comorbidities included 19 with depression disorder(4 with suicide behavior),5 with generalized anxiety disorder,1 with panic disorder,2 with obsessive-compulsive disorder,2 with social phobia,5 with attention deficit hyperactivity disorder(ADHD),7 with tic disorder(5 with transient tic disorder and 2 with de la Tourette' s syndrome),2 with pervasive developmental disorder(both were Asperger' s syndromes).The previous comorbidities included 5 with depression disorder,1 with panic disorder,2 with obsessive-compulsive disorder,1 with social phobia,2 with ADHD,7 with tic disorder.The current comorbidities included 14 with depression disorder,5 with generalized anxiety disorder,2 with obsessive-compulsive disorder,1 with social phobia,3 with ADHD,2 with pervasive developmental disorder.The group with comorbidity had higher scores of total PANSS scale,positive scale and general psychopathic scale than group without comorbidity(Ps<0.05).The group with current comorbidity had longer course of disease than the group with previous comorbidity(Ps<0.05).Conclusion: It is guggcsted the comorbidity in children and adolescents with first-episode schizophrenia be common.Comorbidity exacerbates the symptom of child and adolescents with schizophrenia,so it is very important to pay high attention to comorbidity.
Population-Based Psychiatric Comorbidity in Children and Adolescents With Autism Spectrum Disorder: A Meta-Analysis

Abstract:Psychiatric comorbidity in autism spectrum disorder (ASD) is a subject of critical scientific importance, affecting the quality of life, prognosis, and functional outcomes. The prevalence of psychiatric disorders vary considerably according to variables such as index subject characteristics, study setting, sampling frame, diagnostic methods used, as well as country of geographic origin. To date, most studies comprise clinical or treatment referral samples in tertiary care or subjects enrolled in clinical trials and genetic cohort collections. Such samples carry the potential for overestimation of both the frequency and severity of psychiatric comorbidity. A systematic literature search was performed using PubMed and Web of Science databases restricted to population-based study publications in the English between May 1, 2015, and May 31, 2020. A comprehensive keyword list was generated to investigate co-occurrence of psychiatric disorders in children and adolescents with ASD. A wide range of DSM-5 based disorders such as anxiety, mood, ADHD, intellectual disability/intellectual developmental disorder, eating/feeding, gender dysphoria and sleep-wake disorders were assessed. Initial search revealed a total of 1674 articles after removal of duplicates. Two independent researchers conducted a parallel-blinded screening process to identify the eligible studies based on titles and abstracts; 39 studies were analyzed in the current review. The main findings show prevalence estimates of 22.9% (95% CI: 17.7- 29.2) for intellectual disability; 26.2% (22-31) for attention-deficit hyperactivity disorder; 11.1% (8.6-14.1) for anxiety disorders; 19.7% (11.9-30.7) for sleep disorders; 7% (5.2- 9.3) for disruptive disorders; 2% (1.3- 3.1) for bipolar disorders; 2.7% (1.8- 4.2) for depression; 1.8% (0.4-8.7) for obsessive-compulsive disorder; and 0.6% (0.3-1.1) for psychosis. Psychiatric comorbidity in population-based studies is lower than in clinical and referred samples. However, our results also indicate that the frequency of psychiatric comorbidity in children and adolescents with ASD in the population context is considerable, without the influence of referral bias implicit in clinical and treatment samples. There is a need for better targeted diagnostic tools to detect psychiatric comorbidity in children and youth in future population-based studies, as an essential component in providing care as well as new insights into the nature and mechanisms of its underlying associations. Abstract:Aim: Comorbid psychopathology refers to having a diagnosis of two or more co-occurring psychological disorders. The current study investigated the differences between children and adolescents with no-mild, moderate and severe comorbid psychopathology in children and adolescents with ASD.Method: Parents of 133 children completed the Autism Spectrum Disorder-Comorbid for Children, Behavior Problems Inventory-Short Form, Pediatric Quality of Life Inventory, Vineland Adaptive Behavior Scales, Social Communication Questionnaire, Short Sensory Profile, and Behavioral/Educational Interventions and Complementary/Alternative Medicine (CAM) Interventions of the Autism Treatment Network Registry Parent Baseline Assessment.Results: A significant difference was found between severity of comorbid psychopathology and all types of challenging behavior and all sensory issues except movement. A small effect size was also found between comorbid psychopathology and quality of life.Conclusion: The findings from this study show significant difficulties associated with those with comorbid psychopathology in ASD in challenging behavior, sensory issues and quality of life.

