Abstract:Symptomatic carotid stenosis is closely associated with ischemic stroke,its treatment strategies include risk factor control,medical therapy,and revascularization.This article reviews and summarizes the clinical research of treatment methods in recent years,such as symptomatic carotid stenosis related antiplatelet therapy,antihypertensive treatment,lipid-lowering therapy,carotid endartereetomy and stenting,as well as the treatment strategies recommended by the guidelines.

Abstract:Chronic atherosclerotic occlusion of carotid can lead to low cerebral blood flow and increase the risk of stroke.Drug therapy is the first choice for asymptomatic patients,while the patients with recurrent neurological symptoms may need surgical treatment.Extracranial-intracranial artery bypass (EIAB) is not better than medicine treatment.The operation risks of carotid endarterectomy (CEA) and carotidartery stent (CAS) are high with low success rate and the therapeutic effects remain to be verified.Therefore,the surgical indications should be strictly controlled.In this paper,the current status of treatment for chronic carotid artery occlusion was reviewed.

Abstract:Carotid atherosclerotic plaque is the main cause of ischemic stroke. In recent years, with the continuous innovation of novel imaging technologies, numerous classification standards for carotid plaques provide more powerful evidence for the features of carotid plaques and perioperative vascular assessment, as well as the reference for surgeons in choosing therapeutic decisions. Ultrasound is the preferred non-invasive and convenient screening tool for carotid stenosis. Invasive examinations such as CT angiography and magnetic resonance angiography are suitable for carotid stenosis patients to determine the plaque composition and stability, which can guide surgical decision-making and help to prevent serious cardiovascular and cerebrovascular adverse events. Advances in the treatment of carotid artery stenosis have focused on the improvement and innovation of vascular interventional devices and surgical procedures, including double-layer stents, coated stents and transcarotid artery revascularization. As technology continues to evolve, molecular imaging and more minimally invasive screening as well as therapies will be the way forward.

Abstract:Stroke is one of the diseases that seriously threaten the survival and health of human beings. In Europe and the United States, stroke is the second leading cause of death after heart disease and tumors.([1])Stroke is also one of the fatal diseases in Asian countries. On average, about 1.5 million new stroke patients are added each year in China, and the incidence of stroke is increasing year by year. About 80% of all stroke patients are ischemic stroke, and symptomatic internal carotid atherosclerotic stenosis is another important cause of ischemic stroke. Ipsilateral carotid stenosis >= 50% increases the incidence of transient cerebral ischemia and stroke in the carotid artery region by 10% to 15%, and is also closely related to the recurrence of acute and long-term stroke in patients.([2])Therefore, the clinical application and efficacy of carotid stenting in patients with symptomatic internal carotid artery stenosis are analyzed and evaluated to provide a basis for the selection of clinical treatment options. The clinical data of patients with symptomatic carotid stenosis who underwent carotid stenting and the clinical data of conservative treatment of patients with symptomatic carotid stenosis were retrospectively analyzed, and the carotid stenosis rate, symptoms, and National Institute of Health stroke scale where compared before and after surgery. And activities of daily living score. Because the control group treatment method in this article is completely free for patients to choose, and belongs to a retrospective analysis, the results suggest that it can provide a high-quality treatment approach for the treatment of patients with symptomatic carotid stenosis, without causing any harm to the patient, So no ethical approval is needed, and no patient informed consent is required. In recent years, with the continuous advancement of science and technology and new stent materials, intravascular interventional technology has developed rapidly. In continuous clinical practice and research, the safety and effectiveness of stent technology have also been gradually improved. Arterial stenting has gradually become an important method for the treatment of atherosclerotic carotid stenosis. This technique can not only improve the symptoms and prognosis of patients with symptomatic internal carotid stenosis, but also prevent the occurrence of ischemic events. The promotion of this technology has the effect of reducing disability and mortality. The alternative therapy of drug therapy, namely arterial stent implantation, has become a new way to treat atherosclerotic stroke. This treatment technology can quickly relieve the abnormal hemodynamics of distal blood vessels caused by arterial stenosis, which is ischemic. Cerebrovascular disease provides new ideas for treatment. Carotid angioplasty and stenting for symptomatic internal carotid stenosis under a distal cerebral protection device is a safe and effective treatment.

