Analysis of the accurate staging diagnosis and standardized treatment of reported syphilis in Dongguan general hospitals

Abstract:Objective To analyze the accurate diagnosis and standardized treatment of syphilis in Dongguan general hospitals,and to provide the basis for improvement of syphilis prevention and control.Methods Syphilis cases were collected from the national disease prevention and control information system covering 20 general hospitals from July 1st,2015 to June 30th,2016,with at most 50 cases from each by random sampling.The cases were analyzed based on the national syphilis diagnostic criteria and guidelines for syphilis treatment.Results 893 cases of syphilis were analyzed with 7.91％ (71) of primary syphilis,10.80％ (97) of secondary syphilis,2.23％ (20) of tertiary syphilis,1.46％ (13) of congenital syphilis,and 77.62％ (697) of recessive syphilis.The rates were 90.03％ for syphilis diagnosis accuracy,65.85％ for staging accuracy,and 71.89％ for standardized treatment.There was significant difference between the diagnosis and staging accuracy of syphilis syndrome (x2=23.772,P＜0.001;x2=23.309,P＜0.001),with the lowest rate of fetal syphilis diagnosis and the highest rate of tertiary syphilis.Among the general hospitals,the dermatology & STD division had higher rates of STD diagnosis accuracy (93.45％ vs 86.45％),staging accuracy (72.95％ vs 59.31％) and standardized treatment (91.27％ vs 51.49％) than other divisions.The third-class hospitals had higher rates (87.78％ vs 65.29％) than the second-class hospitals.Conclusion The comprehensive diagnosis rate of syphilis in Dongguan general hospitals are higher.However,the rates of staging accuracy and the standardized treatment are lower.We should strengthen the training and supervision of the medical staff,and improve the referral system to upgrade the rates of syphilis staging accuracy and standardized treatment.
