Abstract:Cerebral conditioning is defined as the mild harmful stimuli applied to the brain before ischemia (preconditioning), during ischemia (perconditioning), immediately, a few hours or days after reperfusion (postconditioning). Various noxious stimuli can precondition, percondition or postcondition the brain, including ischemia, hypoxia or anesthetics to induce tolerance to lethal ischemia/reperfusion injury. The mechanisms underlying cerebral conditioning range from sensors and transducers including extracellular neurotransmitters and receptors, intracellular signals, transcription factors and epigenetic regulation, to effectors involving physiological protection, stem cells, heat shock proteins, Bcl-2, mitochondria, ubiquitin - proteasome pathway and autophagy pathway. Based on the large number of studies conducted in animal models, researchers explore the clinical application of cerebral conditioning. Remote ischemic conditioning is considered the most applicable and clinically relevant form of cerebral conditioning. The translational research also involves development of the biomarkers of conditioning and optimization of organ transplantation. The present review summarized the recent progress in signaling pathways and clinical applications of cerebral conditioning to explore the possibility to develop a potential therapeutic strategy for the prevention and treatment of ischemic stroke.

Abstract:Preconditioning describes a phenomenon whereby a sub-injury inducing insult can protect against a later larger injury. Thus, short-term cerebral ischemia can protect against a prolonged ischemia (ischemic preconditioning). This study examines rats undergoing ischemic preconditioning to test whether preconditioning may cause changes in behavior even though they do not cause an identifiable brain lesion. Rats ad a transient (15 minutes) middle cerebral artery occlusion or a sham occlusion. Forelimb placing and forelimb use asymmetry tests were used to assess behavioral deficits. Brain histology, microglia activation, heat shock protein and ferritin levels were also examined. Ischemic preconditioning did not cause brain infarction, but induced behavioral changes. There were no significant differences between ischemic preconditioning and sham rats in the two behavioral tests at day one. However, the ischemic preconditioning group showed impaired forelimb placing at days 3, 7 and 14 (p < 0.05). That group also had a significant (p < 0.05) behavioral deficit in the forelimb use asymmetry test at days 3 and 7 (but not 14). Our present study demonstrated that a behavioral deficit occurred in ischemic preconditioning. This raises the question of whether induction of protective mechanisms by preconditioning stimuli necessarily involves some form of brain injury, detectable by changes in behavior though not by a lesion. This would be consistent with data suggesting that brain injury can initiate mechanisms potentially favorable to neuroplasticity and neuroprotection.

Abstract:Stroke is one of the leading causes of morbidity and mortality worldwide, and it is increasing in prevalence. The limited therapeutic window and potential severe side effects prevent the widespread clinical application of the venous injection of thrombolytic tissue plasminogen activator and thrombectomy, which are regarded as the only approved treatments for acute ischemic stroke. Triggered by various types of mild stressors or stimuli, ischemic preconditioning (IPreC) induces adaptive endogenous tolerance to ischemia/reperfusion (I/R) injury by activating a multitude cascade of biomolecules, for example, proteins, enzymes, receptors, transcription factors, and others, which eventually lead to transcriptional regulation and epigenetic and genomic reprogramming. During the past 30 years, IPreC has been widely studied to confirm its neuroprotection against subsequent I/R injury, mainly including local ischemic preconditioning (LIPreC), remote ischemic preconditioning (RIPreC), and cross preconditioning. Although LIPreC has a strong neuroprotective effect, the clinical application of IPreC for subsequent cerebral ischemia is difficult. There are two main reasons for the above result: Cerebral ischemia is unpredictable, and LIPreC is also capable of inducing unexpected injury with only minor differences to durations or intensity. RIPreC and pharmacological preconditioning, an easy-to-use and non-invasive therapy, can be performed in a variety of clinical settings and appear to be more suitable for the clinical management of ischemic stroke. Hoping to advance our understanding of IPreC, this review mainly focuses on recent advances in IPreC in stroke management, its challenges, and the potential study directions.