Abstract:Objective To promote the application of angioplasty and stenting technique for the treatment of carotid artery stenosis in China. Methods The angioplasty and stenting were performed in 12 cases with carotid artery stenosis, of which 7 were symptomatic. Of 12 cases, 10 were atherosclerotic and the other 2 were of Takayasu′s disease. Results 11 stents were successfully implanted. Minor stroke occurred in 2 cases during the procedure and both were completely recovered with thrombolytic treatment in less than 1 hour. Follow up of the 11 cases 6 months through 4 and half years, no restenosis and ischemic symptoms were revealed. Conclusion Endovascular stent placement for carotid artery occlusive disease is evolving to an accepted alternative to surgery. The use of cerebral protection during the proceture is needed in the future.

Abstract:Objective To study the indications and process and perioperative managements of carotid endarterectomy (CEA) in treatment of stenosis and occlusion in carotid.Methods Make a retrospective study of 128 patients with carotid stenosis underwent CEA from January 2006 to December 2008. Results Clinical symptom significantly alleviated in most cases, and the cerebral blood streaming of them got obvious increase demonstrated by Duplex scanning postoperetively. The perioperative death was zero.Conclusion Carotid endarterectomy is an effective treatment for severe stenosis of internal carotid artery.

Abstract:Objective To discuss the therapeutic method of extracranial athemsclerotic carotid artery stenosis . Methods A retrospective analysis was conducted in 51 patients with angiography confirmed carotid artery stenosis from January 2012 to June 2012 in our hospital.The carotid endarterectomy (CEA) was performed in 16 cases, while the carotid artery stenting (CAS) was performed in 35 cases. Results All the 51 operations were successfully completed .There were 1 case of transient ischemic attack ( TIA) 3 days after CEA operation , 1 case of postoperative stroke after CAS operation , and 1 case of carotid sinus pressure after CAS operation . The follow-up period of all the cases was 9-15 months, with an average of 13.6 months.Ultrasound review of the carotid artery found no restenosis. Conclusion According to medical information of the patients with extracranial carotid atherosclerotic stenosis , we tend to adopt the CAS treatment in patients with following situations: ①transient ischemic attack at least once within 6 months, with symptoms or signs lasting for more than 24 hours and the degree of carotid stenosis ≥70%;②mild than disabling stroke onset at least once within 6 months, with symptoms or signs lasting for more than 24 hours and the degree of carotid stenosis ≥70%; ③carotid artery stenosis ≥2 cm under neck vascular CTA and cerebral angiography .We tend to adopt CEA treatment in patients with following situations:①asymptomatic carotid stenosis with degree ≥70%;②symptomatic carotid stenosis with degree range from 50%to 69%;③asymptomatic carotid artery stenosis with degree <70%, but with instable situation of lesions under angiography or other examinations .

Abstract:OBJECTIVE:To explore reasonable clinical decision in treating carotid artery stenosis under different conditions.METHODS:The data of 133 carotid artery stenosis patients were retrospectively analyzed. Of the patients, 46 cases were treated with carotid angioplasty and stenting (CAS), 87 patients received carotid endarterectomy (CEA). The length of hospital stay and National Institutes of Health Stroke Scale (NIHSS) grade before and after treatment in both groups were observed; the forward flow were assessed by digital subtraction angiography (DSA) before and after treatment; the degree of carotid artery stenosis were determined by using ultrasound during 3 to 24 months after treatment in both groups; the cumulative incidence of major cardiovascular events was concentrated, including appearance of death, stroke or myocardial infarction during 30 days after CAS and CEA and death or homonymy stroke during 31 days to 2 years.RESULTS:Significant difference was found in hospital stay and when NIHSS exceed 20 after treatment between the two groups (P < 0.05); there was no significant difference in the forward flow before and after treatment in both groups; the carotid artery stenosis had been improved significantly after the operation in both groups; the cumulative incidence of major cardiovascular events in CEA group was significantly higher than in CAS group in 30 days after the operation (P < 0.05), but no statistical difference in 31 days to 2 years after the operation.CONCLUSIONS:CAS and CEA has equivalent effects in treating carotid artery stenosis, and should be selected according to the location of stenosis, etiological factors and the condition of opposite carotid artery.

