Abstract:Aim: To investigate the neuropsychological characteristics of the patients with Parkinson' s disease(PD), Alzheimer' s disease(AD) and vascular dementia (VD) for differential diagnosis. Methods: Mini-mental state examination(MMSE), abilities of daily living scale(ADL) and Wechsler memory scale(WMS) were performed to assay cognitive function of PD(n = 30), AD(n = 30) and VD(n = 30) patients as well as 30 normal elder volunteers, respectively, for comparison between the groups. Results: 1 MMSE: The total score decreased by 8.3% for the PD group, 33.0% for the AD group and 25.7% for the VD group. The orientation, attention and calculation of the PD patients were close to normal level, but those of the AD and VD patients were decreased dramatically. The delayed memory was lowest in the AD patients, and there was no significant difference between the VD and PD groups. 2 ADL: The scores of ADL were better in the AD group than the VD and PD, groups, but there was the no significance among the three groups. 3 WMS: Memory quotient(MQ) was the best in the PD group, better in the VD group and the worst in the AD group. The long-term memory of the PD patients was close to normal level, which was worse in the VD and AD patients. The scores of short-term memory and immediate memory in the AD, VD and PD groups were all lower than those in the normal control group, and the scores were the highest in the PD group and the lowest in the AD group. The scores of the PD, group were higher than those of the VD and AD groups in orientation, counting from 100 - 1, accumulation, visual reproduction, association study and digit span, with significant difference(P < 0.01). In addition, the scores in visual recognition and card recall of the PD group were higher than those of the AD group(P < 0.01). Conclusion: Compared with the normal elderly, different degree of cognitive dysfunction exists in the AD, VD and PD groups. Especially, an overall cognitive dysfunction can be found in the AD and VD patients. The cognitive dysfunction of PD mainly presents a decrease in the short-term and immediate memory, including visual memory, tactile memory, digital span and language function.
