Expression of Vascular Endothelial Growth Factor and Its Clinical Significance in Middle Ear Cholesteatoma

Abstract:Objective This study aimed to analyze the localization and distribution of vascular endothelial growth factor (VEGF) in middle ear cholesteatoma in comparison with normal middle ear mucosa and auditory skin and the relationship between the angiogenesis induced by VEGF and the destructive behavior of middle ear cholesteatoma. Methods The expression of VEGF was studied by immunohistochemistry. The amount of vessels (collagen type Ⅳ staining) was determined by an automatic imaging analyzing system. Results A strongly positive immunoreactivity for VEGF could be detected in basal keratinocytes of the epithelium of cholesteatoma. Definite differences also became obvious between cholesteatoma perimatrix and the subepithelial connective tissue of normal middle ear mucosa and auditory skin concerning VEGF immunoreactivity. And the mean number of vessels increased with the degree of inflammation within the perimatrix (GradesⅠ-Ⅲ) from 9.3±3.6 vessels/mm2 (Grade Ⅰ) to 21.1±3.4 vessels/mm2 (Grade Ⅱ) and 32.5±9.5 vessels/mm2 (Grade Ⅲ) (p0.05). Conclusions Angiogenesis induced by VEGF can support the sustained migration of keratinocytes into the middle ear cavity. And it is a pivotal factor in the destructive behavior of middle ear cholesteatoma.