Abstract:Carotid stenosis (CS) is a buildup of atherosclerotic plaque within the artery leading to a wide range of symptoms, from mild symptoms, including blurred vision and confusion, to much more life-threatening presentations, including paralysis due to stroke. The presentation is insidious, with symptoms exhibiting predominantly at severe stenosis; hence the emphasis is placed on the importance of early diagnosis, treatment, and lifestyle modifications. CS is seen undergoing almost the same pathogenesis of any atherosclerotic plaque formation, from endothelial damage of the artery lumen to the formation of a fibrous cap with a foam cell, lipid-filled core. The findings of our review article were consistent with the recent literature, depicting that comorbid hypertension, diabetes, and chronic kidney disease (CKD), and lifestyle aspects, including smoking and diet, played the most salient role in plaque development. Among several imaging modalities, duplex ultrasound (DUS) imaging is the widely preferred method in clinical practice. Carotid endarterectomy (CEA) and carotid stenting are the primarily advocated procedures for symptomatic severe stenosis, with similar long-term outcomes. Although, earlier clinical trials showed promising results in mitigating the risk of stroke among asymptomatic severe CS with surgical intervention. However, recent advancements have shifted the focus to medical management alone due to comparable results among the asymptomatic population. Both surgical and medical regimens are beneficial in treating patients, but it is still an ongoing debate as to which is predominantly superior. The currently advancing trials and research will help elucidate definitive guidelines. However, the massive impact of lifestyle modifications advocates some degree of individualized multidisciplinary management strategies.

Abstract:Objective To evaluate the surgical treatment of carotid artery stenosis. MethodAccording to the level and degree of the stenosis, different operations were performed on 120 patients who suffered from the extracranial carotid stenosis. Treatment result was retrospectively reviewed. ResultSurgery was successful in all the patients. We performed carotid endarterectomy on 111 cases and other operations on 9 cases. The incidence of postoperative complications was low, especially for carotid endarterectomy. ConclusionCarotid endarterectomy is still the main therapy for the treatment of carotid artery stenosis with a satisfactory result.

Abstract:Carotid endarterectomy (CEA) is the accepted standard therapy for prevention of stroke in patients with high-grade symptomatic and asymptomatic carotid stenoses. In recent years, the endovascular treatment of carotid artery stenosis has been widely used in clinical practice, including balloon angioplasty and stenting. Both single-center and multicenter studies worldwide have demonstrated good therapeutic efficacy, especially in patients with high risk of CEA. The efficacy of carotid angioplasty and stenting with an emboli-protection device are much better. A growing body of evidence has indicated that endovascular treatment may become another important approach after CEA in the treatment of carotid atherosclerotic stenosis.

Abstract:Carotid artery stenosis is a well-recognized risk factor for ischemic stroke.In the late 1980s and in the 1990s,well-designed randomized trials established a definite advantage for carotid endarterectomy.Although those landmark trials have helped to establish the current guidelines,there still remains a great deal of controversy regarding the technical approach to carotid revascularization.In particular,current medical treatment has drastically decreased the risk of stroke in patients with asymptomatic carotid stenosis.Selecting patients amenable to endovascular or surgical treatment still needs to be resolved.
Management of The Carotid Artery Stenosis in Asymptomatic Patients

Abstract:Background: An asymptomatic carotid stenosis (CS) is defined as a stable atherosclerotic luminal narrowing in patients with no history of ipsilateral cerebral or ocular ischemic events in the past six months. The bifurcation of the common carotid artery makes this area vulnerable to atherosclerosis due to the features of haemodynamic flow. The exact prevalence of asymptomatic patients with CS remains unknown and opinions on the treatment of these patients are controversial. Objective: The authors aimed to review the evidence on the management of the asymptomatic CS and describe its clinical characteristics, diagnosis and treatment management. Methods: A comprehensive review of the literature was carried out to collate data from relevant studies concerning patients with extracranial moderate to severe asymptomatic carotid stenosis. The data used was identified by a search using PubMed and Google Scholar with the keywords / MESH terms "carotid stenosis", in combination with the term "asymptomatic". For this study, the authors focused on publications in the past two decades, using English publications. Results: A few studies have addressed the prevalence, natural course and/or prognostic impact of asymptomatic CS in patients under medical treatment or undergoing vascular surgery procedures. The prevalence of asymptomatic CS ranged from 0.3% to 4.5% in women and 0.5% to 5.7% in men - The risk of stroke/TIA in these patients was reported between 2% to 5% annually with a downward trend across time to 0.5% with current best medical therapy. Conclusion: A great proportion of patients with asymptomatic CS should be submitted to conservative management with best medical therapy. However, selective surgical management should be considered if high risk features are present.