Prevalence and treatment of mental, behavioral, and developmental disorders in children with co-occurring autism spectrum disorder and attention-deficit/hyperactivity disorder: A population-based study

Abstract:There is a lack of nationally representative studies examining the co-occurrence of autism spectrum disorder (ASD) and attention-deficit/hyperactivity disorder (ADHD) in children. This study examines comorbid mental, behavioral, and developmental disorders (MBDDs) and associated treatment modalities for children with co-occurring ASD and ADHD. Cross-sectional analyses were conducted using data from the pooled 2016-2018 National Survey of Children's Health (sample n = 102,341). Nationally representative prevalences were estimated for sociodemographic variables, comorbidities, psychotropic medication, and behavioral treatment. We assessed multivariable associations between co-occurring ASD + ADHD and MBDDs, use of psychotropic medication, and receipt of behavioral treatment after adjustment for sociodemographic confounders. Compared to children with ASD without co-occurring ADHD, children with ASD + ADHD had higher prevalence of most MBDDs, including anxiety (AOR 4.03 [95% CI 2.77, 4.87]), depression (AOR 3.08 [95% CI 1.77, 5.36]), behavior or conduct problems (AOR 4.06 [95% CI 2.72, 6.06]), and other mental health conditions. Similarly, compared to children with ADHD without ASD, children with ASD + ADHD had higher odds of anxiety (AOR 3.49 [95% CI 2.65, 4.61]), depression (AOR 1.67 [95% CI 1.21, 2.29]), behavior or conduct problems (AOR 2.31 [95% CI 1.68, 3.17]), and other mental health conditions. Children with ASD + ADHD were significantly more likely to take psychotropic medication than children with ASD without ADHD. Among children with ASD + ADHD, males had higher odds of receiving behavioral treatment, whereas older children and adolescents were more likely to take psychotropic medication. A multidisciplinary approach is necessary to support the complex needs of these children.

Abstract:Autism and attention deficit hyperactivity disorder (ADHD) comorbidity in the school population have been understudied. This study estimates its prevalence considering both parents' and teachers' reports and clinical diagnosis. Sociodemographic, clinical, and cognitive data were compared by diagnostic groups: autism, ADHD, autism and ADHD, subthreshold autism spectrum disorder (ASD), subthreshold ADHD, and children without neurodevelopmental conditions. Following a two‐phase design, 3727 parents and teachers (1802 preschoolers, 1925 school‐age children) participated in the first phase. Subsequently, 781 participants underwent individual assessment for DSM‐5 diagnoses. The estimated prevalence of the comorbid diagnosis was 0.51% (0.28%–0.74%), with significant sex differences (0.16% girls, 0.89% boys). The cooccurrence of symptoms of autism and ADHD reported by parents or teachers was 3.2% and 2.6%, respectively. ADHD comorbidity was observed in 32.8% of autistic children and 31.4% of those with subthreshold ASD. ASD comorbidity was observed in 9.8% of children with ADHD and 5.7% of those with subthreshold ADHD. Comorbidity was reported by at least one informant in 95% of children. Only 15.8% of children with autism and ADHD had been previously diagnosed with both conditions. Early detection and accurate comorbidity diagnosis are crucial to address the clinical and socio‐educational needs of these children.

