Abstract:About 20%-40% of early stage oral squamous cell carcinoma (OSCC) patients have occult neck metastasis, the management of the clinically negative neck (cN0) remains controversial.Recently, clinical options for addressing the neck include elective neck dissection at the time of the excision of the primary tumor, watchful waiting with therapeutic neck dissection for nodal relapse or elective adjuvant radiotherapy.Because of the lack of means to detect occult nodal metastasis accurately, patients may face with the risk of over-treatment or under-treatment.Owing to these controversies,it is important to identify factors that are capable of guiding optimal neck management. This paper reviewed the studies which focused on pathologic predictors to stratify the risk of neck metastasis in patients with early stage OSCC.