Abstract:Stroke is one of the leading causes of death and long-term serious disability. Current therapeutic strategy is limited to thrombolytic agents, consequently, boosting endogenous neuroprotective mechanisms of the brain to protect itself against harmful stimuli and restore from damages are widely studied. Preconditioned brain to tolerate cerebral ischemia/reperfusion injury could be initiated by several different pharmacological and mechanical strategies, such as ischemic preconditioning, ethanol pharmacological preconditioning and other pre- and post-conditions, such as remote ischemic preconditioning and exercise preconditioning. In this article, we will discuss the major mechanism of ischemia/reperfusion injury and provide an overview of preconditioning in all its various forms, describe the underlying mechanisms and review the recent clinical application of this emerging neuroprotective strategy.

[Brain and spinal cord preconditioning for the protection against ischemic injury]

Abstract:Recent studies have suggested that the brain preconditioning could induce tolerance to ischemia in humans. It has been believed that newly synthesized proteins are required for the acquisition of delayed tolerance in the brain and spinal cord. However, the mechanism other than the synthesis of neuroprotective proteins may also play a pivotal role. Preconditioning may reprogram the response to ischemic injury as seen during hibernation. Preconditioning with hyperbaric oxygen, volatile anesthetics, and xenon seems to be the focus of the attention from the standpoint of the clinical setting. Strong neuroprotection by the preconditioning with isoflurane and xenon is reported in animal experiments and may change the traditional idea of neuroprotection by anesthetics. The discovery that erythropoietin exerts neuroprotective properties has opened new therapeutic avenues. Erythropoietin is induced in the brain by hypoxic preconditioning and by the pharmacological preconditioning. In addition, the intravenous administration of erythropoietin has been shown to be safe and beneficial for acute stroke in humans. Therefore, erythropoietin is now one of the most promising neuroprotective agents. The research in the brain and spinal cord preconditioning will contribute to the elucidation of the mechanism of ischemic injury and to the establishment of new therapies for neuroprotection.

Abstract:A large number of animal experiments have confirmed that ischemic preconditioning can produce a powerful organ protective effect,but the progress and results of the conversion of animal experiments to clinical trials are unsatisfactory.It has great significance for studying of molecular mechanisms of ischemic preconditioning mediated neuroprotection,searching for safe and effective preconditioning induced ways which can be converted to clinical practice,improving the tolerance of nerve tissue ischemia and hypoxia in stroke and surgical patients and achieving a safe and effective neuroprotection.This article reviews the molecular mechanisms of ischemic preconditioning mediated neuroprotection from the aspects of pretreatment of activated receptor,mitochondria,transcription factor,and protein kinase.

Abstract:Ischemic postconditioning significantly attenuates acute hypoxic-ischemic brain injury caused by cardiopulmonary arrest followed by resuscitation， which can be further divided into rapid and delayed postconditioning， as well as in situ and remote postconditioning， according to the time and location of implementation. This review focuses on the molecular mechanisms underlying the neuroprotective effects of ischemic postconditioning， involving the maintenance of redox balance， regulation of autophagy and energy metabolism homeostasis， reduction of systemic inflammatory responses， as well as suppression of PANoptosis， thus reducing hypoxic-ischemic brain injury through multi-pathway and multi-target molecular networks. Ischemic postconditioning is expected to be a new approach for the clinical treatment of acute hypoxic-ischemic encephalopathy， providing new perspectives for the development of multi-targeting drugs.

Abstract:Preconditioning is a phenomenon in which brief episodes of a sublethal insult induce robust protection against subsequent lethal injuries. Preconditioning has been observed in multiple organisms and can occur in the brain as well as other tissues. Extensive animal studies suggest that the brain can be preconditioned to resist acute injuries, such as ischemic stroke, neonatal hypoxia/ischemia, surgical brain injury, trauma, and agents that are used in models of neurodegenerative diseases, such as Parkinson's disease and Alzheimer's disease. Effective preconditioning stimuli are numerous and diverse, ranging from transient ischemia, hypoxia, hyperbaric oxygen, hypothermia and hyperthermia, to exposure to neurotoxins and pharmacological agents. The phenomenon of "cross-tolerance," in which a sublethal stress protects against a different type of injury, suggests that different preconditioning stimuli may confer protection against a wide range of injuries. Research conducted over the past few decades indicates that brain preconditioning is complex, involving multiple effectors such as metabolic inhibition, activation of extra- and intracellular defense mechanisms, a shift in the neuronal excitatory/inhibitory balance, and reduction in inflammatory sequelae. An improved understanding of brain preconditioning should help us identify innovative therapeutic strategies that prevent or at least reduce neuronal damage in susceptible patients. In this review, we focus on the experimental evidence of preconditioning in the brain and systematically survey the models used to develop paradigms for neuroprotection, and then discuss the clinical potential of brain preconditioning.
Induction of cerebral ischemic tolerance influences antioxidant activity of brain tissue