Abstract:Background:Attention deficit hyperactivity disorder (ADHD) is a common psychiatric disorder in children that can extend into adulthood and that is otfen associated with a variety of comorbid psychiatric disorders. Aim: Assess the comorbidity of ADHD with anxiety disorders and depressive disorders in school-aged children, and the relaitonship of the severity of ADHD, anxiety, and depressive symptoms in children who have ADHD with the severity of the corresponding symptoms in their parents. Methods:A two-stage screening process identified children 7-10 years of age with and without ADHD treated at the Xin Hua Hospital in Shanghai. ADHD and other DSM-IV diagnoses were determined by a senior clinician using the Schedule for Affecitve Disorder and Schizophrenia for School-Aged Children (K-SADS-PL). One parent for each enrolled child completed three self-report scales: the ADHD Adult Self Report Scale (ASRS), the State-Trait Anxiety Inventory (STAI), and the Beck Depression Inventory (BDI). In total 135 children with ADHD and 65 control group children without ADHD were enrolled; parents for 94 of the children with ADHD and 63 of the children without ADHD completed the parental assessment scales. Results: Among the 135 children with ADHD, 27% had a comorbid anxiety disorder, 18% had a comorbid depressive disorder, and another 15% had both comorbid anxiety and depressive disorders. Parents of children with ADHD self-reported more severe ADHD inatteniton symptoms than parents of children without ADHD and were more likely to meet criteria for adult ADHD. Mothers (but not fathers) of children with ADHD had signiifcantly more severe trait anxiety and depressive symptoms than mothers of children without ADHD. Among children with ADHD, the severity of ADHD symptoms was not signiifcantly correlated with the severity of ADHD symptoms in parents, but depressive symptoms and anxiety symptoms in the children were signiifcantly correlated with the corresponding symptoms in the parents. Conclusions:School-aged children with ADHD commonly suffer from comorbid anxiety and depressive disorders, and the severity of these symptoms parallels the level of anxiety and depressive symptoms in their parents. Self-reported symptoms of ADHD are signiifcantly more common in parents of children with ADHD than in parents of children without ADHD. Longitudinal studies are needed to disentangle the geneitc, biological, and social factors responsible for these complex inter-relaitonships.

Abstract:Background: Autism spectrum disorder (ASD) often co-exists with attention deficit/hyperactivity disorder (ADHD), which may aggravate functional impairment. However, it is unclear how comorbid ADHD symptoms influence the adaptive behavior and social interaction deficits of children with ASD. Methods: The study enrolled 340 children (ranging from 2 to 14 years) with ASD, with comorbid ASD and ADHD, or with typical development (TD). A psychological evaluation involving adaptive behavior and social function was conducted using the Vineland Adaptive Behavior Scale, Second Edition (VABS-II) and the Social Responsiveness Scale (SRS). Results: There was a high prevalence of ADHD symptoms (46.6%) in children with ASD, and children with ASD + ADHD presented the worse profile of ASD symptoms. The ASD + ADHD group had higher scores on VABS and lower scores on SRS in comparison with the ASD alone group and TD group. The regression analysis revealed that ASD symptoms and ADHD symptoms were significantly associated with greater impairments in adaptive behavior and social function. The ADHD symptoms were responsible for an additional 0.8% of the variance in adaptive behavior, and 9.5% of the variance in social function. Conclusions: More severe ASD symptoms and greater impairment in adaptive function and social ability were found in children with ASD and comorbid ADHD, highlighting the need to identify ADHD comorbidities early on in children with ASD and to reduce their negative impact on functioning.