Abstract:Objective:To evaluate the efficacy and complications of carotid artery stenting(CAS) in the treatment of carotid stenosis.Methods:The clinical data of 20 patients with carotid stenosis,who underwent 21 CAS procedures from August 2009 to September 2011,was analyzed retrospectively.Results:The procedural success rate was 100%.The average stenosis degree of 21 carotid arteries was(83±13)% before treatment and(10±5)% after treatment;During the procedures,four patients suffered from bradycardia and hypotension,vasospasm and cerebral hemorrhage occurred respectively in one case;Within one month after CAS,three patients suffered from bradycardia and hypotension,high perfusion syndromes occurred in one patient,major stroke was found in one patient,one patient died after two days;During long-term follow-up after surgery using carotid ultrasound or CTA or MRA and telephone,out of the nineteen patients with symptoms of varying degrees of cerebral ischemia,sixteen had improved,one patient with arteritis developed approximately 60% restenosis after 9 months.Conclusion:CAS is a safe and effective method to treat carotid stenosis,but we should pay attention to the prevention and treatment of its complications.

Abstract:Carotid artery stenosis is a condition where the carotid artery is blocked by fatty cholesterol deposits called plaque, increasing the risk of stroke. Elderly individuals with high cardiovascular risk are more susceptible, along with smokers, those with high cholesterol, males, and older individuals. Young females may also be affected by fibromuscular dysplasia. Carotid stenosis significantly raises stroke risk, and the severity is closely linked to stroke incidence and other cardiovascular events. Early detection and treatment are essential to prevent complications. Treatment options include medical and surgical interventions, such as carotid endarterectomy (CEA) and carotid artery stenting (CAS). The choice between surgery and medical management varies depending on patient characteristics and risk factors. This review explores carotid artery stenosis pathophysiology, risk factors, the importance of early detection and treatment, and the surgical approaches of CEA and CAS, addressing their roles and controversies. Healthcare professionals must understand these aspects to provide optimal care to patients with this condition.

Abstract:Stroke induces heavy socioeconomic burdens particularly in developing countries. Considering the multisectional discrepancies, strategies of stroke prevention and management adapted for western populations are not always applicable for patients in developing countries. Currently, carotid endarterectomy is the recommended treatment for severe carotid stenosis in the west. Carotid angioplasty and stenting is an alternative treatment only for patients with a high surgery risk. However, in developing countries such as China, where carotid endarterectomy is widely unavailable, Carotid angioplasty and stenting has developed rapidly in recent years and is becoming a choice of treatment for occlusive carotid diseases.

Abstract:Despite recent therapeutic advances, acute ischemic complications of atherosclerosis remain the primary cause of morbidity and mortality in Western countries, with carotid atherosclerotic disease one of the major preventable causes of stroke. As the impact of this disease challenges our healthcare systems, we are becoming aware that factors influencing this disease are more complex than previously realized. In current clinical practice, risk stratification relies primarily on evaluation of the degree of luminal stenosis and patient symptomatology. Adequate investigation and optimal imaging are important factors that affect the quality of a carotid endarterectomy (CEA) service and are fundamental to patient selection. Digital subtraction angiography is still perceived as the most accurate imaging modality for carotid stenosis and historically has been the cornerstone of most of the major CEA trials but concerns regarding potential neurological complications have generated substantial interest in non-invasive modalities, such as contrast-enhanced magnetic resonance angiography. The purpose of this review is to give an overview to the vascular specialist of the current imaging modalities in clinical practice to identify patients with carotid stenosis. Advantages and disadvantages of each technique are outlined. Finally, limitations of assessing luminal stenosis in general are discussed. This article will not cover imaging of carotid atheroma morphology, function and other emerging imaging modalities of assessing plaque risk, which look beyond simple luminal measurements.

Abstract:As a classic cerebral revascularization procedure, carotid endarterectomy (CEA) has been widely used in the treatment of high-grade extracranial carotid artery stenosis. In recent years, carotid artery stenting (CAS) has a trend to replace CEA gradually. A large number of clinical studies have found that in addtion to perioperative complications, carotid restenosis after CEA and CAS also has important influence on the prognosis of patients.