Abstract:Objective To establish ischemic preconditioning (IP) model of inducing cerebral ischemic toler- ance and observe the effect of the IP on SOD and GSH-PX activities of brain Methods Animals were given four different pretreatments 24h before MCAO: shamed operation(SHAM).onlv injected with saline into right internal carotid artery(SI), both carotid artery occlusion(BCAO) or given BCAO with SI(BS) simultaneously , focal cere- bral ischemia was made using the method of thread inserting right middle cerebral artery occlusion (MCAO). Cere- bral infarction volume was observed at 72h after MCAO. SOD and GSH-PX activities of ischemic hemisphere brains were observed at 24h after treatments and at 24h 、 48h after MCAO. Results Cerebral infarction volume was sig- nificantly decreased in BS group ( P 0. 05). SOD and GSH-PX activities of right brain were significantly in- creased in BS group at 24h after pretreatment( P 0. 01). SOD and GSH-PX activities of right brain were signifi- cantly decreased at 24h and at 48h after MCAO. Compared to other groups. SOD and GSH-PX activities in BS group were higher( P 0. 01). Conclusion BS pretreatment can induce cerebral ischemic tolerance. The ischemic preconditioning increases antioxidant activity of brain .which may take an important role in the mechanism of cere- bral ischemic tolerance.
Postconditioning-Induced Neuroprotection, Mechanisms and Applications in Cerebral Ischemia

Abstract:Ischemic postconditioning (PostC) is defined as a series of rapid intermittent interruptions of blood flow at the phase of reperfusion, which produces neuroprotection against cerebral ischemia/reperfusion injury via mobilizing the brain's own endogenous adaptive mechanisms. Now the concept of conventional ischemic PostC has been extended to limb remote ischemic PostC and chemical PostC with hypoxia, volatile anesthetic, CO2, etc. According to the different temporal profile of PostC, it is divided into rapid and delayed PostC. Rapid PostC is applied within a few seconds to minutes after reperfusion, while delayed PostC is applied at a few hours to days after reperfusion. Although the neuroprotective mechanisms of PostC are not completely elucidated, a series of mechanisms have been found to connect with PostC in the central nervous system, such as regulating synaptic signaling, attenuating oxidative stress and inflammation, maintaining mitochondrial integrity, inhibiting endoplasmic reticulum stress, regulating autophagy, activating PI3K/Akt pathway, inhibiting apoptosis, protecting neurovascular unit, etc. Based on these multiple protective mechanisms, PostC has high expectations to translate to the clinic, but a few issues should be resolved such as the time window, risks, efficiency, the impact of age, gender, hypertension, hyperlipidemia and t-PA, and clinical maneuverability. Even so, PostC could soon be at the bedside if the clinical trials are carefully planned.

Abstract:Objective To investigate the effect of focal cerebral ischemic preconditioning on the expression of nuclear factor-кB(NF-кB) and its target gene COX-2,and to explore their role in ischemic tolerance in rats.Methods 36 rats weredivided into 3 groups in which control group received sham surgery(SS) only,and the other two groups received 2 hours of middle cerebral artery occlusion(MCAO) followed by 22 hours of reperfusion with or without 10 minutes of ischemic preconditioning(PC) 3 days before.Expression of NF-кB and COX-2 mRNA were evaluated in each group by immunohistochemistry and RT-PCR.Results Compared with the SS group,there was a lower NF-кB immunoreactivity and COX-2 mRNA level in PC group(P0.01).Conclusions 10 minutes of ischemic PC was strong enough to inhibit activation of NF-кB and transcription of COX-2.Down-regulation of NF-кB/COX-2 pathway might be a key event in the molecular mechanism of cerebral ischemic tolerance induced by focal ischemic preconditioning.