Abstract:The Kiddie Schedule for Affective Disorders and Schizophrenia was modified for use in children and adolescents with autism by developing additional screening questions and coding options that reflect the presentation of psychiatric disorders in autism spectrum disorders. The modified instrument, the Autism Comorbidity Interview-Present and Lifetime Version (ACI-PL), was piloted and frequently diagnosed disorders, depression, ADHD, and OCD, were tested for reliability and validity. The ACI-PL provides reliable DSM diagnoses that are valid based on clinical psychiatric diagnosis and treatment history. The sample demonstrated a high prevalence of specific phobia, obsessive compulsive disorder, and ADHD. The rates of psychiatric disorder in autism are high and are associated with functional impairment.

Abstract:Background: Autism spectrum disorder (ASD) is a multifactorial, pervasive, neurodevelopmental disorder, of which intestinal symptoms collectively represent one of the most common comorbidities. Methods: In this study, 1,222 children with ASD and 1,206 typically developing (TD) children aged 2-7 years were enrolled from 13 cities in China. Physical measurement and basic information questionnaires were conducted in ASD and TD children. The Childhood Autism Rating Scale (CARS), Social Responsiveness Scale (SRS), and Autism Behavior Checklist (ABC) were used to evaluate the clinical symptoms of children with ASD. The six-item Gastrointestinal Severity Index (6-GSI) was used to evaluate the prevalence of intestinal symptoms in two groups. Results: The detection rates of constipation, stool odor, and total intestinal symptoms in ASD children were significantly higher than those in TD children (40.098% vs. 25.622%, 17.021% vs. 9.287%, and 53.601% vs. 41.294%, respectively). Autistic children presenting with intestinal comorbidity had significantly higher scores on the ABC, SRS, CARS, and multiple subscales than autistic children without intestinal symptoms, suggesting that intestinal comorbidity may exacerbates the core symptoms of ASD children. Conclusion: Intestinal dysfunction was significantly more common in autistic than in TD children. This dysfunction may aggravate the core symptoms of children with ASD.
Prevalence of autism in first-episode psychosis in two Hong Kong teaching hospitals

Abstract:Autism, Ahead of Print. This study estimated the prevalence of autism spectrum disorder in a 5-year inpatient cohort of children and adolescents diagnosed with first-episode psychosis in two Hong Kong teaching hospitals, and explored the clinical characteristics predicting first-episode psychosis–autism spectrum disorder comorbidity. The autism spectrum disorder diagnoses of 103 patients with first-episode psychosis were confirmed by Autism Diagnostic Interview–Revised. Based on the (1) Diagnostic and Statistical Manual of Mental Disorders (5th ed.) diagnostic checklist and (2) Autism Diagnostic Interview–Revised, three major observations were reported: (1) the prevalence of autism spectrum disorder among children and adolescents with first-episode psychosis was 28.2% (95% confidence interval = (19.7%, 37.9%)); (2) after administering the Autism Diagnostic Interview–Revised, an additional 31.0% of first-episode psychosis patients met the clinical diagnostic cut-off for autism spectrum disorder; and (3) multivariate binary logistic regression analysis showed that first-episode psychosis patients with a comorbid autism spectrum disorder diagnosis were more likely to be a male than a female (adjusted odds ratio = 3.57, 95% confidence interval = (1.29, 9.86), p = 0.014). Adding to the current literature, this study examined first-episode psychosis–autism spectrum disorder comorbidity with a large Chinese, inpatient sample with first-episode psychosis. Consistent with studies conducted overseas, we show that autism spectrum disorder is a common comorbidity among Chinese first-episode psychosis patients. A comprehensive autism spectrum disorder assessment that incorporates both observation-based and parent-report diagnostic tools is warranted for children and adolescents, especially boys, with psychosis.Lay abstractAutistic features are commonly observed in children and adolescents with first-episode psychosis, but they are sometimes overlooked by clinicians and caregivers. By comprehensively examining the clinical profiles of 103 children and adolescents (below 18 years old) with first-episode psychosis and conducting the Autism Diagnostic Interview–Revised (the 'gold standard' autism diagnostic tool) with their primary caregivers, we showed that around 28% of patients with first-episode psychosis had a comorbid autism diagnosis, and boys were 3.57 times more likely to have first-episode psychosis–autism spectrum disorder comorbidity than girls. After administering the Autism Diagnostic Interview–Revised, we also observed that an additional 30% of patients with first-episode psychosis met the autism spectrum disorder diagnostic cut-off; their autism spectrum disorder symptoms were probably overshadowed by prodromal psychotic symptoms and left undetected before this study. The co-occurrence of autism and first-episode psychosis might be more common than we previously thought. Careful autism screening and assessment is highly recommended for clinicians working with patients with psychosis.

Abstract:The comorbidity of epilepsy and autism is a common clinical phenomenon.About 5％～ 37％ of children with epilepsy have autism or have positive screening for autism.About 2％～ 46％ of individuals with autism have epilepsy.The family history of mental disorders,adverse perinatal factors,female,autistic features,intelligence disability,genetic or neurological syndromes and genetic factors increase the risk of comorbidity of the two diseases.In children with the two diseases,epilepsy onsets earlier with two onset peaks before 5 years and during puberty;partial seizure and intractable epilepsy is more common;the symptom of autism is more severe,with more intelligence disability,more motor development and behavior problems and worse adaption behaviors.It is important to recognize,diagnose and treat the two diseases.

Abstract:OBJECTIVE:To study age distribution of attention deficit hyperactivity disorder (ADHD) comorbidities in a relatively large sample, and exam the hypothesis of bad prognosis for ADHD.METHODS:Using semi-structured clinical diagnosis interview scale, we investigated comorbidities of 1,002 ADHD children and adolescents from psychiatric out-patient clinic, and compared comorbidity frequency in 4 age groups.RESULTS:The comorbidity frequencies of disruptive behavior disorder (DBD), mood disorder, tics disorder and learning disorder (LD) in different age groups of ADHD patients differed significantly (P<0.01). DBD and mood disorder in the age group of 12-14 years (124 cases,51.5%;18 cases,7.5%, respectively) were more than those in the 6-8 years group (160 cases,41.7%;9 cases,2.3%, respectively; P<0.05). And the comorbidity frequencies of the 15-17 years group were significantly higher than those in other age groups (P<0.05). Tics disorder in the 12-14 years group (50 cases,20.8%)was more than those in the two age groups of less than 12 years group (for the 6-8 years group, 51 cases,13.4%;for the 9-11 years group , 42 cases,12.5%; P<0.05).The comorbidity frequency of learning disorder grew up with age. The comorbidity frequency of the 9-11 years group (106 cases,31.5%) was significantly higher than that of the 6-8 years group (80 cases,20.9%; P<0.01); and those of both age groups more than 12 years (for the 12-14 years group, 164 cases,68.0%;for the 15-17 years group, 28 cases,70.0%) were higher than those of the two age groups of less than 12 years (P<0.01). ADHD patients without any comorbidities in the two age groups more than 12 years (for the 12-14 years group, 29 cases,12.0%;for the 15-17 years group, 4 cases,10.0%)were less than those in the two age groups of less than 12 years (for the 6-8 years group, 126 cases,32.8%;for the 9-11 years group, 110 cases,32.6%; P<0.01). These characteristics maintained in boys.CONCLUSION:Many comorbid psychiatric disorders increase as the ADHD children become adolescents. The situation is severer in late adolescence. Therefore, ADHD children should accept appropriate treatment as early as possible, and adopt the second degree prevention actively.