Abstract:Many neuroprotectants have shown effectiveness by reducing infarction and improving neurologic functions in animal models of stroke, but few of these neuroprotectants have been successful in clinical scenario. In clinical trials, pharmacological agents have shown to be either ineffective or have potential adverse effects. Consequently, efforts have been directed toward understanding and enhancing the endogenous protective mechanisms by which the brain protects itself against noxious stimuli in an attempt to recover from the damage encountered. Preconditioning-induced ischemic tolerance is an effective approach to understand how the brain protects itself. In this chapter, we summarize the development of preconditioning followed by discussion of various stimuli that can induce brain preconditioning and the downstream signaling pathways involved in preconditioning-induced protection. Specifically, we discuss the potential clinical application of preconditioning for brain injuries such as stroke.

Abstract:Background Penumbra is the major target for cerebral ischemia treatment,but effective and secure strategies of non-recanalization therapeutic have been without breakthrough for many years.However the recent discovery that isoflurane preconditioning can protect penumbra provides new ideas of prevention on cerebral ischemia. Objective To sum up the effect and mechanisms of isoflurane preconditioning-inducéd cerebral penumbral protection. Content First the position and complicacy of penumbra in ischemic diseases have been introduced; then the current situation of effect and the possible mechanisms of isoflurane preconditioning-induced cerebral penumbral protection has been described and analyzed respectively. Trend The involvement of toll like receptor 4 signaling pathway and other protective mechanisms still need further exploration,which can both provide more powerful evidences for its clinical application and contribute to excavate new direction for ischemic treatment.

Abstract:Exercise preconditioning has attracted extensive attention to induce endogenous neuroprotection and has become the hotspot in neurotherapy. The training exercise is given multiple times before cerebral ischemia, effectively inducing ischemic tolerance and alleviating secondary brain damage post-stroke. Compared with other preconditioning methods, the main advantages of exercise include easy clinical operation and being readily accepted by patients. However, the specific mechanism behind exercise preconditioning to ameliorate brain injury is complex. It involves multi-pathway and multi-target regulation, including regulation of inflammatory response, oxidative stress, apoptosis inhibition, and neurogenesis promotion. The current review summarizes the recent studies on the mechanism of neuroprotection induced by exercise, providing the theoretical basis of applying exercise therapy to prevent and treat ischemic stroke. In addition, we highlight the various limitations and future challenges of translational medicine from fundamental study to clinical application.

Ischemic Preconditioning Modulates the Peripheral Innate Immune System to Promote Anti-Inflammatory and Protective Responses in Mice Subjected to Focal Cerebral Ischemia

Abstract:The development of tolerance triggered by a sublethal ischemic episode (preconditioning, PC) involves a complex crosstalk between neurons, astrocytes and microglia, although the role of the peripheral immune system in this context is largely unexplored. Here, we report that severe cerebral ischemia caused by transient middle cerebral artery occlusion (MCAo) in adult male mice elevates blood counts of inflammatory neutrophils and monocytes, and plasma levels of miRNA-329-5p. These inflammatory responses are prevented by ischemic PC induced by 15 min MCAo, 72h before the severe insult (1h MCAo). As compared with sham-operated animals, mice subjected to either ischemic PC, MCAo or a combination of both (PC+MCAo) display spleen contraction. However, protein levels of Ym1 (a marker of polarization of myeloid cells towards M2/N2 protective phenotypes) are elevated only in spleen from the experimental groups PC and PC+MCAo, but not MCAo. Conversely, Ym1 protein levels only increase in circulating leukocytes from mice subjected to 1h MCAo, but not in preconditioned animals, which is coincident with a dramatic elevation of Ym1 expression in the ipsilateral cortex. By immunofluorescence analysis, we observe that expression of Ym1 occurs in amoeboid-shaped myeloid cells, mainly representing inflammatory monocytes/macrophages and neutrophils. As a result of its immune-regulatory functions, ischemic PC prevents elevation of mRNA levels of the pro-inflammatory cytokine interleukin (IL)-1β in the ipsilateral cortex, while not affecting IL-10 mRNA increase induced by MCAo. Overall, the elevated anti-inflammatory/pro-inflammatory ratio observed in the brain of mice pre-exposed to PC is associated with reduced brain infarct volume and ischemic edema, and with amelioration of functional outcome. These findings reaffirm the crucial and dualistic role of the innate immune system in ischemic stroke pathobiology, extending these concepts to the context of ischemic tolerance and underscoring their relevance for the identification of novel therapeutic targets for effective stroke treatment.