Abstract:Objective To investigate the prevalence of autism spectrum disorders (ASD) of children aged 4-6 years in public kindergartens in Shanghai.Methods Songjiang district and Xuhui district were selected randomly as the representative sample.By means of "Clancy Autism Behavior Scale","Social Communication Questionnaire" and "high-functioning Autism Spectrum Screening Questionnaire",all of the 33 public kindergartens in chosen area,which contained 10 385 children aged 4-6 years,were surveyed.Those who were screened as suspected cases would be further diagnosed by "Autism Diagnostic Interview-Revised" and "Diagnostic and Statistical Manual of Mental Disorders,Fifth Edition".Results Among 9 665 valid questionnaires,9 children were diagnosed with ASD.The ratio of male to female was 8:1,the point prevalence rate was 0.93‰.The results of ADI-R corresponded with that of DSM-5.There were two children who never had medical records.For others,"language" problem was the most likely reason for their parents to seek medical help,while "deficits in social communication" was the main symptom of patients.Conclusion The prevalence of ASD was lower than those recorded in literature,home and abroad which might be related to some patients not going to public kindergartens.
Comorbidity of Anxiety and Depressive Disorders in Children

Abstract:Objective To investigate the rates and clinical features of comorbidity of anxiety and depressive disorder in out-patient clinic.Methods One hundred and four children and adolescents with anxiety and depression meeting the CCMD-3 diagnostic criteria were evaluated with the clinician-, parents-and self-report rating scales and the Achenbach Child Behavior Check List (CBCL). The subjects were divided into three groups, 41 children with anxiety disorder, 31 with depression, and 31 with comorbidity of anxiety and depression.Results The anxiety group had higher scores on the CBCL subscales of activities and total competence than the other two groups. The comorbidity group had higher scores on the subscales of withdrawal, anxiety/depression, and internalizing than the anxiety and depression group, and higher score on the attention, aggressive subscale than anxiety group. The comorbidity group also had higher total score on the Screen for Child Anxiety Related Emotional Disorders than the other groups. The anxiety group had higher total score on the clinician-rating children′s global assessment scale than the depression and comorbidity groups. Conclusion The comorbidity of anxiety and depression is common in children with mood disorder, and more internalization problem, attention deficit, aggressive behavior and poor social competence can be seen in these children.

Abstract:This study aimed to obtain the first national estimate of the prevalence of autism spectrum disorder (ASD) in Chinese children. We targeted the population of 6 to 12-year-old children for this prevalence study by multistage convenient cluster sampling. The Modified Chinese Autism Spectrum Rating Scale was used for the screening process. Of the target population of 142,086 children, 88.5% (n = 125,806) participated in the study. A total of 363 children were confirmed as having ASD. The observed ASD prevalence rate was 0.29% (95% CI: 0.26%–0.32%) for the overall population. After adjustment for response rates, the estimated number of ASD cases was 867 in the target population sample, thereby achieving an estimated prevalence of 0.70% (95% CI: 0.64%–0.74%). The prevalence was significantly higher in boys than in girls (0.95%; 95% CI: 0.87%–1.02% versus 0.30%; 95% CI: 0.26%–0.34%; P < 0.001). Of the 363 confirmed ASD cases, 43.3% were newly diagnosed, and most of those (90.4%) were attending regular schools, and 68.8% of the children with ASD had at least one neuropsychiatric comorbidity. Our findings provide reliable data on the estimated ASD prevalence and comorbidities in Chinese children.

The Comorbidity of Schizophrenia Spectrum and Mood Disorders in Autism Spectrum Disorder

Abstract:Individuals with autism spectrum disorder are often diagnosed with at least one or more accompanying disorders. Most studies reported prevalence of the psychiatric comorbidities among these individuals; however, the incidence of developing comorbidities is unclear. This study used Taiwan's claims database and aimed to investigate the incidence of developing major psychiatric comorbidities in individuals with autism spectrum disorder and whether the incidence was moderated by gender, autism-spectrum disorder subtypes, and autism-associated neurodevelopmental conditions. A total of 3,837 individuals with autism spectrum disorder (2,929 autistic disorder, 447 Asperger syndrome, 461 pervasive developmental disorder-not otherwise specified) and 38,370 comparison subjects, who were matched by age and gender, were included. The incidences of schizophrenia spectrum, bipolar, and major depressive disorders was examined. The results showed that the incidences of schizophrenia spectrum (9.7 per 1,000 person-year), bipolar disorder (7.0 per 1,000 person-year), and major depressive disorder (3.2 per 1,000 person-year) were significantly higher than the comparison group across all three subtypes of autism-spectrum disorder. Individuals with pervasive developmental disorder-not otherwise specified had higher risk for major depressive disorder than autistic disorder. Females with Asperger syndrome had significant higher risk for schizophrenia spectrum than males. The comorbidity rate dramatically dropped when the autism-associated neurodevelopmental conditions were taken into account. Our findings suggested that the incidences of major psychiatric comorbidities were higher in autism spectrum disorder and influenced by autism subtypes, gender, and autism-associated neurodevelopmental conditions. LAY SUMMARY: We examined whether people with autism spectrum disorder (ASD) have higher incidence of schizophrenia, bipolar disorders, and major depression using a large claims database. The results showed the incidences of these mental illness among individual with ASD were significantly higher than those without ASD. In addition, the incidences were influenced by autism subtypes, gender, and comorbid neurodevelopmental conditions.

Abstract:Autism spectrum disorder (ASD) is a heterogeneous group of neurodevelopmental disorders that can affect up to 2.6% of the population. Most of these people will have at least one other psychiatric disorder, often diagnosed with a delay or not recognized at all. This study describes the epidemiology, diagnostic difficulties and potential treatment of patients with ASD and comorbid bipolar disorder (BD). The prevalence of bipolar disorder in ASD is estimated at 5-8%. The study with the most numerous group included 700,000 children out of which 9,062 fulfilled ASD criteria at 16 - BD was found to be 6 times more prevalent in this group compared to the control group. Many factors affect the diagnosis. Patients with ASD often have limited insight into understanding the complex emotional states and difficulty in expressing them due to their impairment. The symptoms of bipolar disorder are in their case unspecific and differ from those occurring in the general population, which makes it difficult to make proper diagnosis. Despite the lack of research on the group of patients with ASD, psychometric tools designed to evaluate the general population are used to examine patients. This work aims to show the current state of knowledge and highlight areas that require further investigation.
Longitudinal follow-up of subsequent psychiatric comorbidities among children and adolescents with autism spectrum disorder

Abstract:Background: The mental health of children and adolescents with autism spectrum disorder (ASD) is a concern of recent years. However, a large-scale longitudinal study investigating the risk and the time course of subsequent psychiatric comorbidities is still lacking. Methods: Using the Taiwan National Health Insurance Research Database, 13,382 children and adolescents with ASD, and 53,528 age- and sex-matched non-ASD controls were enrolled between 2001 and 2009, and followed to the end of 2011. The adjusted hazard ratio (HR) with a corresponding 95 % confidence interval for psychiatric comorbidities among children and adolescents with ASD vs matched controls was estimated. The subjects who developed schizophrenia, bipolar disorder, depressive disorder, anxiety disorder, and obsessive-compulsive disorder (OCD) were identified during the follow-up. Results: Children and adolescents with ASD compared with controls were more likely to be diagnosed with schizophrenia (19.21; 13.74, 26.88), bipolar disorder (17.59; 12.66, 24.44), depressive disorder (5.56; 4.72, 6.56), anxiety disorder (5.01; 4.49, 5.59), and OCD (16.12; 11.66, 22.30) later in life. The time course of subsequent psychiatric comorbidity showed that anxiety disorder occurred first, usually in late childhood, with psychotic and affective disorders proceeding in adolescence. Those with ASD and anxiety disorder had an additionally increased likelihood of developing subsequent psychiatric comorbidity compared with those with ASD only. Limitation: In claims data analysis, clinical parameters or possible confounders may not be fully captured. Conclusion: Patients with ASD are predisposed to the development of anxiety disorder in late childhood, as well as schizophrenia, bipolar disorder, depressive disorder, and OCD in adolescence.