Abstract:Background Preconditioning to ischemia is a phenomenon whereby a brief episode of sublethal ischemia and other nonlethal stressors produce protection against a subsequent detrimental ischemic insult. As mitochondrial dysfunction is related to necrotic and apoptotic neuronal death after cerebral ischemia, the authors examined if ischemic preconditioning is capable of inducing mitochondrial tolerance. Methods Forebrain ischemia was induced by bilateral common carotid artery occlusion with simultaneous hypotension for 8 min in Wistar rats (275-300 g). A 3-min ischemic episode performed 48 h before the 8-min ischemia was used for preconditioning. The extents of hippocampal CA1 neuronal damage were evaluated 7 days after reperfusion by neuro-specific nuclear protein immunostaining. Brain mitochondria were isolated 48 h after animals were subjected to the sham operation or the 3-min conditioning ischemia. Loss of cytochrome c from mitochondria after cerebral ischemia in vivo and after exposure of brain mitochondria to calcium in vitro was used as an indication of mitochondrial dysfunction. Results Results showed that ischemic preconditioning induced by a 3-min ischemic episode dramatically reduced the loss of hippocampal CA1 neurons resulting from a subsequent 8-min ischemia 7 days after reperfusion, and this protection was associated with a preservation of mitochondrial cytochrome c as examined after early reperfusion. Exposure of isolated brain mitochondria to calcium produced a dose-dependent increase in cytochrome c release either at 30 degrees C or at 37 degrees C. Compared with those animals receiving only sham operation, cytochrome c release caused by 100 microm calcium was significantly reduced in conditioned animals. Conclusion Regarding the importance of mitochondrial dysfunction in mediating ischemic neuronal death, the above results indicate that mitochondria may serve as end-effecting organelles to ischemic preconditioning.

Effect of Ischemic Preconditioning on the Expression of Glial Fibrillary Acidic Protein after Ischemia-Reperfusion in Rats

Abstract:Objective To observe the effect of focal cerebral ischemic preconditioning on the expression of glial fibrillary acidic protein (GFAP) and to investigate the significance of astrocyte activation in cerebral ischemic tolerance.Methods Thirty-six healthy male SpragueDawley rats were randomly divided into reischenmic,ischemic and control groups (n = 12 in each group) after ischemic preconditioning.The former two groups received 10 minutes middle cerebral artery occlusion (MCAO) preconditioning or sham operation 3 days before the 2-hour MCAO.The rats were killed 24 hours after the second MCAO.The control group only receivedthe two sham operations with an interval of three days.The infarct volume,histopathological changes,and GFAP expression in each group were compared.Results The infarct volume after ischemic preconditioning in the reischenmic and ischemic groups was 136.85 ± 14.51 mm3and 281.37 ± 29.93 mm3 respectively.The former was significantly reduced 53.15%compared to the latter (P =0.007).At the same time,neuronal degeneration and necrosis was reduced significantly,and GFAP expression was upregulated significantly (the mean absorbance for immunohistochemical staining in both groups was 102.66 ± 8.39 and 86.28 ± 6.19respectively,P = 0.009) after ischemic preconditioning in the reischemic group.Conclusions Focal ischemic preconditioning may induce brain ischemic tolerance and promote GFAP expression.The activation of astrocytes may be one of the mechanisms of cerebral ischemic tolerance.
